Movement disorders in Megalencephalic Leukoencephalopathy with subcortical cysts - A case series.

BACKGROUND: Megalencephalic leukoencephalopathy with subcortical cysts (MLC) has been described in the literature mostly as early-onset leukodystrophy with cerebellar ataxia being the main clinical phenotype. However, other associated movement disorders have also been reported discretely. CASES: Here, we present seven cases of MLC. Cerebellar ataxia was common in them, while dystonia was present in six, parkinsonism in one and stereotypy in two. Six of them, belonging to the Agarwal community, had the common c.135dup variant. CONCLUSION: Our observation highlights the presence of movement di…

Juvenile-onset Huntington's disease - Spectrum and evolution of presenting movement disorders.

Juvenile-onset Huntington's disease (HD) is a rare subset of HD with symptom-onset before the age of 18. In contrast to the adult population, children present early-on with behavioral, psychiatric, and cognitive symptoms, in addition to a diverse spectrum of movement disorders. This poses a distinct challenge in diagnosis and management. We here describe the spectrum of movement disorders, accompanied with detailed video recordings, in seven cases of juvenile-onset HD. Our findings highlight early cognitive and behavioral symptoms, preceding motor symptoms. The diverse movement disorder phenot…

Movement Disorders in Patients with Subacute Sclerosing Panencephalitis: A Systematic Review.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a complication of measles, occurring after a latency of 4-10 years. It continues to occur in developing countries although resurgence is being reported from developed countries. Characteristic features include progressive neuropsychiatric issues, myoclonus, seizures, movement disorders and visual impairment. Electroencephalography (EEG) typically shows periodic generalized discharges, and elevated CSF anti-measles antibodies are diagnostic. Movement disorders are being increasingly recognized as part of the clinical spectrum, and range …

The Neuroprotective Role of BCG Vaccine in Movement Disorders: A Review.

Bacillus Calmette-Guérin (BCG) is the first developed vaccine to prevent tuberculosis (TB) and is the world's most widely used vaccine. It has a reconcilable defense in opposition to tuberculosis, meningitis, and miliary disease in children but changeable protection against pulmonary TB. Immune activation is responsible for regulating neural development by activating it. The effect of the BCG vaccine on neuronal disorders due to subordinate immune provocation is useful. BCG vaccine can prevent neuronal degeneration in different neurological disorders by provoking auto-reactive T-cells. In the

Movement disorders in children: recognition and management in the emergency department.

The differential diagnosis for pediatric patients presenting to the emergency department with movement-based neurological complaints is wide. Clinicians must ensure these children receive an appropriate evaluation to recognize rare diseases. Early recognition of these diagnoses helps facilitate coordination with specialists, guides imaging, and ensures proper management and disposition. This issue reviews 3 less-common movement disorders: acute cerebellar ataxia, anti-N-methyl-D-aspartate receptor encephalitis, and acute disseminated encephalomyelitis. The common presentations, evaluation, and…

Functional Movement Disorders and Deep Brain Stimulation: A Multi-Center Study.

BACKGROUND: Functional movement disorders (FMD) are a commonly under-recognized diagnosis in patients with underlying neurodegenerative diseases. FMD have been observed in patients undergoing deep brain stimulation (DBS) for Parkinson's disease (PD) and other movement disorders. The prevalence of coexisting FMD among movement disorder-related DBS patients is unknown, and it may occur more often than previously recognized. METHODS: We retrospectively assessed the relative prevalence and clinical characteristics of FMD occurring post-DBS, in PD and dystonia patients (FMD+, n = 29). We compared …

Evaluation of DBS Timeline in Movement Disorders: A Comparison Between Genders.

BACKGROUND: Deep brain stimulation (DBS) is a pivotal surgical treatment for movement disorders. However, men and women have had differing opinions and referral experiences related to DBS, leading us to assess whether a gender disparity exists in the interval from a movement disorder diagnosis to DBS usage. METHODS: We performed a single-center, retrospective cohort study of 105 patients who had undergone DBS surgery for either Parkinson disease (PD) or essential tremor (ET). We compared gender differences in the median interval between the diagnosis date, DBS consultation date, and DBS surge…

Sleep Dysfunction in Movement Disorders: a Window to the Disease Biology.

PURPOSE OF REVIEW: To comprehensively summarize the sleep pathologies associated with movement disorders, focusing on neurodegenerative diseases. RECENT FINDINGS: Mounting evidence has further implicated both sleep and circadian disruption in the pathophysiology of many movement disorders. In particular, recent data illuminate the mechanisms by which poor sleep quality and circadian dysfunction can exacerbate neurodegeneration. In addition, anti-IgLON5 disease is a recently described autoimmune disease with various symptoms that can feature prominent sleep disruption and parasomnia. Many move…

Molecular Imaging of Extrapyramidal Movement Disorders With Dementia: The 4R Tauopathies.

Two pathologically distinct neurodegenerative conditions, progressive supranuclear palsy and corticobasal degeneration, share in common deposits of tau proteins that differ both molecularly and ultrastructurally from the common tau deposits diagnostic of Alzheimer disease. The proteinopathy in these disorders is characterized by fibrillary aggregates of 4R tau proteins. The clinical presentations of progressive supranuclear palsy and of corticobasal degeneration are often confused with more common disorders such as Parkinson disease or subtypes of frontotemporal lobar degeneration. Neither of

Neurodegenerative movement disorders: An epigenetics perspective and promise for the future.

Neurodegenerative movement disorders (NMDs) are age-dependent disorders that are characterised by the degeneration and loss of neurons, typically accompanied by pathological accumulation of different protein aggregates in the brain, which lead to motor symptoms. NMDs include Parkinson's disease, multiple system atrophy, progressive supranuclear palsy, and Huntington's disease, among others. Epigenetic modifications are responsible for functional gene regulation during development, adult life and ageing and have progressively been implicated in complex diseases such as cancer and more recently …
