METHODS: In this pragmatic clinical trial conducted at 47 emergency departments in five countries, patients (2 months to <18 years of age) with suspected septic shock and abnormal perfusion were randomly assigned to receive fluid resuscitation with either balanced fluid or 0.9% saline for up to 48 hours. BACKGROUND: Iatrogenic hyponatremia is a common complication following intravenous maintenance fluid therapy (IV-MFT) in hospitalized children. Despite the American Academy of Pediatrics' 2018 recommendations, IV-MFT prescribing practices still vary considerably.

Intravenous fluid is life-saving in hypovolemic shock, but fluid sometimes aggravates the bleeding. During the past 25 years, animal models have helped our understanding of the mechanisms involved in this unexpected effect. One hundred consecutive patients scheduled for coronary artery bypass grafting were randomized to receive either blood or crystalloid cardioplegia. Composite effects of pH and halide ions on the transport of HgCl2 into brush border membrane vesicles (BBMV) were investigated in rats. BBMV were incubated for 10 min. in buffer solution at different pH containing 10(-4) M HgCl2. The authors report on their experience with the treatment of 34 consecutive patients suffering from perforated peptic ulcer with peritonitis. All patients has been treated by the laparoscopic approach using sutures according to the Graham-Steele technique and omental plication.
