Comparative effectiveness of linezolid versus vancomycin as definitive antibiotic therapy for heterogeneously resistant vancomycin-intermediate coagulase-negative staphylococcal central-line-associated bloodstream infections in a neonatal intensive care unit.

OBJECTIVES: Heterogeneously resistant vancomycin-intermediate coagulase-negative staphylococci (hVICoNS) are emerging pathogens causing central-line-associated bloodstream infections (CLABSIs) in neonatal intensive care unit (NICU) patients. Given the burden of disease associated with CLABSI and the current lack of therapeutic guidelines, we aimed to compare the effectiveness of linezolid versus vancomycin used as the definitive antibiotic therapy for hVICoNS CLABSI. METHODS: We performed a retrospective cohort study of infants with hVICoNS CLABSI from a single NICU between 2009 and 2014, tre…

Associations of recorded in-hospital linezolid and fluoroquinolone exposure with mortality and documented drug-induced liver injury in non-HIV tuberculous meningitis: a propensity score-weighted real-world cohort study.

BACKGROUND: Tuberculous meningitis (TBM) remains the most severe form of tuberculosis. Linezolid- and fluoroquinolone-containing regimens are used in clinical practice, but their associations with mortality and specific toxicities in non-HIV TBM remain uncertain. METHODS: We conducted a single-center retrospective cohort study of non-HIV patients with TBM hospitalized between October 2013 and January 2024. The exposures were recorded linezolid use, fluoroquinolone-class use, and combined linezolid-fluoroquinolone use during the index hospitalization. Each indexed exposure was compared with no…

Intradiscal linezolid (PP353) treatment for chronic low back pain associated with Modic change type 1: an international, first-in-human, randomised, sham procedure-controlled, double-blind, phase 1b clinical trial.

BACKGROUND: Oral amoxicillin reduced pain and disability in patients with chronic Low Back Pain (cLBP) and vertebral endplate bone oedema (Modic changes type 1; MC1) in two randomised controlled trials (RCTs), providing evidence that cLBP with MC1 may be caused by a chronic bacterial infection of the disc. Disc tissue is poorly vascularised, and oral antibiotics will not achieve optimal antibacterial concentration. Linezolid has been formulated for intradiscal administration (PP353) to deliver effective antibacterial therapy while minimising systemic exposure. This trial aimed to establish whe…

A puzzling microbiological and clinical discrepancy in the management of acute, severe skin-soft tissue and joint staphylococcal infection. In vitro antimicrobial susceptibility to glycopeptides, versus in vivo clinical efficacy of linezolid alone.

We present an intriguing case report of a septicemic post-elective surgical staphylococcal knee arthritis and cellulitis which did not respond to long-term courses of associated rifampicin and teicoplanin or vancomycin despite apparently favourable in vitro susceptibility assays, but rapidly resolved after i.v. followed by oral administration of linezolid. The lack of response to a two-week course of glycopeptides cannot be explained by the in vitro mimimum inhibitory concentrations (MIC90) of involved organisms, which showed full susceptibility of Staphylococcus aureus to vancomycin and teico…

CSF pharmacokinetics-pharmacodynamics of linezolid in critically brain injured patients, with or without central nervous system healthcare-associated infection. The PK-Pop-LCR Study: A Multicentre Pharmacokinetics and Pharmacodynamics Population Study.

Linezolid is an alternative to vancomycin for treating Gram-positive central nervous system (CNS) healthcare-associated infections, but its optimal dosing remains debated. The PK-Pop-LCR prospective population pharmacokinetic/pharmacodynamic (PK/PD) study included acute brain injured (ABI) patients with an external ventricular drainage (EVD) receiving linezolid. A population PK model developed using plasma and cerebrospinal fluid (CSF) data, and Monte Carlo simulations calculated the probability of target attainment (PTA) and cumulative fraction of response (CFR) in CSF against methicillin-res…

Population Pharmacokinetic Model of Linezolid and Probability of Target Attainment in Patients with COVID-19-Associated Acute Respiratory Distress Syndrome on Veno-Venous Extracorporeal Membrane Oxygenation-A Step toward Correct Dosing.

During veno-venous extracorporeal membrane oxygenation (vv ECMO) therapy, antimicrobial drugs are frequently used, and appropriate dosing is challenging due to there being limited data to support the dosage. Linezolid is effective against multidrug-resistant Gram-positive pathogens frequently isolated in ECMO patients. In total, 53 steady-state linezolid levels were obtained following 600 mg intravenous (IV) injections every 8 h, and these were used to develop a population pharmacokinetic (PopPK) model in patients with COVID-19-associated acute respiratory distress syndrome (CARDS) on vv ECMO.…

Multiparameter Optimization of Broad-Spectrum Antibacterial Activity, ADMET Properties, and MAO-A/B Inhibition of Novel Biaryloxazolidinone-Hydrazone Derivatives: Identification of a Lead with In Vivo Efficacy against Linezolid-Resistant Staphylococcus aureus.

In our previous study, oxazolidinone EJMC-8b demonstrated potent antibacterial activity and drug-likeness but was limited by MAO-A inhibition, high plasma protein binding, and inferior in vivo efficacy compared to linezolid. Herein, we report the discovery of a promising compound D13, which exhibited excellent antibacterial activity against S. aureus, MSSA, MRSA, LRSA, and LREFa with MIC values of <0.03, 0.125, 0.25, <0.03, and 1 μg/mL, respectively. Subsequently, D13 displayed significantly reduced MAO-A and MAO-B inhibition (IC50 = 51.3 and 47.0 μM, respectively) and superior PK profiles (F …

Simultaneous quantification of linezolid and its metabolites (PNU-142300 and PNU-142586) in oral fluid and capillary blood by UPLC-MS/MS: method validation and clinical application using non-invasive sampling techniques.

BACKGROUND: Therapeutic drug monitoring (TDM) can improve the safety and efficacy of the antibiotic linezolid (LZD), particularly in patients at risk of toxicity or with high pharmacokinetic variability. Although oral fluid (OF) and capillary blood collected through microsampling have emerged as less invasive alternatives to venous blood, the applicability of these matrices for quantifying LZD and its major metabolites-PNU-142300 and PNU-142586-has not been established. This study aimed to develop and validate a UPLC-MS/MS method for the simultaneous quantification of LZD, PNU-142300, and PNU-…
