Enhancing Communication and Consent Using 3-D Printed Models With a Pediatric Dental Patient Presenting With Odontogenic Keratocysts Associated With Gorlin Syndrome: A Case Report.

AIMS: This case report aimed to describe the use of a patient-specific 3D printed model to support communication and informed consent in a pediatric patient diagnosed with Gorlin syndrome, a rare condition frequently associated with multiple odontogenic keratocysts. METHODS AND RESULTS: A 9-year-old girl with Gorlin syndrome presented with swelling of the maxilla and mandible. Imaging revealed two large cystic lesions. Conventional explanations using radiographs and CBCT slices failed to ensure adequate parental understanding, partly due to language barriers. A digital workflow was developed …

Did Bioethics Matter? A History of Autonomy, Consent, and Intersex Genital Surgery.

This article argues that the rise of bioethics in the post-WWII era and the emergence of the legal doctrine of informed consent in the late 1950s should have had a greater impact on patients with intersex traits (atypical sex development) than they did, given their emphasis on respect for autonomy and beneficence toward patients. Instead, these progressive trends collided with a turn in intersex management toward infants, who were unable to provide autonomous consent about their medical care. Patient autonomy took a back seat as parents heeded physicians' advice in an environment even more hie…

Enhancing comprehension of consent for research in older patients with psychosis: a randomized study of a novel consent procedure.

OBJECTIVE: The ability of individuals with schizophrenia to provide informed consent for research has become the focus of public debate. The authors examined whether a novel consent procedure improved the comprehension of consent for older patients with psychosis. METHOD: Fifty outpatients with schizophrenia or other psychotic disorders and 19 normal comparison subjects aged 40-80 were randomly assigned to groups given either a routine or an enhanced consent procedure. The latter utilized a computerized presentation that included sequential bullet points and summaries of key information. A po…

Sequential improvements in organ procurement increase the organ donation rate.

PURPOSE: Organ demand exceeds availability of transplantable organs. Organ procurement continues to suffer from failures to identify potential donors, inability to obtain consent for donation, as well as failures to retrieve certain organs as donor demographics change. The purpose of this article is to propose how sequentially introduced measures can increase organ donation rates as well as improve organ procurement. METHODS: We analysed the effect of stepwise improvements in the organ procurement process patients in a university-based surgical intensive care unit over a 20-year period. We re

Psychiatric consultation outcomes in deep brain stimulation candidacy: clinical and psychosocial correlates from a retrospective cohort.

BACKGROUND/OBJECTIVES: Preoperative psychiatric assessment in deep brain stimulation (DBS) candidacy extends beyond contraindication screening to include informed consent capacity and psychosocial preparedness. This study examined factors associated with psychiatric consultation outcomes in DBS candidates. METHODS: This retrospective single-centre study reviewed 363 patients referred for preoperative psychiatric consultation at Etlik City Hospital between September 2022 and January 2026. Outcomes were classified as "suitable," "conditionally suitable," or "not suitable." Associations were ass…

Shortcomings of protocols of drug trials in relation to sponsorship as identified by Research Ethics Committees: analysis of comments raised during ethical review.

BACKGROUND: Submission of study protocols to research ethics committees (RECs) constitutes one of the earliest stages at which planned trials are documented in detail. Previous studies have investigated the amendments requested from researchers by RECs, but the type of issues raised during REC review have not been compared by sponsor type. The objective of this study was to identify recurring shortcomings in protocols of drug trials based on REC comments and to assess whether these were more common among industry-sponsored or non-industry trials. METHODS: Retrospective analysis of 226 protoco

Whether to Waive Parental Permission in HIV Prevention Research Among Adolescents: Ethical and Legal Considerations.

Critical ethical questions arise concerning whether studies among adolescents of new behavioral and biomedical HIV preventive interventions such as Pre-Exposure Prophylaxis (PrEP) should obtain parental permission. This paper examines the relevant regulations and ethical guidance concerning waivers of parental permission, and arguments for and against such waivers. Opponents of such waivers may argue that adolescent decision-making is "too immature" and that parents always have rights to decide how to protect their children. Yet requiring parental permission may put adolescents at risk, and/or

Could the organ shortage ever be met?

The organ shortage is commonly presented as having a clear solution, increase the number of organs donated and the problem will be solved. In the light of the Northern Ireland Assembly's consultation on moving to an opt-out organ donor register this article focuses on the social factors and complexities which impact strongly on both the supply of, and demand for, transplantable organs. Judging by the experience of other countries presumed consent systems may or may not increase donations but have not met demand. Donation rates have risen considerably in all parts of the UK recently but there i

Legal aspects of involuntary sterilization.

The foregoing discussion was intended as a review of the pertinent literature of involuntary sterilization. It is by no means all-inclusive (the number of cases precludes an exhaustive compendium) but does include those cases that, in the authors' opinion, are most representative of the contemporary attitude of the courts. The discussion has been confined primarily to the legal aspects of involuntary sterilization. We have omitted any substantial discussion of the moral and ethical aspects of this complex topic but acknowledge the intimate role these issues may play in any decision to undertak

Rapid Ethical Appraisal: A tool to design a contextualized consent process for a genetic study of podoconiosis in Ethiopia.

BACKGROUND: Obtaining valid informed consent from research participants in low-income settings can be difficult, partly due to participants' limited familiarity with scientific research concepts. The situation is complicated when conducting genomic research on a stigmatizing familial disease. METHODS: We used Rapid Ethical Appraisal as a tool to assess local factors that were barriers to obtaining valid informed consent prior to conducting a genetic study of podoconiosis (non-filarial elephantiasis) in two Zones of Ethiopia. The tool included in-depth interviews and focus group discussions wi…

Implementation of a culturally competent APOL1 genetic testing programme into living donor evaluation: A two-site, non-randomised, pre-post trial design.

Given cultural concerns about genetic testing among people of African ancestry, protecting LD candidates' safety is essential to improve informed decisions about donating. Influence of mood state on capacity to consent to research in patients with bipolar disorder.

OBJECTIVES: Experts have debated the ability of patients with bipolar illness to consent to research participation when manic. We compared 25 euthymic bipolar and 26 manic bipolar patients in their understanding of three hypothetical studies of varying complexity. METHODS: Hypothetical studies were presented as written informed consent forms, which were reread up to three times. Questions included assessment of hypothetical study purposes, procedures, risks and benefits, and voluntariness of participation. RESULTS: After adjusting for educational level, subjects with mania had lower mean und…

Intrathecal haematoma after an epidural blood patch.

SummaryThis case demonstrates a rare but potentially serious complication of an epidural blood patch (EBP). Intrathecal haematoma is a rare complication after an EBP and anaesthetists should be able to promptly identify and manage these patients to prevent long-term damage. This case also highlights the importance of informed consent according to the principles of the Montgomery judgement/principle of consent.

Assessing the Ethical Considerations in the Era of Advanced Prenatal Testing and Genetic Counselling - Clinicians' Perspective.

Prenatal testing has become an increasingly common practice in modern healthcare, offering parents valuable insights into the health and development of their unborn child. However, the rapid advancements in this field have also introduced a host of ethical dilemmas that demand careful consideration. Our objective was to explore the ethical considerations that the clinicians face in prenatal testing and genetic counseling, focusing on issues like informed consent, autonomy, the routinization of testing, disability rights, and implications for practice. A qualitative synthesis of the recent lite…
