The underappreciated role of auriculotemporal nerve involvement in local failure following parotidectomy for cancer.

BACKGROUND: Locoregional recurrence rates following parotidectomy for cancer remain as high as 20-30%. The auriculotemporal nerve (ATN) may allow parotid cancers to spread from the facial nerve (FN) toward the skull base, causing local recurrence. METHODS: Retrospective review of 173 parotidectomies for malignancy. Preoperative and post-recurrence imaging were reviewed by a neuroradiologist for signs of tumor adjacent to the ATN. RESULTS: Clinical and imaging signs of possible ATN involvement correlated with FN weakness and sacrifice. Eight patients had pathologically confirmed tumor from th

Endoscopic Transoral Resection of Parapharyngeal Osteoma: A Case Report.

Osteoma is a benign, mesenchymal, slow-growing, osteogenic tumor commonly occurring in the craniofacial bones and is characterized by the proliferation of compact or cancellous bone. Solitary osteomas can be classified as peripheral (parosteal, periosteal, or exophytic) when arising from the periosteum or central (endosteal) when arising from soft tissue. Peripheral osteoma occurs most frequently in the paranasal sinuses. Other locations include the orbital wall, temporal bone, pterygoid processes, external ear canal, and, rarely, the mandible. Osteomas in the oromandibular region often appear

Odontomas--silent tormentors of teeth eruption, shedding and occlusion.

Источник пометил публикацию как отозванную

Odontomas are the most common odontogenic tumours of the jaws, characterised by their slow growth and non-aggressive behaviour. They usually remain asymptomatic, and are diagnosed on routine radiographs. Clinically, they are often associated with delayed eruption or impaction of permanent teeth and retained primary teeth. The purpose of this paper is to review the literature and report two cases of odontomas. In the first case, a compound odontoma was associated with an unerupted maxillary permanent right central incisor, in an 11-year-old boy. In the second case, a 12-year-old girl had retain

[Absence of (jaw)bone may be indicative of malignancy].

Following the loss of several molars in the left maxilla, a 73-year-old man asked his general dentist practitioner to restore his chewing efficiency. Initially, reconstruction of the left maxilla with an implant supported fixed partial denture was considered. However, intraorally a soft-tissue swelling was observed extending from the midline of the palate to the left buccal sulcus. A panoramic radiograph showed absence of bone between the oral cavity and the maxillary sinus. At first, after an incision biopsy and histopathologic examination, the condition was diagnosed as nodular fasciitis. Ho

Clinicopathological studies of odontoma in 47 patients.

A 14-year retrospective study was performed on 47 odontomas from the files of the 1st Department of Oral and Maxillofacial Surgery at Nihon University School of Dentistry. Fifty-seven percent of the patients were male and 42.6% were female. The age distribution was 8 to 48 years with a mean age of 22 +/- 9.0 years. There were no particular symptoms associated with the odontomas, and 63.8% of our patients had no symptoms. However, 12 patients complained of swelling and 9 of pain. The tumor was found in the maxilla in 42.6% and in the mandible in 57.4%. According to the WHO histological type cla

Giant cell tumours in mandible and spine: a rare complication of Paget's disease of bone.

The case of a man who was diagnosed as having polyostotic Paget's disease at the age of 52 years is described. He developed a rare neoplastic complication of Paget's disease--a giant cell tumour in his mandible, which was excised. Nine years after the diagnosis of this tumour he developed a new giant cell tumour arising from the L3 vertebral body. He was born in Avellino in Italy, from where five other cases of giant cell tumours arising from Pagetic bone disease have been reported. No family relationship between our patient and the other cases was established. His Paget's disease was particul

Transmandibular exposure of the skull base.

The combined transmandibular-transcervical approach to the skull base ensures a wide field exposure to the lateral and midline compartments of the middle cranial fossa with attendant vascular control. Splitting the lip and mandible in the midline and dividing the floor of the mouth structures along the lateral border of the tongue exposes the parapharyngeal space, infratemporal fossa (lateral compartment), and clivus, nasopharynx, and cervical spine (midline compartment). A variety of benign and malignant intracranial and extracranial skull base lesions have been treated using this approach.

[Calcifying odontogenic cyst: apropos of 5 cases].

With reference to 5 personal cases involving the mandible 3 times and the lower gum twice, the authors recall the principal characteristics of calcified odontogenic cysts: very little clinical manifestation frequently limited to simple tumefaction, well bounded radiological image with low density, cyst with an epithelial border comprized of calcification and ghost cells. Calcified odontogenic cysts which have a definite morphological likeness to Malherbe's epithelioma, also have the same benigh course as the latter.

Novel Local Full-Thickness Skin Grafts for Closure of Free Fibular Osteocutaneous Flap Donor Sites.

PURPOSE: The fibular flap donor site after skin paddle harvest is usually covered with abdominal full-thickness skin grafts. However, this kind of skin graft creates an inevitable third operative area and additional esthetic damage. The aim of this report was to present a novel approach to manage fibular donor site defects using a full-thickness skin graft from the adjacent area and avoid further esthetic damage in a third area. MATERIALS AND METHODS: Seventeen patients underwent mandibular and maxillary defect reconstruction using free fibular osteocutaneous flaps and the fibular donor-site

NF1 diagnosis criteria and associated sarcomatous tumor review of the literature and case report.

AIM: This paper reports the case of a sarcomatous tumor (probably a pleomorphic sarcoma) in a patient with Neurofibromatosis type 1 (NF1) and discusses the association between these two diseases. BACKGROUND: NF1 is an autosomal dominant tumor predisposition syndrome. Malignant transformation is observed in patients with this disease, usually in the form of malignant peripheral nerve sheath tumors (MPNSTs). REPORT: A 72-year-old female patient with diagnosis of NF1 presented with left mandibular region enlarged for about 6 months, intra-oral examination revealed a mass growth on the floor of

Palatal reconstruction utilizing retrieved forehead flap.

Ablation of tumors involving the tonsil and retromolar trigone complex often require partial resection of the soft palate. The pharyngeal wall defect is well reconstructed with the forehead flap. We have used the distal portion of the severed inserted forehead flap to reconstruct the soft palate defect successfully in 35 cases. The nasopharyngeal surface is lined with a book-flap from the remaining palate. This provides full palatal reconstruction with nasopharyngeal mucosa lining: thus preventing flap contracture and the unwanted sequelae of nasopharyngeal air and fluid reflux.

[Natural killer-like T-cell lymphoma appearing in the duodenum with recurrence in a variety of extranodal organs].

A 48-year-old man was admitted to Keio University Hospital in April 1995 with complaints of right abdominal pain and weight loss. Hypotonic duodenography showed a mass located in the 3rd portion of the duodenum. Endoscopic biopsy specimens disclosed diffuse large cell non-Hodgkin's lymphoma of the duodenum. The patient was initially treated with 2 courses of CHOP, albeit with no response. A pancreatoduodenectomy and radiotherapy yielded a complete remission. A year later, lymphoma recurred in the right mandible salivary gland, and a second complete remission was obtained after 6 courses of CHO

[Osteoradionecrosis. I. Etiology, pathogenesis, clinical aspects and risk factors].

In curative therapy of mouth-cavity and oropharyngeal carcinomas the osteoradionecrosis has to be accepted as a calculated risk with an incidence of 4-35%. It is the question of a radio-caused bone death that comes about by progressive and irreversible morphological alterations at bones and at vessels: Loss of osteocytes, active osteoblasts and osteoclasts (hypocellularity), injury of normal bone metabolism, slackening of regeneration process, extreme susceptibility to infections of the devitalized bone, radio-induced obliterating endarteritis with hyalinization, thrombosis and fibrosing of ve

Effect of fibula length on peri-implant resorption after segmental mandibulectomy of ameloblastoma and dental rehabilitation.

STATEMENT OF PROBLEM: Segmental mandibulectomy followed by microvascularized free fibular flap (FFF) reconstruction and dental implant rehabilitation is a standard treatment for ameloblastoma. However, data on the length of the microvascularized FFF related to long-term peri-implant bone level change are sparse. PURPOSE: The purpose of this retrospective clinical study was to investigate the impact of microvascularized FFF length on the peri-implant bone resorption indicated by changes in the crown-to-implant (C/I) ratio after 10 years of functional loading. MATERIAL AND METHODS: A total of
