Midline or transverse laparotomy? A random controlled clinical trial. Part II: Influence on postoperative pulmonary complications.

In a series of 579 patients undergoing major laparotomy, the direction of incision (midline or transverse/oblique muscle-cutting) was decided randomly. The severity of postoperative pulmonary complications was expressed by a scoring system which allowed categorization into mild (score 0-3), moderate (score 4-6) and serious (score 7 or more) complications. The important determinants of high scores were found to be male sex, preoperative pulmonary dysfunction, postoperative ventilatory depression, hypovolaemic and septic shock, inhalation of gastric contents and embolism. In no stratum did the d…

Polypharmacy and anticholinergic burden are common but not independently associated with outcomes after emergency laparotomy in older adults.

PURPOSE: Anticholinergic burden (ACB) is an accumulative effect of medications with individual anticholinergic properties. ACB arising from polypharmacy is common in older adults and has been linked to poorer outcomes in a variety of clinical contexts. With more older patients undergoing surgery, there is greater exposure to higher risk individuals with polypharmacy and increased ACB, although the significance of these postoperatively is unknown. The purpose of the study was to determine the frequency of polypharmacy and accompanying ACB in older patients undergoing laparotomy and to investiga…

Suture Technique for Fascial Closure in Midline Laparotomy in Patients at High Risk of Post-surgical Evisceration.

The objective of this study is to describe the modified Maingot keel closure technique as a reinforced fascial closure strategy in patients at high risk of postoperative evisceration and to report the short-term clinical outcomes associated with its application. We present a case series of four patients undergoing midline laparotomy who were considered at high risk for postoperative fascial dehiscence according to a Van Ramshorst score greater than 6. The modified Maingot keel closure technique was performed as primary or secondary fascial reinforcement according to the clinical scenario. Four…

Utility of the NELA score and other variables in predicting the need for ICU admission post emergency laparotomy.

The preoperative risk score and key recommendations designed by the National Emergency Laparotomy Audit (NELA) have resulted in improved outcomes for patients undergoing emergency laparotomy. However, the recommended routine intensive care/high dependency unit (ICU) admission for high-risk patients is not always logistically possible and predictive models are needed to identify patients who will benefit. We conducted a study of patients undergoing emergency laparotomy at our tertiary hospital in Western Australia between 2018 and 2023 to identify factors that were associated with the need for …

Effects of Perioperative Fluid Resuscitation on the Need for Renal Replacement Therapy in the Damage Control Laparotomy for Trauma.

BackgroundMinimizing crystalloid intravenous fluid (IVF) is a core tenet of damage control resuscitation strategies in trauma. However, there is a lingering notion that such fluid-restrictive strategies may cause acute kidney injury (AKI). We hypothesized that perioperative crystalloid administration volume was not associated with the need for renal replacement therapy (RRT) after damage control laparotomy (DCL).MethodsA retrospective analysis was performed at an urban level 1 trauma center from 2019 to 2022. Risk factors for the need for RRT were evaluated using univariate and multiple logist…

Small-bites versus large-bites closure of midline laparotomies: A systematic review and meta-analysis.

AIM: Surgical site infection (SSI) and incisional hernia (IH) are common complications following midline laparotomy. The small-bites technique for closing a midline laparotomy has been suggested to improve SSI and IH rates compared with the classic mass closure. The aim of this work was to perform a systematic review, meta-analysis and fragility assessment of existing evidence comparing small-bites and conventional closure. METHOD: The study was registered with PROSPERO. A systematic search of PubMed and EMBASE databases was performed for all comparative studies examining small-bites versus c…

Incisional Negative Pressure Wound Therapy in Trauma Laparotomies and Rates of Superficial Surgical Site Infections.

Objectives: Investigate the relationship between incisional negative pressure wound therapy (iNPWT) and the development of superficial surgical site infections (SSIs) and wound dehiscence. Methods: A retrospective chart analysis was performed. Trauma patients between the ages of 18 and 89 years receiving an emergent exploratory laparotomy on admission between January 1, 2015, and March 31, 2022, were included. A multivariable analysis was performed. The primary and secondary outcomes were the development of superficial SSI and wound dehiscence, respectively, on the basis of the type of incisio…

ASGBI trauma committee position statement: a national audit of trauma laparotomy in the UK - an unmet need.

Patients who undergo laparotomy for major trauma are amongst the most critically unwell patients, and they have high morbidity and mortality rates. Despite 20 yr of improvements in resuscitation practices, those who present with hypotension continue to have mortality rates of up to 50%. Currently there is no mechanism for capturing national audit data on these patients, leading to their exclusion from potential quality improvement initiatives. We argue that there is an unmet need for quality assurance in this patient cohort and outline possible mechanisms to address this.

Two exploratory laparotomies within six days: A case of midgut volvulus in an adult with congenital malrotation.

INTRODUCTION: Midgut volvulus in adults based on congenital malrotation, which required emergency surgery, may occur under the stimulation of adverse factors and is rare and easy to be misdiagnosed. PRESENTATION OF CASE: A young male was taken to the emergency room of a local hospital after six hours abdominal pain. Computed tomography (CT) shows intestinal volvulus and exploratory laparotomy was performed. Postoperative CT revealed remission of small intestinal torsion and congenital malrotation of the midgut. The patient vomited frequently within 48 h after the surgery, and was transferred …

A Comparison of a Small-Stitch Closure With a Standard Closure in Midline Laparotomy Incisions: A Prospective Comparative Study.

Introduction Midline incision provides a rapid and thorough approach to the abdominal cavity and, therefore, is widely employed in both emergency and elective procedures. However, midline laparotomy is associated with many post-operative complications like wound dehiscence, incisional hernia, fistula formation, and surgical site infection (SSI). The purpose of the study is to compare the complications that occurred post-operatively in association with the long and small bite suture techniques for midline laparotomy incisions. Methodology A prospective comparative study was carried out among 90…

Ruptured Rudimentary Horn Pregnancy Revealed on Emergency Laparotomy: A Case of Primigravida Presenting in a Developing Country.

Developmental defects in the Mullerian ducts can result in several uterine anomalies including a unicornuate uterus with a rudimentary horn. Pregnancy in a rudimentary horn is extremely rare occurring in 1:76,000-1:160,000 pregnancies. We present a case of an 18-year-old primigravida with a ruptured rudimentary horn pregnancy (RHP) presenting as acute abdomen at 17 weeks of gestation. Hemodynamic instability led us to perform a life-saving emergency laparotomy. A 1.5 liters hemoperitoneum was encountered with a ruptured non-communicating rudimentary horn. Excision of the horn and unilateral sa…

Traumatic colon injury in damage control laparotomy-A multicenter trial: Is it safe to do a delayed anastomosis?

BACKGROUND: Delayed colonic anastomosis after damage control laparotomy (DCL) is an alternative to colostomies during a single laparotomy (SL) in high-risk patients. However, literature suggests increased colonic leak rates up to 27% with DCL, and various reported risk factors. We evaluated our regional experience to determine if delayed colonic anastomosis was associated with worse outcomes. METHODS: A multicenter retrospective cohort study was performed across three Level I trauma centers encompassing traumatic colon injuries from January 2006 through June 2014. Patients with rectal injurie…

Anorectal autotransplantation in a canine model: the first successful report in the short term with the non-laparotomy approach.

Colostomy is conventional treatment for anal dysfunction. Recently, a few trials of anorectal transplantation in animals have been published as a potential alternative to colostomies; however, further development of this technique is required. In this study, we utilized a canine model of anorectal transplantation, evaluated the patency of our microsurgical anastomoses, and assessed the perfusion of the transplanted anus. We designed a canine anorectal transplantation model, wherein anorectal autotransplantation was performed in four healthy beagle dogs by anastomoses of the lower rectum, the b
