A highly therapy-resistant case of B-cell lymphoma, unclassifiable, with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma showing strong BCL2 staining, otherwise indistinguishable from Burkitt lymphoma.

B-cell lymphoma, unclassifiable, with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma (iDLBCL/BL) is a newly categorized lymphoma in the WHO Classification of Tumors of Haematopoietic and Lymphoid Tissues v4. This lymphoma has characteristics of both DLBCL and BL, morphologically and genetically. We report a case with therapy-resistant iDLBCL/BL, which used to be categorized as BL in the WHO Classification v3. This case showed strong BCL2 staining, otherwise exhibiting features of BL. The case was treated with intensive chemotherapy containing high-dose methotr…

TARC, a CC chemokine, is frequently expressed in classic Hodgkin's lymphoma but not in NLP Hodgkin's lymphoma, T-cell-rich B-cell lymphoma, and most cases of anaplastic large cell lymphoma.

Thymus and activation-regulated chemokine (TARC) has been identified as a lymphocyte-directed CC chemokine that attracts activated T-helper type 2 (Th2) cells in humans. Recent studies showed that the T cells surrounding Reed-Sternberg cells in Hodgkin's lymphomas (HL) are Th2 type. Anaplastic large cell lymphomas (ALCL), T-cell-rich B-cell lymphoma (TCRBCL) can mimic HL in some instances. This study aimed to establish the pattern of TARC expression in these diseases. Immunohistochemical stain using a polyclonal goat anti-human antibody to TARC was performed on 119 cases of confirmed HL; 99 we…

From oral erosions to lymphoma: a case of paraneoplastic pemphigus with occult lymphoma.

A 67-year-old female with type-2 diabetes mellitus presented with recalcitrant painful oral erosions and violaceous chest lesions, that responded minimally to oral corticosteroids. Due to history of decreased appetite, weight loss and poor therapeutic response, further evaluation was performed, revealing elevated C-Reactive Peptide, Desmoglein-3 antibody, Carbohydrate Antigen 19-9, and intraperitoneal lobulated homogeneously enhancing nodal mass, the latter indicating lymphoma. Histopathology from oral mucosa was suggestive of paraneoplastic pemphigus. The patient was referred to oncology depa…

Lymphoma dissemination is a pathological hallmark for malignant progression of B-cell lymphoma.

Extranodal lymphoma occurs in one-third of lymphoma patients and is a key indicator of the international prognostic index, associated with unfavorable outcomes. Due to the lack of ideal models, the causes and characteristics of extranodal lymphoma are greatly underexplored. Recently, we observed a high incidence of extranodal lymphoma in two types of mouse models with tropism for the brain and kidneys. These findings prompt us to rethink the pathological progression of lymphoma colonization in lymph nodes and non-lymphoid organs. Nodal lymphoma, primary extranodal lymphoma and secondary extran…

Diagnosis and Management of Cutaneous Lymphomas Including Cutaneous T-cell Lymphoma.

The cutaneous lymphomas are malignancies of T-cell and B-cell lymphocytes in which the skin is the primary organ of involvement. The cutaneous T-cell lymphomas include variants that can mimic the presentation of common skin diseases or arthropod bites. Mycosis fungoides, the most common cutaneous T-cell lymphoma, usually presents as fixed asymptomatic patches or plaques in sun-protected areas. The cutaneous B-cell lymphomas have fewer variants that often present as papules or nodules that can mimic nonmelanoma skin cancers. Some therapies for cutaneous lymphoma have unique side effects such as…

Novel Therapies for Follicular Lymphoma and Other Indolent Non-Hodgkin Lymphomas.

When selecting therapy for patients with indolent non-Hodgkin lymphoma (iNHL) including follicular (FL), marginal zone (MZL), small lymphocytic (SLL), and lymphoplasmacytic lymphoma (LPL)/Waldenström macroglobulinemia (WM), there are several factors to consider. With a median age around 70 at diagnosis, many patients have accumulated comorbid conditions that may limit treatment options. Although incurable for most, iNHL is a chronic disease with a median overall survival measured in years to decades. This long natural history changes the risk-to-benefit balance with a lower acceptance of toxic…

[The pulmonary complications related lymphoma in the patient with COPD and Non-Hodgkin Lymphoma].

Non-Hodgkin lymphoma (NHL) is a clonal proliferative disease of B or T cell progenitors originating from lymph nodes or extranodal lymphatic tissue. There are several pulmonary complications associated with NHL. We aimed to discuss two pulmonary complications with high morbidity and mortality associated with lymphoma in our patient followed up with COPD and NHL. Seventy one years old male patient was admitted to the emergency department with sudden onset of dyspnea and chest pain. He had a history of bronchodilator use for COPD for 18 years and chemotherapy and local radiotherapy because of NH…

Two for One: B-Cell Lymphomas with Features of Marginal and Follicular Lymphomas.

Low-grade follicular lymphomas are genetically characterized by the translocation t(14; 18)(q32;q21) with BCL2 gene rearrangements. Marginal zone lymphomas are often associated with translocations or transcriptional deregulations of the MALT gene. We report 2 cases of lymphomas which harbor both the t(14;18)(q32;q21) translocation and MALT gene upregulation. Patients presented with numerous circulating atypical lymphocytes. Lymph node biopsy in both cases on HE staining demonstrated vague nodularity readily highlighted by CD10, CD23, or BCL6. Staining with CD20 and BCL2 demonstrated monotonous…

[In situ lymphoma and other early stage malignant non-Hodgkin lymphomas].

The increasing use of immunohistochemical and molecular investigations of lymphatic tissues results in more frequent detection of early lymphoid proliferations. These show some but not all features of malignant lymphomas without fulfilling the diagnostic criteria for the diagnosis of lymphoid malignancy. In addition to well-known premalignant B-cell proliferations, such as monoclonal gammopathy of unknown significance (MGUS) and monoclonal B-cell lymphocytosis (MBL), so-called in situ lymphomas have recently been described with minimal infiltrates of clonal B-cells in morphologically reactive …

Lymphoplasmacytic lymphoma and other non-marginal zone lymphomas with plasmacytic differentiation.

Small B-cell lymphomas with plasmacytic differentiation frequently present diagnostic challenges. Session 3 of the 2009 Society for Hematopathology/European Association for Haematopathology Workshop focused on lymphoplasmacytic lymphoma (LPL). The submitted cases illustrated classic examples of bone marrow-based and nodal LPL and cases with atypical features, including unusual phenotypes or involvement of extranodal sites. Several cases showed varying degrees of overlap with marginal zone lymphoma, and, as acknowledged in the 2008 World Health Organization classification, a definitive distinct…

Autoimmunity and lymphoma: is mantle cell lymphoma a mistake of the receptor editing mechanism?

Mantle cell lymphoma (MCL) is an aggressive B cell malignancy, which is believed to originate from naïve B cells in the mantle zone of lymph nodes. We speculate that a possible mechanistic hypothesis for the generation of MCL is one in which receptor editing and germinal centre exclusion could be involved in the molecular development and in the display of clinical characteristics of this rare, aggressive and scarcely understood lymphoma. The hypothesis is supported by a preferential autoimmune related IGVH gene utilization in MCL, where VH3-21, VH4-34 and VH5-51 genes are predominant, and by t…

Pediatric follicular lymphomas, marginal zone lymphomas, and marginal zone hyperplasia.

Whereas most B-cell lymphomas in children are of diffuse large B-cell or Burkitt type, other B-cell lymphomas, including follicular and marginal zone B-cell lymphomas, rarely occur Pediatric follicular lymphomas (FLs) have many features indistinguishable from those seen in adults but demonstrate an increased proportion that are localized, lack bcl-2 protein expression, lack bcl-2 translocations, and are grade 3 but still indolent. They also tend to have large expansile follicles. Although usually at nodal or tonsillar sites, other extranodal involvement also occurs, with FL of the testis well …

Primary lymphoma of the conjunctiva--a rare manifestation of indolent non-Hodgkin's lymphoma.

Lymphoid tumours of the conjunctiva are extremely rare. Such tumours can appear as an isolated neoplasm or as part of a systemic disease. We describe the clinical and histopathologic findings of a patient with bilateral conjunctival lymphoma who presented with a 5-year history of chronic conjunctivitis. After clinical staging, including (1) thoracic and abdominal computerised tomography scans, (2) magnetic resonance tomography of the brain and orbita, (3) fibre-optic gastrointestinoscopy, (4) colonoscopy and (5) bone marrow biopsy,.no other foci of the lymphoma were found. The patient received…

Report of the symposium on Cutaneous Lymphomas: Sixth International Conference on Malignant Lymphoma.

The symposium discussed the pathobiology, classification, and treatment of cutaneous lymphomas. Drs. Burg and Kadin commented on the pathophysiology of mycosis fungoides/Sézary syndrome and cutaneous CD30+ lymphoproliferative disorders, respectively. A proposed classification of primary cutaneous lymphomas from the EORTC was presented by Drs. Kerl and Sterry. Dr. Jaffe presented a classification of cutaneous lymphomas based on the REAL classification. All speakers agreed that primary cutaneous lymphomas are usually distinctive in their clinical behavior and biology, and differ from their nodal…

Plasmablastic Lymphoma Presenting With Multiple Cutaneous Nodules Mimicking Primary Cutaneous Marginal Zone Lymphoma.

Plasmablastic lymphoma (PBL) is a rare, high-grade large B-cell lymphoma characterized by large atypical cells with plasmablastic or immunoblastic morphology and a terminal B-cell differentiation phenotype. PBL often occurs in patients with immune deficiency or dysregulation, most commonly HIV infection. Cutaneous involvement is rare and occurs most frequently in those with immune deficiency. We report a 73-year-old immunocompetent female presenting with multiple cutaneous lesions and cervical lymphadenopathy. Nasopharyngeal and cutaneous biopsies confirmed EBV-associated PBL with plasmacytic …

Angioimmunoblastic T-Cell Lymphoma after Treatment of Classic Hodgkin Lymphoma: A Case Report.

We report a case of a 24-year-old man who developed angioimmunoblastic T-cell lymphoma (AITL) after treatment for refractory lymphocyte-rich classic Hodgkin lymphoma (LR-CHL). This patient was treated with the BV+AVD (brentuximab vedotin, doxorubicin, vinblastine, and dacarbazine) protocol for LR-CHL but progressed before completing chemotherapy. The pathological imaging showed the typical findings of LR-CHL at the first onset and first progression. Rescue chemotherapy and high-dose chemotherapy combined with autologous hematopoietic stem cell transplantation (AHSCT) were performed for refract…
