Nonerosive reflux disease: a pathophysiologic perspective.

Nonerosive reflux disease (NERD) is the most common phenotype of gastroesophageal reflux disease. By definition, patients with NERD have typical reflux symptoms caused by the intraesophageal reflux of gastric contents but have no visible esophageal mucosal injury. This is in contrast to patients with reflux esophagitis, also known as erosive reflux disease, and Barrett's esophagus, who have obvious esophageal mucosal injury on endoscopy. Only 50% of patients with NERD have pathologic esophageal acid contact time (ACT) as detected on 24-hour pH monitoring (ie, NERD-positive). NERD patients with

Real-time measurement of blood pressure with Nexfin in a patient with thalidomide-related phocomelia.

We report the novel application of photoplethysmographic technology with the Nexfin HD monitor for real-time measurement of blood pressure (BP) in a patient with tetraamelia. The patient was a 58-year-old man with tetraamelia secondary to thalidomide exposure in utero, who presented for surgical excision of a maxillary schwannoma. Because difficulty of cuff use on rudimentary limbs and failure to gain invasive arterial access due to abnormalities of limb vasculature, this population is known to pose some unique challenges for BP measurement. Nexfin may offer an alternative noninvasive method t

Evaluation of North Sea saturation procedures through divers monitoring.

BACKGROUND: Saturation diving is a standard method of intervention for commercial diving during offshore operations. Current saturation procedures achieve a high level of safety with regards to decompression sickness but still put the divers under multiple stressors: 1) Environmental stress (long confinement, heat/cold, dense gases, high oxygen levels), 2) Work stress (muscular fatigue, psychological pressure, breathing equipment, etc.), 3) venous gas emboli associated with decompression, 4) Inflammation related to oxidative stress and microparticles. We present the results of a saturation div

An approach to determining intracranial pressure variability capable of predicting decreased intracranial adaptive capacity in patients with traumatic brain injury.

Nurses caring for traumatic brain injury (TBI) patients with intracranial hypertension (ICH) recognize that patients whose intracranial adaptive capacity is reduced are susceptible to periods of disproportionate increase in intracranial pressure (DIICP) in response to a variety of stimuli. It is possible that DIICP signals potential secondary brain damage due to sustained or intermittent ICH. However, there are few clinically accessible intracranial pressure (ICP) measurement parameters that allow nurses and other critical care clinicians to identify patients at risk of DIICP. The purpose of t

Auditory Neuropathy/Auditory Synaptopathy.

Patients with auditory neuropathy (AN)/auditory synaptopathy (AS) present unique evaluation and management challenges. Communication ability using auditory stimuli varies among patients, with particular difficulty understanding speech in noise. Auditory physiologic responses are key to accurate identification and monitoring of patients with AN/AS. Management approaches should consider individual variation and the possibility of change over time. Many patients with accurately characterized AN/AS demonstrate success with cochlear implants. Areas of discovery, including understanding of synaptic

Pulmonary hypertension.

Pulmonary hypertension may be primary, of unknown etiology, or secondary to existing cardiorespiratory disease. In general, the prognosis is poor, but the superimposed physiologic changes of pregnancy, labor, and delivery may produce a lethal condition. Pregnancy prevention is better than any proposed care. If pregnancy occurs, termination is suggested in the first or early second trimesters. If the choice is to continue the pregnancy, a well-coordinated intensive management plan is necessary. This plan should include midsecond trimester hospital admission of the patient, continuous hemodynami

Cystic hygroma reconsidered: hamartoma or neoplasm? Primary culture of an endothelial cell line from a massive cervicomediastinal hygroma with bony lymphangiomatosis.

A young woman presented with massive enlargement of a giant cervicomediastinal cystic hygroma, which communicated in part with the thoracic duct and was associated with generalized bony lymphangiomatosis. Modern imaging and sophisticated intraoperative physiologic monitoring made one-stage resection feasible. Tissue culture of explants of the hygroma yielded a primary endothelial cell line still surviving after 18 months, which, like the cyst-lining endothelium in the original resected specimen, reacted positively for Factor VIII-associated antigen. These findings, in conjunction with the hist

Developing a Wearable Sensor-Based Digital Biomarker of Opioid Dependence.

BACKGROUND: Repeated opioid exposure leads to a variety of physiologic adaptations that develop at different rates and may foreshadow risk of opioid-use disorder (OUD), including dependence and withdrawal. Digital pharmacovigilance strategies that use noninvasive sensors to identify physiologic adaptations to opioid use represent a novel strategy to facilitate safer opioid prescribing. This study aims to identify wearable sensor-derived features associated with opioid dependence by comparing opioid-naïve individuals to chronic opioid users with acute pain and developing a machine-learning mode

A Narrative Review of HRV Metric Selection for Applied Monitoring: RMSSD as the Default Metric for Sport and Exercise.

Heart rate variability (HRV) monitoring is commonly used in sport and exercise science to assess recovery, training adaptation, and performance readiness. However, the routine reporting of numerous HRV metrics with substantial conceptual and physiological overlap has created inconsistency regarding which indices should be prioritized for applied monitoring. This narrative review synthesizes the contemporary literature to provide practical recommendations regarding HRV metric selection in applied monitoring, with particular emphasis on whether the root mean square of successive differences betw

The Clinical Potential for Therapeutic Drug Monitoring in Alternative Matrices: Considerations for Systemic and Compartmentalized Pharmacology.

Therapeutic drug monitoring (TDM) using alternative matrices beyond traditional venous blood-based approaches, such as capillary blood-based microsampling, cerebrospinal fluid, breast milk, and peripheral blood mononuclear cells, has the potential to enhance precision medicine. Microsampling techniques provide a less invasive and cost-effective alternative for implementing TDM in populations from remote and access-limited areas. Evaluation of drug distribution across physiologic compartments based on pharmacokinetic principles can improve drug efficacy and minimize adverse effects in diverse c

Prone to Success: Strategies to Improve Outcomes in Adult Respiratory Failure Patients.

Acute hypoxemic respiratory failure and acute respiratory distress syndrome (ARDS) are life-threatening conditions marked by impaired gas exchange due to ventilation-perfusion mismatch and intrapulmonary shunting, often from inflammatory alveolar-capillary injury. Recognizing ARDS and initiating lung-protective strategies, such as low tidal volume ventilation, individualized positive end-expiratory pressure, and prone positioning are crucial. Prone positioning improves oxygenation and outcomes in intubated patients and may benefit select awake patients when used early with close monitoring. No

Approach to the Underperforming Athlete.

Children and adolescents who participate in intense sports training may face physical and psychologic stresses. The pediatric health care provider can play an important role in monitoring an athlete's preparation by obtaining a proper sports history, assessing sleep hygiene, discussing nutrition and hydration guidelines, and evaluating physiologic causes of fatigue. Educating parents and athletes on the potential risks of high-intensity training, inadequate rest and sleep, and a poor diet may improve the athlete's performance and prevent symptoms of overtraining syndrome. Quantification of Interstitial Lung Diseases, From the AJR Special Series on Quantitative Imaging.

High-resolution CT (HRCT) plays an important role in diagnosing and monitoring interstitial lung diseases (ILDs). Despite advances, predicting disease progression and treatment response remains challenging. HRCT enables noninvasive visualization and classification of patterns of lung injury and assessment of disease extent. Visual estimation of CT extent of fibrotic lung disease is an independent predictor of mortality and progression but is subjective, with only modest interobserver agreement for radiologic interpretation of ILD. Machine learning-based textural analyses of fibrosis extent on

Laparoscopic and percutaneous radiofrequency ablation of the kidney: acute and chronic porcine study.

OBJECTIVES: The chronic effects of renal radiofrequency ablation are unknown. Herein, we investigate the anatomic and physiologic sequelae of laparoscopic and percutaneous renal radiofrequency ablation in acute and chronic porcine models. METHODS: Our study comprised two phases-an acute phase and a chronic phase. In the acute phase, bilateral laparoscopic renal radiofrequency ablation was performed in 6 animals (12 renal units), which were euthanized immediately after surgery. In the chronic study, bilateral percutaneous renal radiofrequency ablation was performed in 5 animals (10 renal units

Hemodynamic responses to PaCO2 in children after open heart surgery.

We studied the hemodynamic effects of acute alterations in PaCO2 in ten ventilator-dependent children after open heart surgery. Despite end-tidal CO2 monitoring, five children inadvertently developed PaCO2 greater than 50 torr during the study. Increasing and decreasing PaCO2 in these children resulted in significant parallel changes in cardiac index (CI), oxygen delivery, physiologic shunt, mean pulmonary pressure, and right ventricular stroke work index, and inverse changes in systemic vascular resistance index (p less than .01). No significant hemodynamic changes were observed with alterati

Assessment of facial reinnervation by use of chronic electromyographic monitoring.

The study of muscle reinnervation has been difficult because of lack of an accurate, reproducible method to monitor return of function. Visual assessment relies on subjective interpretation. Histology provides anatomic, not functional, information. Electromyography and anatomic tracing have been most effective in evaluating physiologic return of muscle function. It has been difficult to assess the timing of functional return electromyographically because measurements are intermittent and electrode placement varies. A method was designed to allow long-term monitoring of electromyographic (EMG)

[Clinical monitoring: numerical value is the body and timing is life].

Monitoring is more common in today's clinical practice, especially in critical care medicine. Any confusion between monitoring and diagnosis will mislead clinical behavior which could be disastrous. A monitoring parameter usually focuses on a specific physiological point and represents the immediate situation of the point with quantitative values. A series of numerical value of monitoring parameters indicate physiological change or a disease process. If the clinical intervention targeted at the point, the numerical value represents the strength and the direction of the intervention. However, m

Intranasal fentanyl to reduce pain intensity associated with retinopathy of prematurity screening in preterm infants: study protocol for a randomized controlled trial.

BACKGROUND: Preterm infants are frequently exposed to painful procedures associated with adverse short- and long-term neurodevelopmental outcomes. Retinopathy of prematurity (ROP) screening is a necessary examination that is associated with moderate-to-high pain intensity scores and physiologic instability, despite the use of non-pharmacologic comfort measures and topical anesthesia. Current strategies have not reliably reduced procedural pain to mild or no pain levels, highlighting an important gap in neonatal pain management. Intranasal fentanyl provides rapid analgesia without requiring int

Mean Nocturnal Baseline Impedance Correlates With Symptom Outcome When Acid Exposure Time Is Inconclusive on Esophageal Reflux Monitoring.

BACKGROUND & AIMS: Abnormal acid exposure time (AET) is associated with good outcomes of symptoms from antireflux therapy. Low esophageal mean nocturnal baseline impedance (MNBI) is an additional marker of reflux disease. We aimed to evaluate the value of MNBI when analysis of AET produces borderline or inconclusive results. METHODS: We studied a retrospective cohort of 371 patients (mean age, 54.5 ± 0.7 y; 60.0% female) who had persistent reflux symptoms after treatment and underwent ambulatory pH-impedance monitoring off antisecretory therapy at 1 tertiary center in Europe or 1 in the Unite
