Successful Conservative Treatment of a Cholecystoduodenal Fistula Caused by a Cytomegalovirus-associated Duodenal Ulcer.

An 87-year-old woman on oral prednisolone was diagnosed with a cholecystoduodenal fistula (CDF) caused by a cytomegalovirus-associated duodenal ulcer (DU) and was managed conservatively. A CDF caused by a DU is extremely rare. Although surgical repair is recommended for the treatment of a CDF caused by cholecystolithiasis, appropriate treatment for CDF caused by a DU remains controversial. This case report of a CDF caused by a DU suggests that conservative treatment is feasible in the absence of DU-associated complications, such as an untreatable hemorrhage or obstruction; this finding is comp

Surgical management of complicated diverticulitis.

The majority of patients with acute diverticulitis can be managed medically. Some will have a complication of diverticulitis such as free perforation with peritonitis, abscess formation, obstruction, or fistula formation. Perhaps even a larger number will develop recurrent diverticulitis, which is associated with an increased rate of complications. Although the preoperative diagnosis of these problems may be obvious in many patients, elderly or steroid-treated patients may have few manifestations of significant intra-abdominal disease. Of extreme importance in the management of these complicat

Complications of diverticular disease: a review of the barium enema findings.

Complications of diverticular disease are among the most common causes of all acute hospital admissions. The barium enema is the most important investigation in these patients, both for establishing the diagnosis and planning their management. However, few comprehensive reviews have described the radiological appearances of the complications of diverticular disease. We describe the radiological features and emphasize the diagnostic difficulties that may occur.

Biliary enteric fistula.

From 1932 to 1978, 105 patients with biliary enteric fistulas are reported upon from The New York Hospital-Cornell Medical Center. During this 46 year period, 11,808 patients were operated upon for nonmalignant biliary tract disease, representing an incidence of biliary enteric fistulas of 0.9 per cent. The most common location of the fistula was cholecystoduodenal in 77 per cent and cholecystocolic in 15 per cent. Ninety-eight of the 105 patients underwent 109 surgical procedures for symptoms and complications associated with the fistula. The operative mortality was 6 per cent. The recommende

Computed tomography three-dimensional reconstruction in the diagnosis of bleeding small intestinal polyps: A case report.

BACKGROUND: Computed tomography (CT) small bowel three-dimensional (3D) reconstruction is a powerful tool for the diagnosis of small bowel disease and can clearly show the intestinal lumen and wall as well as the outside structure of the wall. The horizontal axis position can show the best adjacent intestinal tube and the lesion between the intestinal tubes, while the coronal position can show the overall view of the small bowel. The ileal end of the localization of the display of excellent, and easy to quantitative measurement of the affected intestinal segments, the sagittal position for the

Blood flow response in small intestinal loops at different depths during negative pressure wound therapy of the open abdomen.

High closure rates of the open abdomen have been reported following negative pressure wound therapy (NPWT). However, the method has occasionally been associated with increased development of intestinal fistulae. We have previously shown that the application of NPWT to the open abdomen causes a decrease in microvascular blood flow in the small intestinal loop and the omentum adjacent to the visceral protective layer of the dressing. In this study we investigate whether the negative pressure affects only small intestinal loops lying directly below the dressing or if it also affects small intesti…

Crohn's disease of the appendix, manifested as acute appendicitis with postoperative fistula.

Two cases are reported of Crohn's disease localized to the appendix and manifested as acute appendicitis; after appendectomy a fistula developed. In none of 18 patients with Crohn's disease reported by other authors, where the appendix was the primary site, did a fistula develop postoperatively. When the appendix is the primary site of Crohn's disease the presence of mild inflammation of adjacent organs such as the terminal ileum may be mistakenly attributed to ordinary appendicitis. If, at exploratory laparotomy performed on a tentative diagnosis of appendicitis, Crohn's disease is suspected

[Emergency surgical treatment of spontaneous internal fistulas of the digestive tract].

The Authors report 16 cases of gastrointestinal fistulas which have been observed during the last 5 years in the Emergency Surgery Division of Milan's Fatebenefratelli Hospital. We have considered only the cases of fistulas between two hollow abdominal organs and which have occurred spontaneously, following local pathology (cholelithiasis, neoplasms, abscesses etc.) and not from iatrogenic causes. Of these 16 patients, all surgically treated, 9 were female and 7 male, their age ranging from 24 to 77 years (average: 58 years). The medium post-operative period was of 11 days; the overall mortali…

[Malignant tumorous urogenital fistula. Apropos of 19 cases].

Malignant tumors were responsible for 19 cases of urogenital fistula, including 10 ureterovaginal and 9 vesicovaginal fistulae, treated between 1974 and 1982. Details of the cases are reviewed and the therapeutic attitude to adopt towards urogenital fistulae of malignant tumor origin discussed. Recommended steps are: repeat plastic surgery as soon as possible after the postoperative appearance of the fistula, conservation of the kidney for as long as possible, and small intestine replacement of the ureter in selected cases. External as compared with internal urinary shunts are preferred in cas

The complicated septic abdominal wound.

Since 1975, we have treated 21 patients with severe postoperative liquefaction fascial necrosis of the abdominal wall (group A, 13 patients), postoperative fascial necrosis with an associated intestinal fistula(e) within the wound (group B, three patients) and postoperative fascial necrosis with multiple internal bowel fistulae causing continuing peritoneal contamination (group C, five patients). Management in group A included general exploratory laparatomy, drainage of intra-abdominal abscesses, debridement of necrotic fascia, and loose closure of the wound with polyethylene (Marlex) mesh. [A woman with hemorrhagic shock].

BACKGROUND: Acute upper gastrointestinal bleeding may be a life-threatening condition that calls for immediate diagnosis and action. MATERIAL AND METHODS: We present an 82-year-old female patient with haematemesis, who during the first few minutes after admission developed haemorrhagic shock. At emergency laparotomy, a profuse bleeding in the duodenum was found. During surgery the patient developed cardiovascular collapse and died. The patient had a previous diagnosis of duodenal ulcer. She was, furthermore, diagnosed with an abdominal aortic aneurysm developing at the cranial anastomosis of

Anal pathology in patients with Crohn's disease.

BACKGROUND: A distinctive feature of patients suffering from Crohn's disease is a predisposition to develop a variety of anal complications. The aetiology of such conditions is unclear, and the reported incidence of anal involvement in Crohn's disease varies party due to the various criteria used for classification. This study aims to review the management of patients with symptomatic anal pathology associated with Crohn's disease at St Vincent's Hospital, Melbourne. METHODS: A database of 306 patients with Crohn's disease referred to the department between January 1978 and October 1994 was r

Perianal Crohn's disease. Results of local surgical treatment.

PURPOSE: This study relates our experience with local surgical management of perianal Crohn's disease. METHOD: Of 1,735 patients with Crohn's disease seen between 1980 and 1990, records of 66 patients (3.8 percent) with symptomatic perianal Crohn's disease treated by local operations were retrospectively reviewed to study outcome of local surgical intervention. RESULTS: All patients had intestinal disease that was limited to the colon in 32 patients (48 percent), ileocolonic region in 22 patients (33 percent), and ileum in 12 patients (18 percent). Types of perianal disease encountered inclu

Use of Inflated Foley Catheters to Prevent Early Empty Pelvis Complications Following Pelvic Exenteration.

For most patients with bulky pelvic tumors, pelvic exenteration remains the only curative option. Although initially reported as a palliative procedure, nowadays it is rather performed with curative intent. Once the resectional phase is ended, a large defect will remain at the level of the pelvic diaphragm, predisposing to severe complications which are generically included under the name of empty pelvis syndrome. It has been widely demonstrated that this type of complication is associated with severe mortality, even if the patient is free of any pelvic recurrence. We present the case of a 56-

[Cholecystoduodenal fistula with subsequent gallstone ileus: case report of an unusual course].

Gallstone ileus is a rare complication of cholecystolithiasis with a high mortality because of the advanced age of the patients and the often delayed diagnosis. Signs of a cholecystoduodenal fistula are often absent in conventional ultrasonic and radiological methods. Treatment options are: 1. Enterolithotomy, 2. One stage procedure or 3. Two stage procedure consisting of enterolithotomy and the surgery of the biliary tract. In the case of our patient we performed a conventional cholecystectomy and the closure of the cholecystoduodenal fistula, which was found accidentally. When the gallstone
