Within the framework of the Study of Men Born in 1913 and 1923, which is a prospective population study in Gothenburg, Sweden, a subsample consisting of 128 men 20, 30, 50, and 60 years old was investigated in order to elicit information on factors influencing the size of the mastoid cell systems, measured planimetrically from X-ray films. The mastoid cell area (MCA) closely correlated with body size, height being the best measure. Young men had larger relative MCAs (adjusted for height) than old men. Within each age group, men with small relative MCA were characterized by a lower social class…

INTRODUCTION: Mastoid cholesteatoma is classically managed through a postauricular mastoidectomy approach. With the increasing use of endoscopic techniques in otologic surgery, various middle ear diseases can now be managed endoscopically. Modified radical mastoidectomy (MRM) still has a role in managing extensive mastoid cholesteatoma.
OBJECTIVES: To evaluate the feasibility of total transcanal endoscopic modified radical mastoidectomy (TTEMRM) for mastoid cholesteatoma at a single tertiary center.
METHODS: This 5-year retrospective review included 16 patients involving 17 operated ears wit…

OBJECTIVE: The objective of this study was to compare the graft outcomes of cartilage myringoplasty for the repair of chronic otitis media (COM) with a complete ossicular chain between mastoid cavity pneumatization and opacification.
MATERIALS AND METHODS: Chronic perforations with non-cholesteatomatous COM were allocated to either mastoid cavity pneumatization (MCP) group and opacification (MCO) group. All patients underwent endoscopic cartilage underlay myringoplasty alone. The graft success rate, audiometric outcomes, soft tissue opacification change, and complications were evaluated at 12…

Object: Three-stage expansion method represents the most common form of microtia reconstruction in hemifacial microsomia (HFM). Although the complication related expander has lowered owing to the current advances, bony depression in mastoid region in microtia patients with hemifacial microsomia was observed in clinical work. The aim of this study was to quantify bony depression after retroauricular expander implantation and identify associated factors. Methods: 42 patients were enrolled and studied prospectively utilizing 3-dimensional (3D) evaluation. Craniofacial computed tomography (CT) was…

OBJECTIVE: To present the results of a 15-year longitudinal study in a pediatric and adult population with secondary obliteration of troublesome mastoid cavities.
STUDY DESIGN: Prospective longitudinal study.
PATIENTS: Children (≤16 yrs) and adults who had obliteration surgery (228 ears) between 2000 and 2017.
INTERVENTIONS: Therapeutic.
SETTING: Tertiary referral center.
MAIN OUTCOME MEASURES: 1) Incidence of recurrent or residual cholesteatoma; 2) creation of a dry ear at 5 years postsurgery; 3) postoperative complications; 4) waterproof status of ear; 5) number of subsequent ear surger…

CASE: Obliteration with S53P4 bioactive glass of the mastoid and middle ear was performed in a patient suffering from chronic otitis media. Control of the chronic otitis media was achieved, and follow-up imaging after 3 years with 18F-NaF PET/CT showed increased uptake in the obliterated cavity indicating new …

OBJECTIVE: To discuss an effect of transmastoid repair of superior semicircular canal dehiscence and its resolution using a mastoid titanium mesh plate.
PATIENTS: A 53-year-old woman with a history of superior canal dehiscence syndrome (SCDS) and subsequent transmastoid repair experienced dizziness and a loud, drum-like sound in her repaired ear when touching the ear. Palpating the skin over the mastoid defect reproduced the symptoms and could be observed on binocular microscopy as tympanic membrane deformation synchronous with mastoid palpation.
INTERVENTION (S): Surgery to place a titanium…

BACKGROUND: Cholesterol granulomas (CG) of the petrous apex (CGPA) are benign lesions that have high recurrence rates after surgical intervention. We describe the use of a robust silicon drain between the petrous apex and mastoid air cells to allow constant aeration of the lesion for preventing recurrence.
CASE DESCRIPTION: A retrospective analysis was performed using the data of four patients treated at the Maastricht University Medical Centre between 2014 and 2016. Using the middle fossa approach, the petrous apex was reached, the cyst was opened, and the content aspirated. Subsequently, a …

Micro-CT scanning of temporal bones has revealed numerous retroauricular microchannels, which connect the outer bone surface directly to the underlying mastoid air cells. Their structure and dimensions have suggested a separate vascular supply to the mastoid mucosa, which may play a role in middle ear (ME) pressure regulation. This role may be accomplished by changes in the mucosa congestion resulting in volumetric changes, which ultimately affect the pressure of the enclosed ME gas pocket (Boyle's law). Further, such mucosa congestion may be susceptible to α-adrenergic stimulation similar to …
