BACKGROUND: The role of repeat kidney biopsy in antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) with suspected renal relapse remains uncertain. Clinical indicators such as hematuria, proteinuria and ANCA reappearance often guide therapy but may not reliably distinguish active vasculitis from chronic damage. Repeat biopsy may also provide prognostic information in patients with prolonged immunosuppressive exposure.
METHODS: We retrospectively analyzed adults with biopsy-proven AAV who underwent a second kidney biopsy (KB2) for suspected renal relapses. Clinical, laborato…

The FDA Modernization Act and the subsequent federal policy changes in 2025 have signaled a shift towards the use of non-animal, human-cente … ssibility but often lack physiologically relevant microenvironments. In this review, we discuss how kidney-on-chip and iPSC-derived kidney organoid models serve as physiologically relevant platforms for modeling nephrotoxicity, kidney development, and pathophysiology. Kidney-on-chip models can recapitulate in vivo mechanical forces, such as fluid shear s …

Anaemia is common in chronic kidney disease (CKD) encompassing non-dialysis dependent CKD (NDD-CKD) and dialysis dependent CKD (DD-CKD); people on peritoneal dialysis (PD) and haemodialysis (HD); and kidney transplant recipients (KTR). Iron deficiency and erythropoietin deficiency are the most common causes of anaemia in people with CKD, especially those requiring kidney replacement therapy (KRT). The Renal National Service Framework and National Institute for Health and Clinical Excellence in the UK, and Kidney Disease Improving Global Outcomes (KDIGO), all advocate treatment of anaemia in pe…

PURPOSE OF REVIEW: For those with insulin dependent diabetes mellitus and renal failure, both simultaneous pancreas and kidney (SPK) and pancreas after kidney (PAK) transplant can free the recipient of renal replacement and insulin therapies and provide survival advantage over ongoing dialysis and diabetes. Yet, pancreas transplants are notably declining in the United States, particularly for PAK.
RECENT FINDINGS: Pancreas transplant continues to provide better glycemic control than all present medical therapies. Outcomes for both SPK and PAK also continue to improve, and overall patient surv…

BACKGROUND: Prospective studies on kidney outcomes in critically ill children with acute kidney injury (AKI) are scarce from low- and middle-income countries (LMIC). We conducted a pilot study to evaluate the continuum of transient AKI-persistent AKI-acute kidney disease (AKD) and chronic kidney disease (CKD).
METHODS: Children (1-18 years) admitted to our tertiary Pediatric Intensive Care Unit (PICU) and developing AKI with no known pre-existing kidney co-morbidities from January 2021 to June 2022 were included with follow up visits at 1 and 3 months after AKI onset. AKI and CKD were defined…

Kidney transplantation remains the gold standard treatment for end-stage kidney disease (ESKD), effectively alleviating numerous comorbidities and offering a substantial survival advantage over long-term dialysis. Despite advancements in immunosuppressive regimens and improvements in graft and patient survival rates, extended patient longevity brings an accumulating burden and complexity of bone disease in this population, which often goes underrecognized. The present study reviews the pathophysiology of CKD-MBD in pediatric KTR, focusing on the progression of bone disease before and after tra…

BACKGROUND: Hyperparathyroidism (HPT) is common in end-stage kidney disease and resolves in less than half of kidney transplant (KT) recipients. The ideal timing of parathyroidectomy (PTX), before or after KT, remains unclear. We sought to understand differences in morbidity and mortality after PTX pre-KT and post-KT.
METHODS: We identified adult patients who underwent PTX pre-KT or post-KT between 2012 and 2021 utilizing the National Surgical Quality Improvement Program database. Demographics, clinical characteristics, morbidity, and mortality were compared. Adjusted logistic regression with…

KEY POINTS: In cirrhotic patients with AKI, tubular secretion differentiates those with acute tubular necrosis versus other causes. Different secretory function by cause of AKI may affect treatment approaches and drug dosing in patients with cirrhosis and AKI.
BACKGROUND: AKI is a common and severe complication among hospitalized patients with cirrhosis. The most common causes of AKI in cirrhosis are prerenal azotemia (PRA), hepatorenal syndrome (HRS), and acute tubular necrosis (ATN), and treatment depends on its causes. Kidney proximal tubular secretion is an essential mechanism for elimina…

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic disorder characterized by bilateral renal cysts that detach from parental tubules, progressively leading to renal function loss. Patients with ADPKD represent a unique subset of chronic kidney disease (CKD) individuals, sharing common CKD features while also exhibiting distinct traits specific to ADPKD. This review described the unique patterns of mineral and bone disorders present in ADPKD in the early CKD stages. The principal points are: the extraosseous FGF23 production by cystic tissues in the kidneys and live…

Acute kidney injury (AKI) is common in hospitalized patients and increases short-term and long-term mortality. Treatment options for AKI are limited. Gut microbiota products such as the short-chain fatty acid butyrate have anti-inflammatory actions that may protect tissues, including the kidney, from injury. However, the molecular mechanisms of tissue protection by butyrate are poorly understood. Treatment with oral butyrate for two weeks prior to folic acid-induced AKI and during AKI improved kidney function and decreased tubular injury and kidney inflammation while stopping butyrate before A…

The long-term effects of a single episode of acute kidney injury (AKI) induced by bilateral ischemia-reperfusion injury (BIRI) on kidney lymphatic dynamics are not known. The purpose of this study was to determine if alterations in kidney lymphatics are sustained in the long term and how they relate to inflammation and injury. Mice underwent BIRI as a model of AKI and were followed up to 9 mo. Although kidney function markers normalized following initial injury, histological analysis revealed sustained tissue damage and inflammation for up to 9 mo. Transcriptional analysis showed both acute an…

BACKGROUND: Autosomal Dominant Polycystic Kidney Disease (ADPKD) is a genetic disorder that causes uncontrolled kidney cyst growth, leading to kidney volume enlargement and renal function loss over time. Total kidney volume (TKV) and cyst burdens have been used as prognostic imaging biomarkers for ADPKD.
OBJECTIVE: This study aimed to evaluate nnUNet for automatic kidney and cyst segmentation in T2-weighted (T2W) MRI images of ADPKD patients.
METHODS: 756 kidney images were retrieved from 95 patients in the Consortium for Radiologic Imaging Studies of Polycystic Kidney Disease (CRISP) cohort…

KEY POINTS: Kidney stones are common in patients with autosomal dominant polycystic kidney disease compared with matched controls with CKD. The increase in imaging seen in patients with autosomal dominant polycystic kidney disease correlates with the kidney stone burden in these patients.
BACKGROUND: Patients with autosomal dominant polycystic kidney disease (ADPKD) may have more imaging studies than patients with other forms of CKD. We characterized the imaging burden of patients with ADPKD relative to a CKD population to determine which factors lead to increased imaging in patients with ADP…

INTRODUCTION: The incidence of chronic kidney disease (CKD) is growing rapidly, along with the increasing geriatric population. CKD patients have higher incidence of fractures, stroke, and hospitalizations requiring rehabilitation. This is accompanied with the need for suitable rehabilitation programs to decrease disability and improve functionality to help elderly CKD patients maintain independence in activities of daily living. Considering that survivors of acute kidney injury (AKI) tend to experience decreased quality of life with increased frailty, rehabilitation in the elderly with kidney…

Diabetic kidney disease (DKD) is a major microvascular complication of diabetes mellitus (DM) and is closely associated to programmed cell death. However, the complex mechanisms of necroptosis, an alternative cell death pathway, in DKD pathogenesis are yet to be elucidated. This study indicates that necroptosis is involved in DKD induced by high glucose (HG) both in vivo and in vitro. HG intervention led to the activation of RIPK1/RIPK3/MLKL signaling, resulting in renal tissue necroptosis and proinflammatory activation in streptozotocin/high-fat diet- (STZ/HFD-) induced diabetic mice and HG-i…

Patients with autosomal dominant polycystic kidney disease benefit from specialized care over their lifetimes, starting with diagnosis of the condition with ongoing discussion of both the renal course and extra-renal issues. Both renal and extra-renal issues may continue to cause major morbidity even after successful kidney transplant or initiation of RRT, and extra-renal disease aspects should always be considered as part of routine management. In this review, we will focus on updates in pain/depression screening, cardiac manifestations, liver and pancreatic cysts, kidney stone management, an…
