Treatment Delays in Nonsyndromic Craniosynostosis: A 30-Year Retrospective Case-Control Analysis of the Impact of Socioeconomic and Family Status on Access to Care.

BACKGROUND: Patients with nonsyndromic craniosynostosis (NSC) generally undergo corrective surgery before 1 year of age to the mitigate morbidities and risks of delayed repair. The cohort of patients who receive primary corrective surgery after 1 year and factors associated with their gaps to care is poorly characterized in literature. METHODS: A nested case-control study was conducted for NSC patients who underwent primary corrective surgery at our institution and affiliates between 1992 and 2022. Patients whose surgery occurred after 1 year of age were identified and matched 1:1 by surgical

Hospital survival of aortic dissection in children.

BACKGROUND: Thoracic aortic dissection (AD) is a rare occurrence in childhood and mostly associated with connective tissue disorders or congenital abnormalities. We examined the characteristics associated with AD and predictors of survival. METHODS: The Kids' Inpatient Database (1997-2009) was used to identify thoracic ADs occurring in patients <20-y-old. Clinical characteristics, as well as determinants of survival, were analyzed using standard statistical methods. RESULTS: One hundred sixty-eight cases of thoracic AD were identified during the study period. Overall survival was 83%. Averag

Patient- and Provider-Perceived Barriers to Reconstructive Surgery for Patients With Upper Limb Spasticity.

PURPOSE: The benefits of upper-extremity reconstructive surgery for patients with spastic deformities are well documented, but a small portion of eligible patients undergo surgery. We sought to determine perceptions of upper-extremity reconstructive surgery among brain injury patients and nonsurgical providers to identify potential barriers to surgical evaluation. METHODS: Electronic medical records at a referral center were reviewed for patients diagnosed with upper limb spasticity following brain injury. A patient-specific survey was distributed by email to all eligible patients. An anonymo

Surgical versus Nonsurgical Management of Postmastectomy Lymphedema: A Prospective Quality of Life Investigation.

BACKGROUND: Postmastectomy secondary lymphedema can cause substantial morbidity. However, few studies have investigated longitudinal quality of life (QoL) outcomes in patients with postmastectomy lymphedema, especially with regard to surgical versus nonoperative management. This study prospectively investigated QoL in surgically versus nonsurgically managed patients with postmastectomy upper extremity lymphedema. METHODS: This was a longitudinal cohort study of breast cancer-related lymphedema patients at a single institution, between February 2017 and January 2020. Lymphedema Quality of Life

Socioeconomic and sociodemographic factors and utilization of revascularization surgery in moyamoya disease: a systematic review and meta-analysis.

INTRODUCTION: Moyamoya disease (MMD) is a rare cerebrovascular disorder in which timely surgical revascularization can prevent stroke. Socioeconomic and sociodemographic factors may influence utilization of revascularization. This systematic review and meta-analysis aimed to quantify associations of household income, insurance type, race or ethnicity, and sex with receipt of revascularization surgery among patients with MMD. METHODS: A search of PubMed, EMBASE, Europe PMC and Scopus was performed on 11 August 2026 following the PRISMA 2020 statement. Studies reporting socioeconomic factors (h

Interhospital Variability in Treatment of Pediatric Medial Epicondyle Fractures of the Humerus.

BACKGROUND: Fractures of the medial humeral epicondyle are common, yet there is no consensus on the most effective treatment method. Both casting alone and open reduction with internal fixation have been shown to promote reliable healing, but there is limited evidence supporting the superiority of either approach. The aim of this study is to assess the variation in treatment approaches for MEF and to identify which patient characteristics or hospital factors influence a surgeon's decision to perform surgery. METHODS: We conducted a retrospective cohort study to evaluate treatment variations b

Effects of time to chemoradiation on high-grade gliomas from the Buenos Aires Metropolitan Area.

High-Grade Gliomas (HGG) are the most frequent brain tumor in adults. The gold standard of clinical care recommends beginning chemoradiation within 6 weeks of surgery. Disparities in access to healthcare in Argentina are notorious, often leading to treatment delays. We conducted this retrospective study to evaluate if time to chemoradiation after surgery is correlated with progression-free survival (PFS). Our study included clinical cases with a histological diagnosis of Glioblastoma (GBM), Anaplastic Astrocytoma (AA) or High-Grade Glioma (HGG) in patients over 18 years of age from 2014 to 202

Mapping and crosswalk of patient-reported pain measures after surgery in the Michigan surgical quality collaborative registry.

OBJECTIVE: Patient-reported pain is a crucial measure for postoperative recovery, but the correlation between numeric and categorical scales remains unclear. The Michigan Surgical Quality Collaborative (MSQC) used categorical rating scales (CRS) from 2017 to 2020 and 2022, switching to numeric rating scales (NRS) in 2021, offering a chance to compare both. METHODS: We analyzed surgical patients ≥18 years of age across 70 MSQC hospitals. We compared the 2020 CRS regarding pain in the week after surgery with 2021 NRS surgical site (primary outcome) and overall body (secondary outcome) pain ques

To Explant or not to Explant Neural Implants: an Empirical Study into Deliberations of Dutch Research Ethics Committees.

BACKGROUND: Scaphoid fractures treated non-operatively and operatively may be complicated by nonunion. QUESTIONS/PURPOSES: We sought to test the primary hypothesis that the incidence density of scaphoid fracture treatment is higher than previously estimated, to determine the frequency and risk factors for nonunion treatment, and to determine whether the frequency of surgical treatment increased over time. METHODS: The MarketScan® database was queried for all records of treatment (casting and surgery) for closed scaphoid fractures over a 6-year period. We examined subsequent claims to determi

Opioid Dependency After Craniomaxillofacial Trauma Surgery.

OBJECTIVE: The opioid crisis has led to significant morbidity and mortality. A component of this is attributable to overdoses on prescribed opioids. A prior study estimated that 39% of opioid prescriptions after craniomaxillofacial (CMF) trauma were potentially inappropriate prescriptions. The goal of this study was to identify factors associated with potentially inappropriate and prolonged opioid prescriptions in the postoperative CMF trauma patient population. STUDY DESIGN: Retrospective cohort. SETTING: Merative™ MarketScan® Commercial & Multi-State Medicaid Databases (2015-2022). METHOD

Relationship between uterine fibroids and risk of cancers: Population-based retrospective cohort study.

OBJECTIVE: Numerous studies have assessed the relationship between estrogen and cancer, and an association between uterine fibroids and an increased risk of endometrial cancer and thyroid cancer has been found. However, previous studies are limited by small sample sizes and lack of information on patient characteristics. This population-based retrospective cohort study aimed to confirm the association between uterine fibroids and cancer risk. METHODS: This population-based retrospective cohort study used insurance information recorded in the Korea's Health Insurance Review and Assessment Serv

Healthcare disparities in adult and pediatric spinal deformity: a state of the art review.

PURPOSE: A health disparity refers to a greater disease burden or negative health outcomes influenced by social, economic, and environmental factors. Numerous studies in the surgical literature show that social drivers of health affect health outcomes. Similar disparities may affect treatment and outcomes of spine deformity patients. This study aims to review existing literature on healthcare disparities in patients with spinal deformities. METHODS: A comprehensive search of articles from 1/2002 to 7/2024 in two databases included keywords and Medical Subject Heading terms: "health disparitie

Financial Toxicity Following Surgical Treatment for Breast Cancer: A Cross-sectional Pilot Study.

BACKGROUND: Despite growing national attention, there is limited understanding of the patient- and treatment-level characteristics related to treatment cost-associated distress ("financial toxicity") in breast cancer patients. Our aim is to identify risk factors for financial toxicity amongst breast cancer patients undergoing surgical treatment. METHODS: This is a single-institution cross-sectional survey of adult female breast cancer patients who underwent lumpectomy or mastectomy between January 2018 and June 2019. Financial toxicity was measured via the 11-item comprehensive score for fina

National Trends in the Diagnosis of CRPS after Open and Endoscopic Carpal Tunnel Release.

Background  Complex regional pain syndrome (CRPS) occurs in 2 to 8% of patients that receive open or endoscopic carpal tunnel release (CTR). Because CRPS is difficult to treat after onset, identifying risk factors can inform prevention. We determined the incidence of CRPS following open and endoscopic CTR using a national claims database. We also examined whether psychosocial conditions were associated with CRPS after CTR. Methods  We accessed insurance claims using diagnostic and procedural codes. We calculated the incidence of CRPS following open carpal tunnel release and endoscopic carpal t

Trends in the "Off-Label" Use of GnRH Agonists Among Pediatric Patients in the United States.

BACKGROUND: Gonadotropin-releasing hormone (GnRH) agonists are FDA approved for the treatment of precocious puberty. The therapy consists of histrelin acetate (Supprelin), a surgically implanted device, or Lupron injections. In recent years, the use of these agents has been extended to include the off-label treatment of children with normally timed puberty. Trends in the off-label use of GnRH agonists in children across the U.S. have not been previously described in the literature. METHODS: We analyzed data on the use of Supprelin and Lupron reported to the Pediatric Health Information System

National medical expenditures associated with pediatric surgical care in the United States.

PURPOSE: Surgical care accounts for a large proportion of U.S. healthcare costs, yet comprehensive data on pediatric surgical expenditures remain limited. This study provides nationally representative estimates of medical expenditures associated with pediatric surgery in the United States. METHODS: We performed cross-sectional analysis of children ages 0-18 years participating in the 2017-2022 Medical Expenditure Panel Survey (n = 39,926). Children were considered to have undergone surgery if they had at least one medical encounter with the primary reason being an "operation or surgical proce
