The future of biostatistics: expecting the unexpected.

The paper explores the future of (clinical) biostatistics. In the first part of the paper important trends in biostatistics are reviewed: new applications and more complex data; causal models for observational data; cross-validation-based model building; graphical chain and random effect models; faster computing and new algorithms as Markov chain Monte Carlo; generalized estimating equations and pseudo-likelihood; pooled data sets for meta-analysis and prognostic modelling. In the second part some dreams and nightmares of the author are sketched. Dreams are: implementation of prognostic and di

Leveraging data-driven insights for esophageal and gastric cancer diagnosis.

BACKGROUND: Oesophago-Gastric Cancer (OGC) and its precursor, high-grade dysplasia (HGD), provide a significant health concern owing to their rapid development and often late diagnosis. Timely identification of individuals is crucial to avert illness advancement and enhance clinical results. Nonetheless, there has been little research on machine learning models and effective statistical techniques for predicting OGC risk. AIMS: This research seeks to uncover risk variables linked to HGD and OGC and to assess the prediction efficacy of several machine learning models using diverse patient data.

A General Framework for the Multiple Nonparametric Behrens-Fisher Problem With Dependent Replicates.

In many trials and experiments, subjects are not only observed once but multiple times, resulting in a cluster of possibly correlated observations (e.g., brain regions per patient). Observations often do not fulfill model assumptions of mixed models and require the use of nonparametric methods. In this article, we develop and present a purely nonparametric rank-based procedure that flexibly allows the unbiased and consistent estimation of the Wilcoxon-Mann-Whitney effect P ( X < Y ) + 1 2 P ( X = Y ) $$ P\left(X<Y\right)+\frac{1}{2}P\left(X=Y\right) $$ in clustered data designs. Effects of Statistical Practices for Longitudinal Group Comparison of the Penetration-Aspiration Scale on Power and Effect Size Estimation: A Monte Carlo Simulation Study.

Multiple bolus trials are administered during clinical and research swallowing assessments to comprehensively capture an individual's swallowing function. Despite valuable information obtained from these boluses, it remains common practice to use a single bolus (e.g., the worst score) to describe the degree of dysfunction. Researchers also often collapse continuous or ordinal swallowing measures into categories, potentially exacerbating information loss. These practices may adversely affect statistical power to detect and estimate smaller, yet potentially meaningful, treatment effects. This st

Modeling myometrial smooth muscle contraction.

Existing models of uterine contractions assumed a top-down approach in which the function at the organ or tissue level was explained by the behavior of smaller basic units. A new model of the excitation-contraction process in a single myometrial myocyte was recently developed. This model may be used in a bottom-up approach for the description of the contribution of cellular phenomena to the overall performance of the tissue or organ. In this review, we briefly survey current knowledge of uterine electrophysiology and contractility as well as current modeling techniques, which were successfully

Evaluation of generic EMG-Torque models across two Upper-Limb joints.

Advanced single-use dynamic EMG-torque models require burdensome subject-specific calibration contractions and have historically been assumed to produce lower error than generic models (i.e., models that are identical across subjects and muscles). To investigate this assumption, we studied generic one degree of freedom (DoF) models derived from the ensemble median of subject-specific models, evaluated across subject, DoF and joint. We used elbow (N = 64) and hand-wrist (N = 9) datasets. Subject-specific elbow models performed statistically better [5.79 ± 1.89 %MVT (maximum voluntary torque) er

Narrative review of the development of an ischaemic heart disease prognostic scoring tool (i-IHD score) among patients with type 2 diabetes mellitus in Malaysia.

INTRODUCTION: Ischaemic heart disease (IHD) remains a major cause of mortality among individuals with type 2 diabetes mellitus (T2DM) in Malaysia. Conventional cardiovascular risk models, such as the Framingham risk score, often show limited calibration in Asian populations. Artificial intelligence (Al)-calibrated models have emerged as potential alternatives, yet their generalisability and clinical utility across different populations remain uncertain. This narrative review aimed to summarise existing prognostic models for IHD in patients with T2DM and identify methodological gaps relevant to

A comprehensive investigation of statistical and machine learning approaches for predicting complex human diseases on genomic variants.

Quantifying an individual's risk for common diseases is an important goal of precision health. The polygenic risk score (PRS), which aggregates multiple risk alleles of candidate diseases, has emerged as a standard approach for identifying high-risk individuals. Although several studies have been performed to benchmark the PRS calculation tools and assess their potential to guide future clinical applications, some issues remain to be further investigated, such as lacking (i) various simulated data with different genetic effects; (ii) evaluation of machine learning models and (iii) evaluation o

The roles of predictors in cardiovascular risk models - a question of modeling culture?

BACKGROUND: While machine learning (ML) algorithms may predict cardiovascular outcomes more accurately than statistical models, their result is usually not representable by a transparent formula. Hence, it is often unclear how specific values of predictors lead to the predictions. We aimed to demonstrate with graphical tools how predictor-risk relations in cardiovascular risk prediction models fitted by ML algorithms and by statistical approaches may differ, and how sample size affects the stability of the estimated relations. METHODS: We reanalyzed data from a large registry of 1.5 million p

Model evaluation and case series data.

We discuss aggregation of data from neuropsychological patients and the process of evaluating models using data from a series of patients. We argue that aggregation can be misleading but not aggregating can also result in information loss. The basis for combining data needs to be theoretically defined, and the particular method of aggregation depends on the theoretical question and characteristics of the data. We present examples, often drawn from our own research, to illustrate these points. We also argue that statistical models and formal methods of model selection are a useful way to test t

P3LS: Point Process Partial Least Squares.

Many studies collect data that can be considered as a realization of a point process. Partial least squares (PLS) is a popular analytic approach that combines features from linear modeling as well as dimension reduction to provide parsimonious prediction and classification. However, existing PLS methodologies do not include the analysis of point process predictors. In this article, we introduce point process PLS ($P^3LS$) for analyzing latent time-varying intensity functions from collections of inhomogeneous point processes. We develop a novel estimation procedure for $P^3LS$ that utilizes the

Validation by simulation of a clinical trial model using the standardized mean and variance criteria.

OBJECTIVE: To develop and validate a model of a clinical trial that evaluates the changes in cholesterol level as a surrogate marker for lipodystrophy in HIV subjects under alternative antiretroviral regimes, i.e., treatment with Protease Inhibitors vs. a combination of nevirapine and other antiretroviral drugs. METHODS: Five simulation models were developed based on different assumptions, on treatment variability and pattern of cholesterol reduction over time. The last recorded cholesterol level, the difference from the baseline, the average difference from the baseline and level evolution,

Syndromic surveillance models using Web data: the case of scarlet fever in the UK.

Recent research has shown the potential of Web queries as a source for syndromic surveillance, and existing studies show that these queries can be used as a basis for estimation and prediction of the development of a syndromic disease, such as influenza, using log linear (logit) statistical models. Two alternative models are applied to the relationship between cases and Web queries in this paper. We examine the applicability of using statistical methods to relate search engine queries with scarlet fever cases in the UK, taking advantage of tools to acquire the appropriate data from Google, and

Artificial neural network approach to modelling of metal contents in different types of chocolates.

The relationships between the contents of various metals in different types of chocolates were studied using chemometric approach. Chemometric analysis was based on the application of artificial neural networks (ANN). ANN was performed in order to select the significant models for predicting the metal contents. ANN equations, that represent the content of one metal as a function of the contents of other metals were established. The statistical quality of the generated mathematical models was determined by standard statistical measures and cross-validation parameters. High agreement between exp

When is the vestibular aqueduct enlarged? A statistical analysis of the normative distribution of vestibular aqueduct size.

BACKGROUND AND PURPOSE: The size of vestibular aqueducts (VAs) seen on CT studies varies. The current practice of calling a VA enlarged when it exceeds a certain threshold (eg, 1.5 mm at the midpoint) is arbitrary. Our hypothesis was that statistical analysis of the range of VA widths in a normal-hearing population would lead to a mathematic definition of the upper-limit-of-normal VA width. MATERIALS AND METHODS: The VA midpoint and opercular widths were measured in 73 children with normal hearing. Statistical analysis yielded values of the 99 th, 97.5th, 95th, 90th, 75th, and 50th percentile

Incubation period of Creutzfeldt-Jakob disease in human growth hormone recipients in France.

OBJECTIVE: To estimate the statistical distribution of the incubation period of Creutzfeldt-Jakob disease (CJD) in human growth hormone (hGH) recipients in France. BACKGROUND: Published papers suggest that the median incubation period of hGH-related CJD is approximately 15 years, but there are as yet no statistical data that support this assertion. METHODS: Of the 1,361 hGH recipients who were included in this study, 55 had developed CJD at the time of the study. Individual data on hGH treatment history were available. Different mathematical models were used to estimate the statistical distr

Generative AI for efficient statistical computation of fluids.

We present GenCFD, a generative modeling approach for fast, accurate, and robust statistical computation of three-dimensional turbulent fluid flows. While motivated from conditional score-based diffusion models, GenCFD is both empirically and theoretically validated on generating turbulent flows. Extensive numerical experimentation of challenging three-dimensional fluids demonstrates that GenCFD provides an accurate approximation of relevant statistical quantities of interest while also efficiently generating high-quality realistic samples of such flows. Moreover, we present rigorous theoretic

Standardized survival probabilities and contrasts between hierarchical units in multilevel survival models.

BACKGROUND: In the medical literature, in which time-to-event (such as time to death or disease recurrence) outcomes are commonly studied, hierarchical data is frequently encountered with patients nested within hospitals or regions. Multilevel hierarchical mixed-effects survival models are routinely used in these settings to accommodate the correlation between study subjects belonging to the same cluster and any potential unobserved heterogeneity. However, these analyses usually focus on fixed effects while marginalizing over the random effects, with fully conditional or marginal (on the rando

A Novel Approach for Analyzing Non-Proportional Hazards With Long-Term Survivors: The Defective Yang-Prentice Model.

In survival analysis, addressing non-proportional hazards and the presence of long-term survivors poses substantial methodological challenges, as traditional models often fail to capture these features adequately. To overcome these limitations, a novel long-term survival model is proposed that integrates the Yang and Prentice framework with a defective Gompertz distribution. The proposed model provides a robust approach for analyzing time-to-event data with non-proportional hazards and a cure fraction. Comprehensive simulation studies and an application to colorectal cancer data illustrate its
