Eight children with acute leukemia in relapse were hypokalemic during their hospital course. All had accompanying hypophosphatemia, and three had mild metabolic alkalosis. Potassium chloride supplementation in each case resulted in resolution of the electrolyte and acid-base disturbances. These findings were not present in patients with conditions newly diagnosed or those in remission. The pathogenesis of the electrolyte and acidbase disturbances was not evident and was not related to antibiotic or cytotoxic drug therapy, but may have been related to the patients' poor nutritional status. Seve…

We report a 69-year-old female who was a human T-cell leukemia virus type 1 carrier and exhibited a unique clinical course of developing three hematological malignancies within a short period: diffuse large B-cell lymphoma (DLBCL), chronic myelomonocytic leukemia (CMMoL), and acute myeloid leukemia (AML). Although the blast cells in AML showed typical morphological and immunophenotypical features of acute promyelocytic leukemia (APL), it did not harbor RARα gene fusion and thus initially diagnosed as APL-like leukemia (APLL). The patient developed heart failure with a fulminant clinical course…

BACKGROUND: Aberrant DNA methylation is a hallmark of acute myeloid leukemia (AML) and contributes to leukemogenesis, treatment response, and clinical heterogeneity. While genome-wide methylation studies have identified prognostic methylation signatures, the impact of DNA methylation within pharmacologic pathways and AML-relevant disease genes remains incompletely understood. We investigated the association of DNA methylation in genes of pharmacokinetic/pharmacodynamic (PK/PD) pathways of drugs used to treat AML and in myeloid leukemia-related genes with treatment outcomes in pediatric AML.

CD19 directed immunotherapy with monoclonal antibodies and chimeric antigen receptor T (CAR-T) cell therapy has shown treatment efficacy in the relapsed/refractory setting for non-Hodgkin's lymphoma (NHL) and chronic lymphocytic leukemia (CLL). However, the CD19 directed bispecific T-cell engager, blinatumomab, is currently only approved in B-cell acute lymphoblastic leukemia (ALL) and has largely not been investigated in other clinical contexts for CLL. We report a case of a patient who presented with both Philadelphia chromosome positive (pH+) ALL and CLL who achieved measurable residual dis…

A 13-year-old feline leukemia virus-negative cat was diagnosed with acute lymphoid B-cell aleukemic leukemia. The diagnosis was based on the combination of pancytopenia, large and atypical round cells present on liver and splenic cytology, bone marrow cytology and histopathology showing a majority nucleated population of atypical blast cells, immunohistochemistry, and a lack of malignant lymphoblasts observed in the peripheral blood. The cat was initially treated with L-asparaginase, vincristine, doxorubicin, cyclophosphamide, and prednisolone, which led to a resolution of the cytopenias. Afte…

Relapse of acute myeloid leukemia (AML) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) affects 40-50% of recipients and remains the leading cause of posttransplant mortality; median overall survival is approximately 5 months and 1-year survival is below 20%. Conventional salvage-chemotherapy, immunosuppression withdrawal, donor lymphocyte infusion (DLI), hypomethylating agents, targeted agents, and second transplantation-benefits a minority and has improved only incrementally over two decades. Relapse in this setting reflects leukemia that has survived an engrafted alloge…

BACKGROUND: Leukemia cutis (LC) is a rare complication of acute myeloid leukemia (AML) and considered a marker of poor prognosis. Molecular and immunophenotypic characteristics of LC remain poorly understood.
OBJECTIVE: To investigate molecular and immunophenotypic profiles of AML patients with LC and compare them with AML patients without LC.
METHODS: This retrospective matched-cohort study examined 47 AML patients with LC propensity-matched to 141 AML patients without LC. Frequency of mutations, overall survival, and biomarker concordance between skin and bone marrow samples were analyzed …

OBJECTIVE: The role of NPM1 immunostaining as a surrogate marker for acute myeloid leukemia (AML) with nucleophosmin (NPM1) mutation (AML-NPM1) in leukemia cutis has not been investigated.
METHODS: NPM1 immunostaining was performed using a polyclonal antibody on leukemia cutis diagnosed in 2017-2024 of 15 patients with and 15 without the NPM1 mutation. Targeted next-generation sequencing assays were performed on the initial bone marrow biopsy specimens.
RESULTS: There were 18 skin biopsy specimens from 15 patients (11 men, 4 women, 33-90 years, median: 66 years) with AML-NPM1. Thirteen (87%)…

Leukemia stem cells (LSCs) are widely believed to reside in well-characterized bone marrow (BM) niches; however, the capacity of the BM niches to accommodate LSCs is insufficient, and a significant proportion of LSCs are instead maintained in regions outside the BM. The molecular basis for this niche-independent behavior of LSCs remains elusive. Here, we show that integrin-α9 overexpression (ITGA9 OE) plays a pivotal role in the extramedullary maintenance of LSCs by molecularly mimicking the niche-interacting status, through the binding with its soluble ligand, osteopontin (OPN). Retroviral in…
