Surgical management of two complex cases of large juvenile orbital ossifying fibroma.

Ossifying fibroma is a rare benign tumour of the craniofacial skeleton, which can have an aggressive biological behaviour in the paediatric population. Complete surgical excision, where possible, is the ideal. This case report discusses the multidisciplinary, surgical management of two complex cases of large juvenile ossifying fibroma (JOF), involving the orbit. Both patients underwent resection of the orbital ossifying fibroma with no macroscopic evidence of recurrence at 4 years follow-up. Full functional outcome was achieved with preservation of vision and facial nerve function as well as g

[Neurinoma of the hard palate in an 11-year-old girl. Case report].

BACKGROUND: Neurinomas or schwannomas are benign, encapsulated growing tumors of the Schwann cells of the nerve sheets; 25-45% are located in the head and neck region. They are found rarely in the oral cavity. Most of the intraoral schwannomas are located on the tongue. Other less common locations are the buccal mucosa, palate, floor of the mouth, gingiva and lips. The therapy of choice is complete surgical excision. CASE REPORT: An 11-year-old girl presented with a 3-month history of painless swelling of the hard palate. CT scan showed a right palatinal mass of 2 cm, reaching into the right

Substance P- and calcitonin gene-related peptide-like immunoreactive neurons in the rat trigeminal ganglion--with special reference to meningeal and pineal innervation.

The distribution of perikarya showing substance P- (SP) or calcitonin gene-related peptide-like immunoreactivity (CGRP-LI) in the rat trigeminal ganglion (TG) were investigated by means of immunohistochemical methods. Approximately 50% of the perikarya contain CGRP while SP-LI was observed in 1/3 of the cells. IR fibres were seen to leave the ganglion via the ophthalmic, maxillary, and mandibular nerves. The combination of peptide histochemistry and retrograde labelling of cells in the ganglion following injection of a fluorescent tracer into the pineal gland reveals that few SP- or CGRP-LI tr

Median palatine cyst.

Median palatine cysts are rare, nonodontogenic lesion of the hard palate that do not involve the palatine papilla or incisive canal and that usually present as asymptomatic, fluctuant swellings. They may involve the floor the nasal cavity but are treated by simple enucleation, without recurrence. They are composed histologically of a fibrous collagenous tissue wall, with infiltration of chronic inflammatory cells, and lined by stratified squamous and/or respiratory epithelium. They are differentiated from the nasopalatine and other anterior maxillary cysts by the following diagnostic criteria:

Transient unilateral oculomotor and trigeminal nerve palsy following intranasal local anesthetic infiltration: a case report.

BACKGROUND: Complications from in-office inferior turbinate (IT) and septal swell body coblation typically include pain, congestion, and bleeding. However, rare cases of cranial nerve palsies from local anesthetic infiltration can occur. We present a rare case of an oculomotor nerve palsy and palsies of the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve following intranasal local anesthetic infiltration prior to coblation, a complication not previously documented in the literature. Awareness of such rare and transient complications can help surgeons and clinicians avoid un

Endoscopic Endonasal Resection of Schwannoma of Pterygopalatine Fossa.

Trigeminal schwannomas are benign slow-growing tumors originating from the peripheral nerve sheath. They account for 0.1%-0.4% of all intracranial tumors and 1%-8% of all intracranial schwannomas.1-3 While most of these tumors develop in the trigeminal ganglion within the middle fossa, trigeminal schwannomas can develop anywhere along the course of the trigeminal nerve. As a result, they can be intradural, interdural, and extradural.4,5 Trigeminal schwannomas from the pterygopalatine fossa (PPF) are extremely rare and very difficult to remove because of limited access to this region and the ri

Anterior superior alveolar nerve (ASAN).

The anterior superior alveolar nerve (ASAN) is a branch of the infraorbital nerve. Only few studies have morphometrically evaluated the course of the ASAN. Midfacial segments of ten hemisectioned fresh adult cadaver heads were dissected to uncover the anterior wall of the maxilla. Specimens were subsequently decalcified and the bone overlying the ASAN was removed under a microscope to expose the ASAN. Its branching pattern from the infraorbital nerve was recorded, and the course of the ASAN within the anterior wall of the maxillary sinus was morphometrically assessed measuring distances to pre

Three-dimensional courses of zygomaticofacial and zygomaticotemporal canals using micro-computed tomography in Korean.

The zygomatic nerve (ZN), which originates from the maxillary nerve at the pterygopalatine fossa, enters the orbit through the inferior orbital fissure. Within the lateral region of the orbit, the ZN divides into the zygomaticofacial (ZF) and zygomaticotemporal (ZT) nerves. The ZF and ZT nerves then pass on to the face and temporal region through the zygomaticoorbital foramen and enter their own bony canals within the zygomatic bone. However, multiple zygomaticofacial and zygomaticotemporal canals (ZFCs and ZTCs, respectively) can be observed, and their detailed intrabony courses are unknown. …

The front door to meckel's cave: an anteromedial corridor via expanded endoscopic endonasal approach- technical considerations and clinical series.

OBJECTIVE: Tumors within Meckel's cave are challenging and often require complex approaches. In this report, an expanded endoscopic endonasal approach is reported as a substitute for or complement to other surgical options for the treatment of various tumors within this region. METHODS: A database of more than 900 patients who underwent the expanded endoscopic endonasal approach at the University of Pittsburgh Medical Center from 1998 to March of 2008 were reviewed. From these, only patients who had an endoscopic endonasal approach to Meckel's cave were considered. The technique uses the maxi

Immunohistochemical study of nerve fibres with substance P- or calcitonin gene-related peptide-like immunoreactivity in the junctional epithelium of developing rats.

The beginning of innervation in the junctional epithelium of maxillary first molars was examined in gingival tissues from 19 to 32-day-old rats. Substance P- or calcitonin gene-related peptide (CGRP)-like immunoreactivity was demonstrated by the avidin-biotin peroxidase complex method. In 19-day-old rats, nerve fibres with substance P- or CGRP-like immunoreactivity were seen in the connective tissue and oral epithelium, but not in the reduced enamel epithelium, which would be transformed into the junctional epithelium. In 21-day-old rats, the fibres with substance P- or CGRP-like immunoreactiv

Quantitative histochemical and microchemical changes in the adult mouse central nervous system after section of the infraorbital and optic nerves.

The sensory projections from the whiskers of mice and other rodents synapse somatotopically in 3 subnuclei in the brainstem trigeminal complex, in the ventrobasal complex of the thalamus and in the somatosensory cortex. Deafferentation of the whiskers in adult animals results in qualitative and quantitative changes in activities of the metabolic enzymes in the somatosensory cortex (e.g. J. Neuro-sci., 1 (1981) 929-935). We determined the time course and extent of changes in the subcortical trigeminal centers of adult mice after deafferentation. The right infraorbital nerve was sectioned in mic
