Genomic dissection of the 1994 Cronobacter sakazakii outbreak in a French neonatal intensive care unit.

BACKGROUND: Cronobacter sakazakii is a member of the genus Cronobacter that has frequently been isolated from powdered infant formula (PIF) and linked with rare but fatal neonatal infections such as meningitis and necrotising enterocolitis. The Cronobacter MLST scheme has reported over 400 sequence types and 42 clonal complexes; however C. sakazakii clonal complex 4 (CC4) has been linked strongly with neonatal infections, especially meningitis. There have been a number of reported Cronobacter outbreaks over the last three decades. The largest outbreak of C. sakazakii was in a neonatal intensiv

[Laser treatment for retinopathy of prematurity in neonatal intensive care units. Premature Eye Rescue Program].

INTRODUCTION: Retinopathy of prematurity is a leading cause of childhood blindness around the world. AIM: The Department of Ophthalmology at the Semmelweis University and the Peter Cerny Neonatal Emergency and Ambulance Service started an innovative Premature Eye Rescue Program to reduce the non-essential transport of premature babies suffering from retinopathy of prematurity. METHOD: During the first 5 years 186 eyes of 93 premature babies were treated at the bedside with stage 3 retinopathy of prematurity in the primary hospitals. RESULTS: In this first 5-years period the authors reduced

Late-onset Streptococcus pasteurianus sepsis in a preterm baby in a neonatal intensive care unit.

Apnea, cyanosis, lethargy and prolongation in capillary filling time developed on the postnatal 37(th) day in a preterm baby who was born at the 30(th) gestational week with a birth weight of 1 300 g. Acute phase reactants and immature/total neutrophil count ratio were found to be high. The patient who was diagnosed with sepsis was successfully treated with meropenem which was started empirically. In his blood culture Streptococcus pasteurianus grew. S. pasteurianus is in the subgroup of streptococcus bovis which is one of the D group streptococci and its previous name is S. bovis type II/2. Operating Room Within the Neonatal Intensive Care Unit--Experience of a Medical Center in Taiwan.

BACKGROUND: Most neonates who reside in the neonatal intensive care unit (NICU) and require surgery are transferred to the operating room (OR) or undergo bedside surgery. However, critically ill neonates who are transferred often encounter the risk of complications. An OR in our NICU was therefore launched in 2009. This study was to appraise the surgeries performed in the NICU OR and compare results with the traditional main OR outside the NICU. METHODS: This was a retrospective study in the NICU of a tertiary center. Retrospective chart review was conducted for all neonates who underwent sur

A 4-year surveillance of device-associated nosocomial infections in a neonatal intensive care unit.

BACKGROUND: The aim of this study was to determine the rate of health care-associated infection (HC-AI) and device-associated health care-associated infections (DA-HAIs), and distribution of causative microorganisms and etiologic factors responsible for these infections in a neonatal intensive care unit (NICU) of a state hospital in southeastern Turkey. METHODS: A laboratory-based, active, prospective nosocomial infection surveillance study was performed in NICUs from January 2008 to December 2011. The rates of HC-AIs were determined on a daily basis. The findings were evaluated by applying t

Radiation exposure in extremely low birth weight infants during their neonatal intensive care unit stay.

BACKGROUND: Extremely low birth weight (ELBW <1000 g) infants may have increased sensitivity to radiation exposure. Our objective was to estimate the radiation exposure in survivors of ELBW infants during their neonatal intensive care unit (NICU) stay. METHODS: In this retrospective cohort study, medical records of all ELBW infants who had been admitted to our NICU between May 1999 and October 2009 were reviewed. The infants' total entrance skin exposure [ESE in micro-Gray (μGy)] was estimated. RESULTS: Among 450 survivors, the mean gestational age (GA) was 26.3±2.1 weeks, and the mean birth

Early prediction of cerebral palsy after neonatal intensive care using motor development trajectories in infancy.

UNLABELLED: Neonatal intensive care unit (NICU) patients are at high risk for developmental disabilities such as cerebral palsy (CP). Early identification of CP is essential to effective rehabilitation, but diagnosis is often delayed, especially in preterm infants. We hypothesized that through the longitudinal evaluation of motor trajectories in the NICU follow-up clinic, we could distinguish infants who develop CP by 3 years of age. STUDY DESIGN AND SUBJECTS: This was a retrospective study of 606 patients in the NICU Follow-up Clinic at Vanderbilt University with birth weight < 1500g or a di

Reducing hazardous chemical exposures in the neonatal intensive care unit: a new role for nurses.

A wide range of toxic chemicals have been found in the umbilical cord blood of newborns, indicating the potential for health risks from chemical exposure that begin in utero. The neonatal intensive care unit (NICU) setting may also have potential chemical exposures that create health risks. Given the extreme vulnerability of this patient population, it is critical to minimize unnecessary hazardous chemicals. Neonatal intensive care unit nurses have an important role to play in making the NICU as safe as possible for their patients and themselves. This article will focus on the human health eff

Assessment and intervention for dysphagia in infants and children: beyond the neonatal intensive care unit.

Over the last 10 years, the assessment and intervention for feeding and swallowing problems in infants and children have attracted increased attention on a national and international level. Increases in the population of children with dysphagia are due, in large part, to advances in medical and surgical management of at-risk term infants, improved medical support for viability of younger and smaller preterm infants, and increases in the number of children on the autism spectrum. Because of legislative initiatives, settings in which children are seen for assessment and intervention have shifted
