Community-driven impact of a newborn-focused behavioral intervention on maternal health in Shivgarh, India.

OBJECTIVE: To assess the effect on maternal health outcomes of a community-based behavior change management intervention for essential newborn care leading to a reduction in neonatal mortality. METHODS: A cluster-randomized controlled trial involving 1 control and 2 intervention arms was conducted in Shivgarh, India, between January 2004 and May 2005. Risk-enhancing domiciliary newborn care behaviors, including those posing a concomitant risk to maternal health, were targeted through home visits and community meetings. Secondary outcomes included knowledge of maternal danger signs, self-repor…

Employment, income, and education and risk of postpartum depression: the Osaka Maternal and Child Health Study.

BACKGROUND: Epidemiological evidence regarding the associations of employment, income, and education with the risk of postpartum depression is inconsistent. This prospective study investigated the association between employment, type of job, household income, and educational level and the risk of postpartum depression. METHODS: Subjects were 771 Japanese women. Postpartum depression was defined as present when subjects had an Edinburgh Postnatal Depression Scale score of 9 or higher between 3 and 4 months postpartum. Adjustment was made for age, gestation, parity, cigarette smoking, family st

Impact of the Jamaican birth cohort study on maternal, child and adolescent health policy and practice.

The Jamaica Perinatal Morbidity and Mortality Survey (JPMMS) was a national study designed to identify modifiable risk factors associated with poor maternal and perinatal outcome. Needing to better understand factors that promote or retard child development, behaviour and academic achievement, we conducted follow-up studies of the birth cohort. The paper describes the policy developments from the JPMMS and two follow-up rounds. The initial study (1986-87) documented 94% of all births and their outcomes on the island over 2 months (n = 10 508), and perinatal (n = 2175) and maternal deaths (n =

Associations of gestational weight gain with short- and longer-term maternal and child health outcomes.

The authors investigated the rate of gestational weight gain associated with the lowest combined risk of 5 short- and longer-term maternal and child health outcomes for 2,012 mother-child pairs recruited in 1999-2002 into Project Viva, a prebirth cohort study in Massachusetts. Within each maternal prepregnancy body mass index (BMI, kg/m(2)) stratum, they performed a logistic regression analysis predicting all 5 outcomes, from which they determined the rate of gain at which average predicted prevalence of the adverse outcomes was the lowest. The mean rate of total gestational weight gain was 0.

Testing a scalable community-based approach to improve maternal and neonatal health in rural Nepal.

OBJECTIVE: The aim of the study was to determine the feasibility of improved maternal-neonatal care-seeking and household practices using an approach scalable under Nepal's primary health-care services. STUDY DESIGN: Impact was assessed by pre- and post-intervention surveys of women delivering within the previous 12 months. Each district sample comprised 30 clusters, each with 30 respondents. The intervention consisted primarily of community-based antenatal counseling and dispensing and an early postnatal home visit; most activities were carried out by community-based health volunteers. The implications of objectification theory for women's health: menstrual suppression and "maternal request" cesarean delivery.

Menstrual suppression with oral contraceptives and cesarean delivery on maternal request (CDMR) are relatively new options for women and increasingly are available, particularly in developed countries, around the world. In this article, I explore these issues using objectification theory as a framework for deconstruction. I offer a provocative hypothesis: Women who objectify their bodies are not only interested in menstrual suppression, but also in surgicalized childbirth at their request. I argue that because patriarchal societies have aligned women's reproductive functions with nature, disin

Maternal depressive symptoms and children's receipt of health care in the first 3 years of life.

BACKGROUND: Maternal depression is widely recognized to negatively influence mother-child interactions and children's behavior and development, but little is known about its relation to children's receipt of health care. OBJECTIVE: To determine if maternal depressive symptoms reported at 2 to 4 and 30 to 33 months postpartum are associated with children's receipt of acute and preventive health care services in the first 30 months. DESIGN: Cohort study of data collected prospectively as part of the National Evaluation of Healthy Steps for Young Children (HS). Data sources included medical rec

Integrated maternal and infant health care in the postpartum period in a poor neighborhood in Santiago, Chile.

An integrated postpartum health-care program was established by the Consultorio San Luis de Huechuraba (CSLH), a nongovernmental organization in a neighborhood of extreme poverty in Santiago, Chile. The main components were education, maternal and infant health care, support for the mothers, and active participation of women from the community served. The program was evaluated through indicators of contraceptive use, breastfeeding performance, infant growth and health, and a qualitative assessment of women's satisfaction. Controls were women of similar characteristics attending the nearby publ

The Ngaglik study: an inquiry into birth interval dynamics and maternal and child health in rural Java.

Preliminary analysis of the Ngaglik Study has been able to confirm much of what we knew about birth interval dynamics from past retrospective studies, while also adding new insights into birth spacing and maternal and child health in rural Java. In Ngaglik, a median duration of breast-feeding of over 2 years is associated with durations of lactational amenorrhoea of over a year on average, despite the very early introduction of other foods in the infant diet. Postpartum abstinence appears to play a role in birth spacing for many women, though as previous research has shown, it does not extend

Quality of intrapartum midwifery care in Austria: Implications for birth satisfaction and maternal mental health.

PROBLEM: A substantial proportion of people who give birth experience childbirth as negative or traumatic, which may adversely affect postpartum mental health. BACKGROUND: Respectful, supportive and woman-centred intrapartum care is a recognised determinant of positive birth experiences, yet evidence linking perceived midwifery care quality with birth satisfaction and trauma-related outcomes in Austria remains limited. AIM: To examine the associations between perceived quality of intrapartum midwifery care, birth satisfaction and birth-related post-traumatic stress disorders (PTSD). METHODS

Maternal nutritional status and the risk of metabolic syndrome in pregnancy: implications for public health nutrition.

BACKGROUND: Maternal nutritional status is considered essential for metabolic adaptation during pregnancy. Both malnutrition and obesity are associated with metabolic abnormalities; however, the link between these factors and metabolic syndrome (MetS) in pregnancy remains poorly studied from a public health perspective. AIM: The aim of the current study was to evaluate the association between nutritional status and the risk of metabolic syndrome in pregnant females. METHODS: In a retrospective observational study, data were collected from the medical records of 850 pregnant women from tertia

The Effect of Maternal Pertussis Vaccination on Neonatal Health Outcomes in the Dutch Pregnancy Drug Register.

BACKGROUND: The currently high pertussis-related morbidity and mortality among infants has led to increased international attention for pertussis vaccination during pregnancy. We studied the safety of maternal pertussis vaccination in the Netherlands with respect to neonatal health. METHODS: Data from the cohort the Dutch Pregnancy Drug Register were used. Pregnant participants self-reported their pertussis vaccination and the health outcomes of their offspring. Participants with a singleton live birth ≥ 24 weeks gestation were included. Using log-binomial regression analysis and correction f

An evaluation of a trauma-focused group intervention for women accessing a maternal mental health service.

BACKGROUND: Psychological birth trauma affects 1 in 3 women and 4-5% go on to develop childbirth-related post-traumatic stress disorder (CBPTSD). Maternal Mental Health Services (MMHS) have been established in the UK to provide psychological assessment and intervention for women experiencing CBPTSD following psychological birth trauma. CBPTSD symptoms can impact the transition to parenthood and the parent-infant relationship. Providing timely psychological intervention is therefore crucial in supporting women and families. AIMS AND OBJECTIVE: This service evaluation aimed to evaluate a trauma

Oral Health in Early Childhood: A Cross-Sectional Study on Maternal Knowledge, Attitude and Practice.

INTRODUCTION: Oral health in early childhood is critical for long-term well-being; however, parental knowledge, attitude and practice (KAP) significantly influence children's oral health outcomes. This hospital-based cross-sectional study aimed to evaluate the KAP of mothers regarding their children's oral health and to inform strategies for reducing the prevalence of oral diseases. The objectives were to assess maternal knowledge of oral health, attitudes toward dental check-ups and hygiene practices, oral health-related behaviors, and the impact of education and income on KAP, providing insi

Equity of maternal and child health services in Afghanistan: a spatiotemporal analysis of national survey datasets.

INTRODUCTION: Afghanistan's healthcare system faces geopolitical instability and inequities in maternal and child health (MCH) services, particularly associated with a temporary collapse in funding in 2021. We analysed coverage levels and spatiotemporal trends in sociodemographic inequalities in the country using data from the 2010/2011 and 2022/2023 Afghan Multiple Indicator Cluster Surveys. METHODS: The study's primary outcome was an adapted Composite Coverage Index (CCI) that combined seven essential MCH interventions with corresponding inequality measures, evaluated using Slope Indices of

A Review of Modulation of Gut Microbiota to Mitigate Gestational Diabetes: Implications for Maternal and Child Health.

The increasing prevalence of gestational diabetes mellitus (GDM) is a pressing global public health concern. Despite extensive identification of GDM risk factors and the implementation of screening programs, there has been a notable lack of significant reduction in maternal, fetal, and neonatal complications. It is vital to recognize that the health trajectory of future generations begins before birth, during intrauterine life and even before conception. The development of 'diabetogenic' and 'obesogenic' changes in the DNA of offspring can be triggered not only by adverse intrauterine conditio

Maternal metabolic health conditions and risk of stillbirth in India: evidence from a nationwide survey.

BACKGROUND: Stillbirth, defined by foetal death at or beyond 28 weeks of gestation, represents a significant challenge in India, contributing to approximately 500,000 foetal deaths each year. The country's stillbirth rate of 12.2 per 1000 births underscores the imperative to address this preventable occurrence. While maternal metabolic conditions diabetes, and hypertension, are widely recognized as established risk factors for stillbirth worldwide, the extent of their impact on India's stillbirth burden remains inadequately elucidated due to limited evidence. METHODS: This cross-sectional stu

Associations between season of conception and maternal and perinatal health: a prospective birth cohort study.

BACKGROUND: Global maternal health progress stagnated during the Sustainable Development Goal era, while the impact of meteorological conditions on maternal-infant outcomes remains contentious. We aimed to investigate the relationship between the season of conception and adverse perinatal outcomes. METHODS: We conducted a prospective cohort study, collecting basic demographic characteristics of pregnant women and estimating the season of conception based on the date of the last menstrual period. We did a follow-up until delivery to monitor pregnancy health issues, such as gestational diabetes…
