The clinical and radiological outcomes of 25 surgically treated fractures of the proximal third of the fifth metacarpal were retrospectively analysed. Many different methods of osteosynthesis were used. At follow-up after a mean of 3.3 years, 15 of 25 patients had no pain. Most patients regained a nearly full range of motion in the adjacent joints and more than 90% of the contralateral grip strength. X-ray signs of degenerative arthritis in the metacarpohamate joint were observed in 10 of 25 patients. Pain was found to be directly correlated with the presence of degenerative changes.

Twenty-two displaced metacarpal shaft fractures in 19 patients were stabilized with multiple intramedullary K-wires. These were inserted percutaneously through a small window in the base of the metacarpal and were buried in the bone. No external splintage was routinely used postoperatively and all patients were encouraged to mobilize their fingers immediately: formal physiotherapy was not usually required. All the fractures that we were able to follow-up united, but the K-wires bent at the fracture site in two instances, producing 20 degrees angular deformities. The buried K-wires had to be re

Distraction osteogenesis was used to correct congenital hand deformities in 12 digits. The patients' ages at the time of distraction ranged between 2.5 and 7 years. A total of nine patients underwent lengthening of 12 bones. The average period of distraction was 31.1 +/- 17.6 days. The average length of distraction was 23.6 +/- 7.3 mm. Seventeen percent of the patients after lengthening had an angular deformity that required remodeling of the regenerate. No infections were noted. Distraction osteogenesis provides a useful tool for reliable and predictable correction of osseous defects with vas

Simultaneous reconstruction of a traumatic defect of the trapeziometacarpal joint and the first metacarpal with a composite dorsalis pedis flap is described. The transplant, consisting of the second metatarsal and the second metatarsophalangeal joint, was reversed at the defect site for the metatarsophalangeal joint to become a new trapeziometacarpal joint. A long vascular pedicle was necessary for direct micro-vascular anastomoses to the radial vessels at the wrist. This transfer can be expected to be effective in reconstructing composite hand defects involving the trapeziometacarpal joint an

A 30-year-old man sustained a displaced closed fracture of the fourth metacarpal bone, which was treated by open reduction and internal fixation with Kirschner wires. Severe postoperative infection led to a segmental defect with shortening of the fourth metacarpal bone and infected defect nonunion of the fifth metacarpal bone. After serial debridements and intravenous antibiotics, the infection was controlled. An AO external minifixator was applied to restore the length of the fourth metacarpal bone and to stabilize the fourth and fifth metacarpals, and iliac bone grafting was performed, leadi

This paper presents a case report on a 59-year old patient of small stature with moderate mental retardation, strabismus and several rare skeletal anomalies. We found an os odontoideum, a dysontogenetic 3fold fusion of cervical vertebrae, multiple double-sided wrist bone dysplasias and brachytelephalangical digiti pedis I, but no anomalies of the heart or genitals. Clinical signs, biochemical parameters and results of x-ray and ultrasonic examination are demonstrated in this complex malformation syndrome that combines different anomalies known from other syndromes.

Many patients with Williams syndrome (WS) are not diagnosed until they are old enough to demonstrate the characteristic personality and facial changes. A number of these changes are quite subtle and none of them is present in all affected individuals. The cause of WS remains obscure and consequently, there are no cytogenetic, biochemical, or molecular studies to help in the diagnosis of patients in whom the diagnosis is uncertain. We have generated a mean WS metacarpophalangeal pattern profile (MCPP) on 21 clinically diagnosed individuals with WS. This mean syndrome profile shows that WS hands

A new photodensitometric technique (digital image processing, DIP) was performed to assess the bone mineral content of the metacarpal bone as a screening test for bone loss in 340 healthy or osteoporotic women as well as 35 steroid-dosed patients and 46 patients with chronic renal failure on hemodialysis. This procedure was compared with single photon absorptiometry and dual photon absorptiometry in terms of clinical value. Two bone mineral indexes, metacarpal index and sigma GS/D (the integrated bone mineral density of cortical and trabecular bone on a cross section in the middle of the secon

A case of congenital fusion of metacarpal bones with polydactyly is reported. The fusion occurred between the proximal parts of the third and the fourth metacarpal bones. The same type of anomaly was induced in rat fetuses by oral administration of myleran. The anomalies for which the critical period is the same as both metacarpal fusion and polydactyly, were analysed. From the results of the experimental study, the authors propose that the cause of the metacarpal fusion associated with polydactyly is the combination of a disorder of ectoderm-mesoderm interaction and a deficit of mesenchymal c

Over a two-year period, the authors analyzed 76 records using a "hand" computer card divided into two parts: "work-up of the initial lesions" and "functional results". Analysis of 102 cases of fractures in this manner led to the conclusion that outcome was most often favorable when the fracture was extra-articular whatever the treatment, except in the case of complex compound fractures. In contrast, treatment of articular fractures often required secondary surgery (50% of cases) and led to 20% poor or average results. This was the case whatever initial treatment (functional, orthopedic or oper

The present investigation was undertaken to develop a standard method of recording the bone measurements of the normal hand for use in comparative studies of genetic abnormalities of the skeletal system affecting the hand. The data for this study are obtained from the radiographs of the hands of normal 461 males and 565 females aged from 20 to 84 years. Measurements of length and width of all the phalanges and metacarpals are recorded. The phalangeal index and metacarpal index of each bone are obtained by dividing the length by the width. Frequencies of brachymesophalangy of the little finger

Normal values for the metacarpal index (MCI) in British children and adults have been determined. In infancy before the appearance of the metacarpal epiphyses the index is constant up to about two years of age (5.2 in males and 5.5 in females). During childhood the index rises to reach a peak at about 16 years, and then falls slightly during adult life. The curves produced are almost identical to those previously found in black children, and one set of values may be used for children of either ethnic origin. The left hand should be used to calculate the MCI, and the results expressed as either

Two pairs of siblings are described with proportionate dwarfism due to skeletal hypoplasia of prenatal onset. The head size was normal for age and disproportionately large for height. The patients had a characteristic face different from that seen in the Silver-Russell syndrome. The family data are in accordance with autosomal recessive inheritance. In spite of some similarities, the bulk of clinical and genetic evidence suggests that the described intrauterine growth retardation syndrome is different from the Silver-Russell syndrome and presents an apparently "new" entity which has been desig

Positive correlation between the mineral content of the mid site of right third metacarpal and neck of femur has been established using the psi-ray absorption technique described by Shimmins et al. (1972b) to make measurements on excised human bones. An X-ray method (Anderson, Shimmins and Smith, 1966; Shimmins et al. 1972a) was used to measure the mineral content of the right third metacarpal in 101 female and 22 male patients with subcapital fracture of the ferum. When their metacarpal "whole bone densities" were compared with those of control subjects of the same age and sex, they were foun

Septic arthritis/tenosynovitis in the horse can have life-threatening consequences. The purpose of this cross-sectional retrospective study was to describe ultrasound characteristics of septic arthritis/tenosynovitis in a group of horses. Diagnosis of septic arthritis/tenosynovitis was based on historical and clinical findings as well as the results of the synovial fluid analysis and/or positive synovial culture. Ultrasonographic findings recorded were degree of joint/sheath effusion, degree of synovial membrane thickening, echogenicity of the synovial fluid, and presence of hyperechogenic spo

Extracorporeal shock wave therapy (ESWT) is an accepted form of treatment for chronic cases of proximal suspensory desmitis (PSD). Subjective evaluation of horses shortly after being treated with ESWT has led clinicians to comment on an immediate reduction in lameness. This study aimed to evaluate the analgesic effect of ESWT on 16 horses with PSD or PSD-like pain in a fore- or hindlimb. To objectively assess lameness, gait analysis was performed on an instrumented treadmill before and 6, 24, 48 and 72h after ESWT of the origin of the suspensory ligament and the results compared to the effects

Brachydactyly, classically described as shortening of III, IV, and V metacarpals and I distal phalanx, is the typical and most specific sign of Albright's hereditary osteodystrophy, a peculiar phenotype reported in subjects with pseudohypoparathyroidism type Ia (PHP-Ia) caused by mutations in the GNAS gene, which encodes for the alpha-subunit of the stimulatory G protein (Gsalpha). It has been reported in 70% of PHP subjects from routine radiological examinations, but there are no specific data for hand alterations in genetically characterized PHP-Ia subjects. We evaluated the metacarpophalang
