Oscillations, Timing, Plasticity, and Learning in the Cerebellum.

The highly stereotyped, crystal-like architecture of the cerebellum has long served as a basis for hypotheses with regard to the function(s) that it subserves. Historically, most clinical observations and experimental work have focused on the involvement of the cerebellum in motor control, with particular emphasis on coordination and learning. Two main models have been suggested to account for cerebellar functioning. According to Llinás's theory, the cerebellum acts as a control machine that uses the rhythmic activity of the inferior olive to synchronize Purkinje cell populations for fine-tuni

Cardiometabolic medicine in China: Burden, challenges, and strategic priorities.

The development of cardiometabolic medicine (CMM) reflects a shift in cardiovascular disease (CVD) management from organ-based care toward greater cross-system integration. CMM integrates cardiovascular, endocrine, renal, and public health domains to address the bidirectional interplay between metabolic dysfunction and CVD. In this review, we trace the conceptual evolution from metabolic syndrome to cardiometabolic disease (CMD), synthesize evidence for the biological integration of cardiovascular, metabolic, and renal disease, individual cardiometabolic therapies, and emerging integrated care

Blood from a Stone: A Rough Guide for Group Therapy Implementation in Community Mental Healthcare.

Despite its demonstrated efficacy, group therapy remains underutilized in community mental health centers (CMHCs), where structural barriers and a chronic shortfall of funds impede implementation. This article examines the disconnect between academic research and the financial, organizational, and policy constraints that shape care in safety-net settings. Drawing from direct experience at a CMHC serving marginalized populations, we offer a financial analysis that illustrates how current reimbursement models create steep challenges for sustaining group therapy. Guided by domain theory, we prese

Toward trustworthy healthcare AI: designing academic research for translation readiness.

In controlled proof-of-concept studies, healthcare artificial intelligence (AI) systems now routinely match or exceed specialist-level performance. Despite these impressive technical achievements, most systems remain confined to research settings, highlighting an opportunity to better prepare academic AI research for subsequent clinical translation. Successful translation requires attention to technical, regulatory, organizational, and infrastructural factors, ideally from the earliest stages of research. This Perspective focuses on trustworthiness as a key factor that academic researchers can

Anesthesiology 5.0: Thriving amid Volatility, Uncertainty, Complexity, and Ambiguity.

Anesthesiology is experiencing simultaneous crises of workforce depletion, moral distress, rising complexity, and economic constraint-manifestations of a volatile, uncertain, complex, and ambiguous environment. Traditional, efficiency-driven solutions have failed to restore balance and have deepened instability. Anesthesiology 5.0 provides a framework for redesigning perioperative systems around human capacity, shifting responsibility from individual resilience to organizational design. Drawing on Industry 5.0, it emphasizes three interdependent pillars-human-centricity, sustainability, and re

Navigating the labyrinth of healthcare workers' mental health.

The prevalence of mental health issues such as burnout, anxiety, and depression, and the negative impact on both healthcare workers and the healthcare system are notably high. Here, we introduced a dynamic model of mental health that includes stress, coping mechanisms, and resilience, and incorporates individual, organizational, and socio-cultural factors. It emphasized the importance of addressing moral dilemmas, high workloads, and systemic deficiencies that contributed to mental health risks. Additionally, innovative tools like AI-based predictive models were used to identify at-risk indivi

The daily relations between workplace anger, coping strategies, work outcomes, and workplace affiliation.

This study examines the daily relations among workplace anger, coping strategies, work outcomes, and employee dispositions using a conceptual framework based on affective events theory and cognitive perspectives on emotions. A sample of 214 full-time employees took part in a two-week study, contributing 1,611 daily observations through an experience sampling approach. Contrary to the assumption that workplace anger always detrimentally relates to work outcomes, the results showed a nonsignificant relation between workplace anger and workplace resource depletion, as well as a positive link betw

Emotional Exhaustion in Healthcare Workers: Moving Beyond Coping Skills to Improve Organizational Conditions.

OBJECTIVE: Emotional exhaustion (EE)-the first stage of burnout-is related to preventable work environment exposures. We examined the understudied impact of organizational support for safety (OSS) and safety hazards (SH) on EE in a mixed licensed and unlicensed population of healthcare workers (HCWs). METHODS: A work environment exposures survey was conducted in five US public healthcare facilities in 2018-2019. A total of 1059 questionnaires were collected from a predominantly female population of mixed HCWs. RESULTS: Mean EE scores were higher among women, direct care workers, and younger

Long-term effects of COVID-19 on work routines and organizational culture - A case study within higher education's administration.

As a sudden, external event, the COVID-19 pandemic rapidly disrupted the workplace and required organizations to digitalize their working approaches. To understand how such external events affect organizations in the short- and long-term, we investigated the case of a higher education institution's administration, which combines features of public and private organizations. We applied a longitudinal case study and conducted interviews with 39 German higher education institution (HEI) employees at two time points during the first (2020) and second (2021) lockdown. Content analyses revealed that

Internal and Environmental Predictors of Physician Practice Use of Screening and Medications for Opioid Use Disorders.

Medications for opioid use disorder (MOUD) remain highly inaccessible despite demonstrated effectiveness. We examine the extent of screening for opioid use and availability of MOUD in a national cross-section of multi-physician primary care and multispecialty practices. Drawing on an existing framework to characterize the internal and environmental context, we assess socio-technical, organizational-managerial, market-based, and state-regulation factors associated with the use of opioid screening and offering of MOUD in a practice. A total of 26.2% of practices offered MOUD, while 69.4% of prac

Systems medicine: a new approach to clinical practice.

Most respiratory diseases are considered complex diseases as their susceptibility and outcomes are determined by the interaction between host-dependent factors (genetic factors, comorbidities, etc.) and environmental factors (exposure to microorganisms or allergens, treatments received, etc.) The reductionist approach in the study of diseases has been of fundamental importance for the understanding of the different components of a system. Systems biology or systems medicine is a complementary approach aimed at analyzing the interactions between the different components within one organizationa

'Make me normal': the views and experiences of pupils on the autistic spectrum in mainstream secondary schools.

The following article reviews literature pertaining to the association between child maltreatment and self-injury and the ways it varies according to maltreatment type. Research supporting various mediators of the relations between different maltreatment types and self-injury is summarized. Informing mediator models, dominant theories of functionality, particularly affect regulation theories, are summarized and granted empirical support. Following from explanations of its functionality, three developmental pathways (regulatory, representational/interpersonal, and reactive/neurobiological) lead

Admission to general hospital psychiatric wards in Italy. 1. A comparison between two catchment areas with differing provision of outpatient care.

Epidemiological comparisons carried out in Italian settings regarded very different areas. In this paper, we investigate the effects on the hospital activity of two different organizational models of care in two neighbouring areas in Northern Italy, Cremona and Mantua, very similar in sociodemographic features. Cremona is hospital oriented, while Mantua has well developed community services. All the patients of the catchment areas admitted during 1986 were studied. Age distribution for males, marital status, education and percentage of employed were very similar in the two samples. However, ag

The legend and lessons of Geel, Belgium: a 1500-year-old legend, a 21st-century model.

Geel, Belgium, is the home of a legendary system of foster family care for the mentally ill. The current status of Geel's modern system of integrated community care and the 700-year history of this system (including the 10-year-long, international, multi-disciplinary Geel Research Project) are described. As a case study, Geel offers a microcosmic encapsulation of major issues related to mental illness. Though these issues have been dealt with across time and in all places, here they are contained in a single community. This is a case study that can help other communities to identify significan

Sequential Intercept Mapping: Developing Systems-Level Solutions for the Opioid Epidemic.

Sequential intercept mapping, a community-based application of the sequential intercept model, was recently adapted in Ohio to address the complex challenge of the opioid crisis. Sequential intercept mapping for opioids provides a framework for criminal justice, mental health and addictions treatment providers, family members and opioid-involved individuals, and other stakeholders to develop community-based responses that emphasize prevention, regulation, and treatment for opioid dependency, with a goal of reducing unintended deaths and overdoses. The authors describe a promising approach to u

A model to improve preparedness and strategically enhance resiliency at a community level: The Garden State approach.

This paper focuses on community resiliency planning in high-density areas, concentrating on balancing critical infrastructure recovery needs with real-life limitations in funding, and knowledge for how emergency management is done. A case study examines how a New Jersey government agency and state university were able to pilot a community resiliency concept in order to rapidly inject emergency management knowledge into a community that was affected by Superstorm Sandy. The lessons learned are intuitively understood by emergency planning professionals: preparing continuity of operations plans,

The path from a volunteer initiative to an established institution: evaluating 15 years of the development and contribution of the Lighthouse trust to the Malawian HIV response.

BACKGROUND: The HIV epidemic has triggered the development of new health institutions with a special focus on HIV care. The role of these relatively new institutions within the health systems of low-income countries like Malawi is not clearly determined. We evaluate and describe the development of one example, the Lighthouse Trust (Lighthouse), over a period of 15 years (2000-2015). METHODS: Data from multiple sources, including a document review, participatory observation and interviews were analysed, triangulated and synthesized. The institution's development, function, cooperation, financi

Geriatric hip fracture management: keys to providing a successful program.

BACKGROUND: Hip fractures are a common event in older adults and are associated with significant morbidity, mortality and costs. This review examines the necessary elements required to implement a successful geriatric fracture program and identifies some of the barriers faced when implementing a successful program. INTERVENTION: The Geriatric Fracture Center (GFC) is a treatment model that standardizes the approach to the geriatric fracture patient. It is based on five principles: surgical fracture management; early operative intervention; medical co-management with geriatricians; patient-cen

Evaluating deceased organ donation: a programme theory approach.

PURPOSE: Organ donation and transplantation services represent a microcosm of modern healthcare organisations. They are complex adaptive systems. They face perpetual problems of matching supply and demand. They operate under fierce time and resource constraints. And yet they have received relatively little attention from a systems perspective. The purpose of this paper is to consider some of the fundamental issues in evaluating, improving and policy reform in such complex systems. DESIGN/METHODOLOGY/APPROACH: The paper advocates an approach based on programme theory evaluation. FINDINGS: The
