MRI evaluation of the temporomandibular joints in juvenile rheumatoid arthritis: a retrospective review.

The purpose of this study was to evaluate the MRI findings of children with rheumatoid arthritis (JRA) affecting the temporomandibular joints (TMJ) and correlate these findings with symptoms. MRI studies of the TMJ in 26 children with a clinical diagnosis of JRA were retrospectively reviewed. All studies included oblique and sagittal T1, T2*, proton density/T2, and coronal T1-weighted images. T1 and proton density/T2-images were repeated with the mouth open. Post contrast sagittal and coronal T1-images were obtained in 19 patients. All studies were done on either 1.5 Tesla or 3.0 Tesla units w…

Pattern of joints involvement in Kashin-Beck disease: a local osteochondropathy in China.

BACKGROUND: Kashin-Beck Disease (KBD) is an endemic osteochondropathy. The disease starts in childhood and attacks the growth of joint cartilage, the joints become deformed and painful, and the worst forms result in dwarfism. The most fiequent joints involved are the finger, wrist, ankles, knees and elbows. In this study the pattern of joints involved in KBD was studied. METHODS: A total of 368 patients aged above 13 from 6 villages in endemic areas of the Shaanxi province, located in the northwest of China, were selected through multistage stratified sampling. The patients were diagnosed bas…

Lumbosacral transitional vertebrae in cats and their effects on morphology of adjacent joints.

The present study aimed to describe the radiographic appearance of lumbosacral transitional vertebrae in the cat and compare prevalence of hip dysplasia and lumbosacral spondylosis with a population of cats without transitional vertebrae. Pelvic radiographs of cats were reviewed retrospectively, providing a population of 100 cats without transitional vertebrae and 14 examples of lumbosacral transitional vertebrae. All cats were assessed for hip dysplasia and lumbosacral spondylosis; the lumbosacral transitional vertebrae identified were also assessed for asymmetry or rotation of sacro-iliac (S

Pathoanatomy of the lower cervical spine facet joints in motor vehicle crash fatalities.

Non-lethal injuries to the cervical spine facet joints have previously been described in decedents from motor vehicle crashes and in clinical settings these joints have been identified as potential culprits in chronic neck pain syndromes. The aim of this study was to conduct a detailed examination of the lower cervical spine facet joints in a forensic cohort of motor vehicle crash victims and controls using comparable data from medicolegal autopsy, stereomicroscopy and histological evaluations. Injuries to the cervical spine facet joints were common in the trauma cases and included facet fract…

Qualitative and quantitative assessment of degeneration of cervical intervertebral discs and facet joints.

Degeneration of intervertebral discs and facet joints is one of the most frequently encountered spinal disorders. In order to describe and quantify degeneration and evaluate a possible relationship between degeneration and biomechanical parameters, e.g., the intervertebral range of motion and intradiscal pressure, a scoring system for degeneration is mandatory. However, few scoring systems for the assessment of degeneration of the cervical spine exist. Therefore, two separate objective scoring systems to qualitatively and quantitatively assess the degree of cervical intervertebral disc and fac…

Statins and dietary fish oils improve lipid composition in bone marrow and joints.

There have been numerous efforts to alter the lipid content of cardiovascular tissues. Although equally important, only limited information is available about musculoskeletal tissues. I characterized joint and bone marrow lipids in patients having joint replacement surgery and explored the effects of fish oils and statins on lipid composition in bone marrow and joint fluid. Joint drainage catheters were used to collect marrow lipids from 84 patients having 128 hip and knee replacements for osteoarthritis, osteonecrosis, and femoral neck fractures (osteoporosis). Statins reduced the amount of l

Role of cathepsin K in normal joints and in the development of arthritis.

Cysteine cathepsins are a large family of proteolytic enzymes active at acidic pH as found in lysosomes. Since its discovery in 1990's, cathepsin K has been shown to be a key enzyme in osteoclastic bone resorption through its activity in the resorption lacuna. Although characteristic to osteoclasts, the expression of cathepsin K has also been observed at other sites in skeleton. Several recent observations have demonstrated up-regulation of cathepsin K in osteoarthritic cartilage and inflamed synovial tissue. As cathepsin K is one of the few extracellular proteolytic enzymes capable of degradi

Celecoxib inhibits nitric oxide production in chondrocytes of ligament-damaged osteoarthritic rat joints.

The purpose of this study was to demonstrate the direct effects of celecoxib, one of the selective cyclo-oxygenase (COX)-2 inhibitors, on nitric oxide (NO) and prostaglandinE2 (PGE2) synthesis in cultured osteoarthritic chondrocyte comparing with those of indomethacin. Articular chondrocytes were isolated from rat osteoarthritic knee joint with damaged anterior cruciate ligament and also from the sham knee joint. Chondrocytes were preincubated with or without IL-1 alpha, and were exposed to celecoxib, indomethacin (non-selective COX inhibitor), or nothing. The amounts of NO and PGE2 in culture

Osteoarthritis in other joints (hip, elbow, foot, ankle, toes, wrist) after sports injuries.

Osteoarthritis of nonknee joints, although less common than knee osteoarthritis, remains a significant and disabling condition for many present and former athletes. These injuries can be caused by repeated loads or following a specific traumatic event. The resulting pain and loss of motion can limit function and ability. Arthroscopic techniques in many cases enable surgeons to symptomatically treat limitations of range of motion and pain, prolonging active careers. Joint replacement remains the ultimate solution for hip osteoarthritis, and may be a viable option in ankle osteoarthritis.

Magnetic resonance images of the temporomandibular joints of patients with acquired open bite.

OBJECTIVES: Acquired anterior open bites were reported as the consequence of condylar collapse, which was associated with inflammatory TMJ disorders. However, we have seen such malocclusion patients whose condylar changes seemed to be related to TMJ degeneration associated with internal derangement. The aims of this study were to review the clinical history and to study the TMJ MRI of these patients. STUDY DESIGN: TMJ MRIs of patients, who had presented acquired anterior open bite at first visit, were retrieved from the image database for the analysis. Clinical histories focused on internal d

[Development of muscle strength in knee, hip and ankle joints after ACL reconstruction].

INTRODUCTION: We searched for answers for following questions: How does the knee muscle strength change in dependence of post OP time after the same ACL-rehabilitation's program? Which changes in muscle strength occur in hip and ankle muscles after ACL? METHODS: In this study 69 patients with ACL-Reconstruction participated, mean 31 years. The Lysholm- and OAK-scores of all were good respectively very good. 28 patients were 3 - 6 months post OP (Gr. I), 20 were 6 - 12 months post OP (Gr. II), 21 was 12 - 24 months post OP (Gr. II). All patients were tested on the isokinetic Cybex 6000, for dy

Etiology, pathogenesis and treatment of habitual dislocations of the T.M. Joints [original work].

Definite treatment of any pathological condition, is based on the understanding of its correct pathogenesis and etiology. Without this basic understanding correct and lasting treatment is not possible. When there is no knowledge of exact pathogenesis and etiology, various theories and treatments are suggested by various workers. This is what exactly happened in the case of habitual dislocations of the T.M. Joints. This in term has happened because the dental surgeons and so called oral surgeons, never expanded their vision, beyond their restricted field of 20 deciduous teeth and 32 permanent t…

Increased interleukin-1beta production in the synovium of glenohumeral joints with anterior instability.

Macroscopic synovitis of the glenohumeral joint is frequently seen during arthroscopy in patients with anterior instability. Interleukin-1beta is known to be expressed in inflamed tissue, to correlate with the magnitude of inflammation, and to affect articular cartilage in the joint. We hypothesized that chronic synovitis may occur in the glenohumeral joint in patients with anterior instability. The purpose of this study was to examine the expression of interleukin-1beta in the synovium of the glenohumeral joint with anterior instability and to discuss its clinicopathologic significance. Hydatid disease of bones and joints. 8 cases followed for 4-16 years.

Hydatid disease is a rare parasitic disease that seldom involves the skeleton. Treatment is difficult because of problems with the preoperative diagnosis, the invasive nature of the bony involvement and the variable anaphylactic reaction to the cyst fluid antigen. We present 8 cases with osseous hydatidosis who were treated over a period of 11 years. The spine was involved in 2 cases, the ilium in 2, the hip in 2, the tibia in 1 and the humerus in 1. We point out that diagnosis is difficult and the prognosis is often poor.

Allogeneic vascularized grafting of human knee joints under postoperative immunosuppression of the recipient.

Vascularized knee joint transplantations have been performed in various animal systems. Up to now no allogeneic vascularized transplantation of a fresh and perfused human knee joint has been realized. This paper reports on the first four grafted human knee joints, performed between April 1996 and July 1997 at the Trauma Center Murnau. The indication for transplantation of a human knee joint is total loss of the joint, including the extensor apparatus, following severe trauma. Management of this defect is first to effect closure of the soft tissue defect combined with external transfixation and…
