UNLABELLED: Systemic mastocytosis is a rare, clonal mast cell disease neoplasm driven by the KIT D816V mutation in greater than 95% of cases. The complex clinical presentation of systemic mastocytosis can make diagnosis challenging. Treatment strategies often focus on management of symptoms, but many patients' symptoms are not well controlled on these regimens and have poor quality of life. We report the case of a 35-year-old Caucasian female who suffered repeated life-threatening anaphylactic episodes that greatly decreased her quality of life and that the best supportive care measures failed

The "Goldilocks Zone", a term borrowed from astrophysics, describes the optimal range where conditions are just right to support life. In healthcare, this metaphor captures the imperative to balance underuse and overuse of medical services, ensuring care is neither excessive nor insufficient but instead maximally effective, equitable, and sustainable. As health systems confront rising costs, workforce constraints, and growing demands for person-centred care, the search for this balance has become increasingly urgent. This review explores how the concept of the Goldilocks Zone can be operationa

Objective: Untreated perinatal mental health disorders can have serious consequences for the whole family system. The purpose of this study was to examine the effectiveness of peer support intervention in improving postpartum psychological distress. Method: We evaluated a remote peer support intervention, Mom Supports Mom, in a randomized controlled trial. The primary study outcome was the level of depressive symptoms at week six after delivery. Secondary outcomes included levels of anxiety symptoms, quality of life and the presence of psychiatric disorders at week six after delivery. Trial pr

BACKGROUND: Chronic kidney disease (CKD) is a growing global health concern requiring effective self-management to mitigate progression and improve quality of life. While self-management is increasingly recognised as a socially embedded practice, the specific contributions of social network members to this process in CKD are not well understood.
OBJECTIVE: To synthesise and interpret existing literature on how social networks support individuals living with early-stage CKD and identify gaps in understanding.
METHODS: We conducted a narrative review. Fourteen studies involving 560 participant

IMPORTANCE: In the US, dialysis remains the reflexive treatment option for end-stage kidney disease. Conservative kidney management (CKM), a nondialytic pathway, is infrequently utilized despite the burden dialysis often imposes on patients, caregivers, and health care systems.
OBJECTIVE: To examine why older adults choose CKM, how caregivers influence this choice, and how nephrologists support CKM through advance care planning and palliative care referrals.
DESIGN, SETTING, AND PARTICIPANTS: This qualitative study included secondary analysis of clinical notes and visit data from patients, c

Stroke is a leading cause of long-term functional disability and often requires caregiver support during recovery. Informal caregivers of post-stroke patients face multiple challenges related to medical and nursing competence, workload, and access to information. These challenges remain insufficiently explored and inadequately addressed in healthcare systems. Using predefined criteria, we searched PubMed and Google Scholar for English-language studies involving human participants and published between January 2015 and August 2025 on issues related to informal caregiving. After initial abstract

Young adults experiencing homelessness have high mental health needs but low service utilization due to barriers such as lack of insurance, transportation, and mistrust of systems. Many struggle to recognize or acknowledge their mental health needs which impacts engagement in care. The point of transition into housing presents a key opportunity to connect young adults with mental health support, yet housing programs do not systematically integrate these services. To address this gap, we developed the Healing Partner (HP) role through a participatory approach that integrated mental health suppo

Neisseria meningitidis is a human-restricted bacteria that is a normal nasopharyngeal resident, yet it can also disseminate, causing invasive meningococcal disease. Meningococci are highly adapted to life in humans, with human-specific virulence factors contributing to bacterial adhesion, nutrient acquisition and immune evasion. While these factors have been explored in isolation, their relative contribution during infection has not been considered due to their absence in small animal models and their expression by different human cell types not readily combined in either in vitro or ex vivo s

PURPOSE: The purpose of this study is to explore the experiences of parents of children with type 1 diabetes mellitus (T1DM) who are living in rural areas. Individuals living in rural areas face barriers to accessing health care that lead to significant health disparities with increased morbidity and mortality. There is a need to understand the unique experiences of those living in rural communities to support their health needs.
METHODS: In this qualitative study, a convenience sample of 11 parents of children who were diagnosed with T1DM in the last 10 years living in Vermont were recruited

Argatroban is a parenteral direct thrombin inhibitor that requires close monitoring to ensure safety and efficacy. Limited data exist to describe its effect in critically ill patients. This was a retrospective, single-center, cohort study that aimed to compare argatroban dosing requirements in those receiving extracorporeal life support (ECLS), continuous renal replacement therapy (CRRT), or neither. Organ dysfunction was assessed using a modified version of the Sequential Organ Failure Assessment (modSOFA) that incorporated the use of extracorporeal support systems. Eighty patients were inclu

Bacteria have evolved a wide array of immune systems to detect and defend against external threats including mobile genetic elements (MGEs) such as bacteriophages, plasmids, and transposons. MGEs are often selfish, exploiting their bacterial hosts to propagate, however they can also provide adaptive advantages through horizontal gene transfer. Gene transfer agents (GTAs), which are non-infectious domesticated prophages, represent a unique class of beneficial MGEs that facilitate bacterial gene transfer. Despite their domestication, GTAs retain phage-like features, including the requirement for

INTRODUCTION: Coordinated medical evacuations represent an important strategy for emergency response when healthcare systems are impaired by armed conflict, particularly for patients diagnosed with life-threatening conditions such as cancer. In this study, we compare the experiences of two parallel medical evacuation systems developed to meet the medical needs of Ukrainians affected by war.
METHODS: This retrospective study compared outcomes of two medical evacuation systems, developed by the European Union Emergency Response Coordination Centre (ERCC) and Supporting Action for Emergency Resp

Varied hospital systems demonstrate diverse and often very different approaches to patient care. This may best be analyzed by looking at specific disorders and outcomes in a population with these disorders. As one such lens, hypertensive disorders of pregnancy (HDPs) continue to pose a severe health risk for mothers and infants, and breastfeeding outcomes play a crucial role in determining long-term maternal and fetal health. This pilot study investigated breastfeeding outcomes in two hospitals, as representatives for differing healthcare systems, among patients diagnosed with HDPs. Data were

Purposes of this study were to (a) assess and describe needs and concerns of single, low-income mothers during their transition to parenthood from the perspective of 16 Midwestern community leaders working closely with families and (b) evaluate social support mechanisms that are available for families. Focus group questions were organized around social support theory to gather information. The following themes evolved from focus group discussions: (a) social support issues (emotional, tangible, informational, and appraisal support; positive and negative support); (b) personal barriers to succe

INTRODUCTION: Health systems including mental health (MH) systems are resilient if they protect human life and produce better health outcomes for all during disease outbreaks or epidemics like Ebola disease and their aftermaths. We explored the resilience of MH services amidst Ebola disease outbreaks in Africa; specifically, to (i) describe the pre-, during-, and post-Ebola disease outbreak MH systems in African countries that have experienced Ebola disease outbreaks, (ii) determine the prevalence of three high burden MH disorders and how those prevalences interact with Ebola disease outbreaks

Children with medical complexity (CMC) are a high priority population with chronic illnesses dependent on the use of health services, on technological systems to support their vital functions and characterized by multiple health needs. One of the main challenges linked to chronic conditions is finding solutions to monitor CMC at home, avoiding re-hospitalization and the onset of complications. Telemedicine enables to remotely follow up patients and families. An integrative review was performed to assess whether telemedicine improves health outcomes for CMC.
