Autopsy findings in patients with mental handicap.

Autopsies were carried out on 60 mentally handicapped patients and the brain was examined in detail in all cases. The clinical records were studied and correlated with the pathological findings. A variety of pathological changes were found in the central nervous system, and there was a high incidence of microcephaly and gross abnormalities among the patients with epilepsy and neurological dysfunction. The incidence of Alzheimer's disease was very high among patients with Down's syndrome, but in those patients without Down's syndrome, the incidence appeared to be much the same as in the general

Motor and mental side effects of clozapine.

Long-term experience with clozapine has shown that the agent has a motor and mental side effect profile that is distinct in many ways from classical neuroleptics. It can produce a parkinsonian-like bradykinesia and mild akathisia, but no rigidity and rarely tremor. In patients with tardive dyskinesia induced by other neuroleptics, clozapine permits the dyskinesia to disappear in about half the cases. That clozapine may induce tardive dyskinesia in extremely rare cases cannot be excluded, but it seems more likely that this tardive dyskinesia in clozapine-treated patients is due to previous trea

Male mentally handicapped sex offenders.

BACKGROUND: Sex offences are overrepresented in the mentally handicapped, but information about the characteristics and offence behaviour of this group is limited. METHOD: A retrospective case note survey was made of 47 male patients referred for antisocial sexual behaviour. RESULTS: Of 191 offences and/or incidents committed, 55.5% were heterosexual, 24% indecent exposure, 12.4% homosexual, 13.6% serious and 3.6% involved physical assault. Average age of the offenders was 23.9 years, with a mean IQ of 59.5, and there was a high prevalence of family psychopathology, psychiatric illness, mino

Mental retardation, microcephaly and blepharochalasis in brothers.

Details are presented on two brothers who, in many ways, resemble the cases described by Mutchinick (J Med Genet (1972) 9: 60-63). These similarities are explored and other diagnostic possibilities discussed.

Clinical interpretation of the mini-mental state.

The Mini-Mental Status Examination (MMSE) is a brief, structured test of cognitive function. The test is often used as a screening or case-finding instrument for the detection of organic mental disorders or cognitive impairment. However, many clinicians incorporate the MMSE into their clinical mental status examination. In both contexts, cutoff scores are often utilized to interpret the results, with scores below the cutoff being interpreted as evidence of cognitive dysfunction, and scores above the cutoff being interpreted as evidence against such dysfunction. However, when the test is done a

[Neurologic findings in mild mental retardation].

The author subjected to neurological examination all children from a special school for retarded children (39 children, 22 boys, 17 girls). All children had a positive neurological finding. Central hypotonic syndrome was revealed most frequently--31 children; static tremor of the upper extremities--11 children and static tremor of the lower extremities--8 children. Minor cerebellar symptomatology was found in 10 children, developmental dyspraxia-dysgnosia in 6 children. Sixteen children were related (brother and sister, two sisters and cousins, etc.)

Innovations in mental health care delivery.

In serious mental illness (SMI) even good community care does not usually make a major impact on clinical or social function, but patients and relatives prefer community to hospital care, and it tends to be cheaper. Any gains are lost if the required community services are not resourced, coordinated, and maintained indefinitely. A few SMI patients continue to need asylum under one roof. CPNs see more anxiety/depression than SMI. Their patients come increasingly from GPs, and they tend to work in practices with less need. Their cost-effectiveness is uncertain, although nurse behaviour therapist

[Mental retardation in children. Prevalence and etiology].

The prevalence of mental retardation defined as IQ less than 70 has been estimated as ranging between 6.7-11.9 per thousand children in the Nordic countries. Etiological factors can be found in 80% of patients with severe mental retardation (IQ less than 50) and in 50% with mild mental retardation (IQ 50-70). Down's syndrome is the most common specific cause of mental retardation, followed by the Fragile X syndrome, which mainly affects boys. Since the middle of the 1980s Rett's syndrome has been recognized as the most frequent cause of progressive mental retardation in girls. Recent advances

Assessing the loneliness of workers with mental retardation.

This study was designed to determine whether a reliable instrument could be developed to measure the loneliness experienced by workers with mental retardation. In addition, supervisors were asked to rate subjects on loneliness and four other behavioral subtypes in order to determine whether there was a relation between these ratings and the loneliness questionnaire. Three groups of subjects were assessed: persons with mild, moderate, and severe mental retardation. Results indicated that the Worker Loneliness Questionnaire was a reliable instrument for use with persons with mild and moderate me

Imaginative play in children with mental retardation.

We examined the play behavior of 18 kindergarten children without mental retardation and 55 children with different levels of mental retardation, all with a developmental age of 4 to 5 years, and found few differences between the groups with regard to activity, types and quality of play, and play content. Qualitative differences as described in the literature were not confirmed. This may have been due to (a) the play situation, which was an individual contact with a stimulating adult and/or (b) the fact that MA and not CA was the standard of comparison. Consequently, mild to moderate mental re

Analysis of the dynamics of the disorders has shown that in the acute period following trauma, the patients' status was "abnormal personality in nature, being determined by affective and shock reaction in the hyperkinetic and hypokinetic forms. The subacute period was characterized by the predominance of posttraumatic stress disorders (PTSD), whereas the long-term posttraumatic period by personality disorders pertaining to the asthenic (type I) and asthenic (type II) sphere. The clinical characteristics of type I patients was determined by a range of asthenic, psychasthenic, autistic, sensitiv

[Poverty, mental health and survival strategies].

The relation between "poverty and mental health" has long been established. However, the dynamic underlying the relation between social and psychic processes has received much less attention. This article presents certain preliminary results of research whose aim is to promote the emergence of the multiple dimensions behind the problematic of mental health in social conditions characterized by extreme poverty. In addition, the authors base their approach on the assumption that human beings, even underprivileged, are very active players, and explore the strategies that are hereby developed in o

Mental development in Down syndrome mosaicism.

Serial assessments of mental status of 30 subjects with Down syndrome mosaicism were compared with those of 30 subjects with trisomy 21 Down syndrome. The samples were matched for age, sex, and parental socioeconomic background. The mean IQ level of the mosaic Down syndrome group was significantly higher than that of the trisomy 21 group. Many subjects with mosaicism showed better verbal abilities, and some also demonstrated normal visual-perceptual skills in paper- and-pencil tasks. Children with Down syndrome with IQs over 60 at 5 years of age and with relatively normal speech can be viewed

Classifying depression by mental state signs.

The possibility that separation of a categorical depressive disease ('melancholia') from remaining depressive disorders can be improved by assessment of mental state signs was examined in patients treated by representative Sydney psychiatrists and patients referred to a specialised mood disorders unit. A set of signs, principally assessing retardation, was derived within the two samples by principal-components and latent-class analyses. Scores were significantly correlated with clinical, DSM-III, and RDC diagnoses, and appeared independent of severity, suggesting that melancholia can be define

Family planning for the mentally disordered and retarded.

Increasing perception of healthy reproductive functioning and conception prevention has been accompanied by greater recognition of the needs for emotional and sexual fulfillment of individuals with mental disorder or retardation. Although family planning services have burgeoned in the United States and many other countries and the special concerns of mentally disordered and retarded persons have been well documented, organized efforts to include counseling on fertility regulation in mental health and in training programs have, with few exceptions, been sparse. Recent trends are discussed in te

[Mental health of the population and the migration processes].

This is a comparative clinico-epidemiological++ investigation of the rate and syndrome structure of psychopathologic states in a mobile population in a new region, and in a stable population. Investigated were also the migrational attitudes in the mobile population. Individuals with borderline psychopathology, slow-progredient mental diseases and subjects predisposed to these disorders displayed an active involvement into migration flow. Psychopathologic syndromes were detected which accumulated into the mobile population and those related with elimination from this population. The role of dis
