Three-dimensional surgical planning for maxillary and midface distraction osteogenesis.

Computerized surgical planning protocols for distraction osteogenesis are still in their rudimentary phase. The authors have developed a system to plan distraction osteogenesis in craniofacial and maxillofacial surgery that uses three-dimensional computed tomography scans and computer simulation in a virtual reality environment. This involves the creation of a three-dimensional bone model of the craniofacial skeleton, which incorporates virtual globes. Virtual osteotomies are performed on the bone model and the movements of the bone segments are simulated. The program generates a recipe for th

Incidence of nasolacrimal injury and turbinectomy--associated atrophic rhinitis with Le Fort I osteotomies.

Injury to the lacrimal system can occur with trauma, craniofacial surgery, rhinoplasty and nasal antrostomy. Nasolacrimal duct injury may potentially occur with Le Fort I superior repositioning. Atrophic rhinitis is also a potential complication secondary to turbinectomy required during Le Fort I superior repositioning. A review of thirty-four cases over three years of Le Fort I osteotomies with superior repositioning found no postoperative lacrimal injury and no subsequent atrophic rhinitis associated with turbinectomy. An anatomical study relating the position of the nasolacrimal duct openin

Basilar aneurysms: a new transclival approach via maxillotomy.

A transoral transclival approach to vertebrobasilar aneurysms, using a Le Fort I maxillary osteotomy rather than splitting the soft and hard palates, was employed successfully in three patients. This technique gave much improved access to the clivus, and eased exposure of the aneurysms without the need for traction on the brain stem or cranial nerves. There were no postoperative cerebrospinal fluid fistulae and no neurological complications. In one patient, a human-derived fibrin adhesive was used for dural repair. The postoperative cosmetic results were excellent and no problems relating to m…

A new surgical approach to extensive tumors in the pterygomaxillary fossa and the skull base.

OBJECTIVE: The article describes a new surgical approach to extensive tumors in the pterygomaxillary fossa and the middle skull base. METHODS: The first incision divides the upper lip in the midline, passes under the nasal pyramid, and extends laterally, reaching the level of the temporamandibular joint, at which point it exits to meet the vertical coronal/preauricular incision. An incision is then made along the maxillary buccogingival fold on the involved side, running from the midline to the retromolar area. Another incision is made along the mandibular buccogingival fold on the involved s

Clinical and radiographic evaluation for two crestal sinus lift techniques: osteotome versus osseodensification. a systematic review and meta-analysis.

PURPOSE: Maxillary sinus floor elevation is a safe and effective surgical technique for achieving vertical bone height, performed through either a lateral or crestal approach. The latter includes both the osteotome technique and osseodensification. The aim of this systematic review was to compare the outcomes of the classic crestal sinus lift technique and the osseodensification sinus lift approach in terms of the bone gain, marginal bone loss, survival rate, follow-up time and complications. METHODS: This review was performed following PRISMA guidelines. An electronic search was conducted ac

Midface Including Le Fort Level Injuries.

Le Fort fractures occur at uniform weak areas in the midface often due to blunt impact to the face. Sporting injuries are a common cause of facial trauma; however, use of protective equipment has reduced the number of sports-related injuries. All patients with traumatic injuries should be evaluated using Advanced Trauma Life Support protocol. Le Fort fractures can contribute to airway obstruction, and urgent intubation may be indicated. Surgery is indicated for most displaced Le Fort fractures to restore function and facial harmony. To facilitate reduction, the original occlusive relationship

A Less-Invasive Window Design for Lateral Wall Maxillary Sinus Augmentations.

The purpose of this article is to compare the larger (8 × 10-mm single implant and 15 × 20-mm multiple implant) lateral window designs used in the sinus elevation procedure with a newly proposed, less-invasive window design and discuss the advantages and limitations of the new design. The less-invasive window designs differ from the larger window designs in both the size and shape of lateral windows. Following creation of a round-window osteotomy, the quality of the sinus membrane and its ability to be elevated are assessed. The initial window can then be enlarged as a vertical oval window for

Transorbital Endoscopic Approach to the Pterygopalatine Fossa: A Less Invasive Alternative to Traditional Access.

PURPOSE: The purpose of this article and accompanying video is to demonstrate a transorbital endoscopic approach for accessing the pterygopalatine fossa (PPF). This technique does not require a skin incision, avoids dissection of critical neurovascular structures, and utilizes a comparatively small osteotomy. The 2 cases presented in this article highlight the utility of a transorbital endoscopic approach for accessing an anatomic region that has traditionally required more invasive techniques to reach. METHODS: Description of surgical technique with 2 illustrative clinical cases and accompan

Modified Osteotome Sinus Floor Elevation Technique for Multiple Edentulous Spaces: A Non-Randomized Controlled Trial.

Objectives: We aimed to demonstrate our modified osteotome sinus floor elevation (OSFE) technique for placing two implants in multiple maxillary posterior edentulous spaces with residual bone height (RBH) < 5 mm, to evaluate the clinical effect and explore the prognosis. Methods: We identified 18 appropriate patients with RBH < 5 mm and 12 patients with RBH ≥ 5 mm. After drill preparation, variously shaped curettes were applied to adequately release the tension of the membrane around the cavity and between two implants by blunt dissection. Then, an osteotome was used to elevate the membrane to

Delayed Bleeding of Sphenopalatine Artery as a Complication in Le Fort I Osteotomy.

Rates of severe complications in orthognathic surgery are low, but when they occur they can be fatal. This article reports a case of laceration of the junction of the posterior lateral nasal artery and the sphenopalatine artery, resulting in severe delayed bleeding. Patient undergoes a multiple segment Le Fort I osteotomy with no intraoperative or immediate postoperative complications. On the fourth postoperative day, he presents with epistaxis and intractable postnasal discharge, is admitted to emergency with signs of shock, and bleeding is detected endoscopically originating from the right s

Osteotome sinus augmentation with less than 5 mm of native bone: a membrane visualization technique using a tapered platform-shifting implant.

As the usage of implants has increased, a trend in dental implantology is a move towards minimally invasive surgical techniques, which offer the advantages of both decreased healing time and lessened surgical morbidity. These improved procedures encourage more patients to consider this treatment. In addition, improved bone-grafting augmentation materials and enhanced implant surface design characteristics have resulted in reduced healing times. The authors describe and discuss the advantages of a modification to the classic osteotome sinus augmentation procedure. This technique, which requires

[Technical note:simple tools in preimplant surgery].

Osteotomes, the bone scraper, and the bone trap are simple tools used for preprosthetic surgery. They are not very invasive, present little risks, and they can be used in private surgical practice with local anaesthesia. The osteotome preserves, dilates, and condenses the alveolar bone instead of eliminating it like drilling does. It does not require any motor or irrigation. It increases the width and the height of the alveolar crest. It allows the localized expansion of narrow or misshapen alveolar crest when its width ranges between 3 and 4.5mm. It also allows performing a localized sinus li

Schneiderian membrane perforation rate during sinus elevation using piezosurgery: clinical results of 100 consecutive cases.

The lateral window sinus elevation procedure has become a routine and highly successful preprosthetic procedure that is used to increase bone volume in the posterior maxilla for the placement of dental implants. Many surgical techniques have been proposed that provide access to the maxillary sinus through the lateral wall to allow for elevation of the sinus membrane. Among these are the multiple variations of the hinge and complete osteotomy techniques, which make use of rotary cutting instruments for the antrostomy. The most common intraoperative complication with these surgical approaches is
