Clinical errors and therapist discomfort with client disclosure of troublesome pornography use: Implications for clinical practice and error reduction.

Mental health professionals are increasingly aware of the need for competence in the treatment of clients with pornography-related concerns. However, while researchers have recently sought to explore efficacious treatments for pornography-related concerns, few explorations of potential clinical judgment issues have occurred. Due to the sensitive, and at times uncomfortable, nature of client disclosures of sexual concerns within therapy, therapists are required to manage their own discomfort while retaining fidelity to treatment. The present paper explores clinician examples of judgment errors

Application of psychoimmunotherapy in patients with alopecia universalis.

Mental stress and immunological abnormality have recently been listed as causes of alopecia universalis. This disease is difficult to treat with only ordinary pharmacotherapy. Thus, from the standpoint of psychoneuroimmunology, stress was relieved by relaxation and image therapy, and administration of small doses of a strong immunosuppressant was effective, leading to clinically favorable results. In addition, changes were recognized in the subpopulation of peripheral lymphocytes and in beta-endorphin before and after relaxation and image therapy. The treatment of alopecia universalis favorabl

Experiences of assessing mental capacity in England and Wales: A large-scale survey of professionals.

Background: The Mental Capacity Act (2005) of England and Wales described in statute a test to determine whether a person lacked the "mental capacity" to make a particular decision. No large-scale survey has explored experiences of capacity assessment across professional groups. Methods: We administered an opportunistic self-report questionnaire survey of professionals who undertake capacity assessments in England and Wales (n= 611). Topics of interest included; how often and where capacity assessment took place, self-ratings of competency and challenges experienced in assessment, use of psych

The effectiveness of mobile app-based interventions to improve parental mental health: a meta-analysis.

BACKGROUND: Parental mental health is a critical determinant of family functioning and child development. Many parents experience heightened vulnerability to stress, depression, and anxiety during the transition to parenthood and across different stages of the parenting process. Structural barriers—including stigma, limited access to mental health services, and competing caregiving demands—often impede timely support. In this context, mobile health (mHealth) applications have emerged as scalable and accessible tools for delivering preventive and early-stage mental health interventions to paren

Interaction between mental health conditions and hip fracture on the 30-day risk of infection among older adults: a Danish matched cohort study.

PURPOSE: Infections are a frequent complication following hip fracture (HF) surgery. Although approximately 30% of patients undergoing HF surgery have a mental health condition, its potential interaction on the risk of post-operative infection remains uncertain. METHODS: Using Danish nationwide registries, we included a cohort of patients with HF aged ≥ 65 years operated during 2004-21 (n=100 335) and an age- and sex-matched comparison cohort from the general population without HF at the index date (n=498 943). Mental health conditions were identified using diagnosis codes F00-F48. Outcomes w

Developing a Health Campaign for Mental Health Awareness and Substance Use Prevention in Response to the COVID-19 Pandemic.

BACKGROUND: Designing effective health campaigns requires a thorough understanding of the intended audience. For stigmatized health issues like substance use disorder and mental health disorders, campaigns must be strategic, culturally competent, and evidence based. This manuscript details the process of creating a comprehensive statewide health campaign focused on mental health awareness and substance use prevention in Texas. METHODS: This study employs a multifaceted data collection approach. It includes a review of relevant health communication literature and an environmental scan to evalu

We Have Spent Time, Money, and Effort Making Self-Help Digital Mental Health Interventions: Is Anyone Going to Come to the Party?

Although efficacious psychotherapies exist, a limited number of mental health care providers and significant demand make their accessibility a fundamental problem. Clinical researchers, funders, and investors alike have converged on self-help digital mental health interventions (self-help DMHIs) as a low-cost, low-burden, and broadly scalable solution to the global mental health burden. Consequently, exorbitant financial and time-based resources have been invested in developing, testing, and disseminating these interventions. However, the public's assumed desirability for self-help DMHIs by ex

Development & evaluation of a training programme to address complex post-COVID psychological needs.

Background & objectives In India, mental health treatment gaps are significant, and related to shortage of trained personnel. Published literature has identified gaps in existing training programmes including - delivering psychological interventions for complex concerns such as intensified post-COVID grief, trauma, suicidality; ongoing mentoring; and culture-sensitive interventions. Despite government initiatives, gaps in treatment and training have been compounded by a surge in psychological distress following COVID-19. This study aimed to develop, implement and evaluate a training programme

Psychosocial challenges affecting patient-defined medication for opioid use disorder treatment outcomes in a low-income, underserved population: Application of the social-ecological framework.

INTRODUCTION: Successful engagement with medication treatment for opioid use disorder is an important focus in reducing mortality associated with the opioid crisis. Mortality remains at unacceptably high levels, pointing to a need for improved understanding of factors that affect medication for opioid use disorder outcomes. This study aims to understand how barriers co-occur and interact to interfere with outcomes in methadone treatment for a low-income, underserved patient population with opioid use disorder. METHODS: This study was conducted at a community-based drug treatment center that s

Postoperative drainage for 6, 12, or 24 h after burr-hole evacuation of chronic subdural hematoma in symptomatic patients (DRAIN-TIME 2): study protocol for a nationwide randomized controlled trial.

BACKGROUND: Chronic subdural hematoma (CSDH) is a common acute or subacute neurosurgical condition, typically treated by burr-hole evacuation and drainage. Recurrent CSDH occurs in 5-20% of cases and requires reoperation in symptomatic patients, sometimes repeatedly. Postoperative subdural drainage of maximal 48 h is effective in reducing recurrent hematomas. However, the shortest possible drainage time without increasing the recurrence rate is unknown. METHODS: DRAIN-TIME 2 is a Danish multi-center, randomized controlled trial of postoperative drainage time including all four neurosurgical d

Testing the effectiveness of Family Spirit Strengths (FSS) - a culturally informed intervention designed to prevent mental health and substance misuse for indigenous families through home visiting.

BACKGROUND: Families represent the foundation of health for many Indigenous communities. Yet, Indigenous women, particularly women of the age who are starting families, face profound challenges related to mental health and substance use. These challenges are rooted in historical trauma, ongoing discrimination, systemic racism, and chronic underfunding of mental health and substance use services, and result in significant gaps in access to care. Family-based home visiting has the potential to address multiple types of mental health and substance use concerns in a way that is more accessible and

Negotiating participation in conversations about care planning between people living with mild neurocognitive disorders and their healthcare proxies: a single case analysis.

BACKGROUND AND OBJECTIVES: The time between diagnosis with a mild neurocognitive disorder (MND) and definitive loss of decisional capacity presents a window of opportunity to participate in advance care planning (ACP). To implement early planning, we need to know how to promote healthy relationships between people with MND and their healthcare proxies. Our objective is to examine how people with MND and their proxies discuss engagement in ACP and how proxies orient toward the ability and right of the person with MND to provide answers. RESEARCH DESIGN AND METHODS: We undertake a conversation

When Teens With Consent Authority Refuse Life-Saving Care: Weighing Capacity and Confidentiality.

What should a clinician do when an adolescent refuses a treatment that might save their life? What if the proposed treatment is in the realm of reproductive health care, which demands heightened attention to autonomy and confidentiality? This Ethics Rounds considers the case of a 15-year-old patient who refused treatment of an ectopic pregnancy, a potentially life-threatening condition, raising difficult questions about whether a clinician should override the treatment decisions of a minor patient who has legal authority in her state to make her own decisions, or breach medical confidentiality

Surrogate decision-making for people with congenital or acquired incapacity of judgement in the absence of known preferences: a scoping review.

BACKGROUND: Medical decision-making for individuals lacking the capacity to consent often involves ethically and emotionally challenging deliberations, how to make appropriate surrogate decisions in acute settings, and how to plan in advance. This is particularly true for two patient populations: (1) adults who have lost their decision-making capacity and whose presumed wishes are unknown, and (2) individuals with congenital incapacity due to lifelong cognitive impairments. This study aimed to critically synthesize the existing literature on surrogate decision-making and advance care planning

Assessment of Decision-Making Capacity in 97 Hospitalized Patients With Cancer: A Call for Standardization.

Hospitalized patients with cancer face pivotal decisions that will affect their cancer care trajectory and quality of life, but frequently lack decision making capacity (DMC). Standardization is conspicuously missing for inpatient oncology teams and for consultation-liaison psychiatrists performing DMC assessments for patients with cancer. This study sought to characterize a single institutional experience of psychiatric consultations to assess DMC. We conducted a retrospective chart review of 97 consecutive psychiatric consultations for DMC from 2017 to 2019. Demographic, hospital-based, and

Capacity to consent to treatment in severe eating disorders.

Eating disorders represent a disabling, deadly and costly condition, whose principal treatment is constituted by weigh restoration and psychotherapy. Partial or total refuse of treatment is very common, leading some authors to question their decision-making capacity (DMC) to consent to treatment. However, very few studies have investigated treatment DMC, leading to contrasting results. Forty-five women were enrolled at the Psychiatric and Eating Disorders Unit of the University Hospital Policlinico Umberto I of Rome. Psychiatric symptoms severity (Brief Psychiatric Rating Scale Expanded, BPRS-
