Difficulties with hydroxyapatite orbital implants in two patients with dysfunctional levator/superior rectus muscle complex.

Hydroxyapatite orbital implants are recognized for their excellent results in anophthalmos. Few complications are reported in the literature. We implanted hydroxyapatite spheres in two patients with acquired anophthalmos and denervated or nonfunctional levator palpebrae superioris and superior rectus muscles. In both patients, the implant rotated anteriorly, obliterating the superior fornix and causing proptosis. Prosthetic fitting was impeded, necessitating surgical revision in both patients. A hydroxyapatite implant may rotate and migrate anteriorly in the presence of a nonfunctional superio

Exposure of the forearm and distal radius.

Approaches to the forearm use internervous planes to allow adequate bone exposure and prevent muscle denervation. The Henry approach utilizes the plane between muscles supplied by the median and radial nerves. The Thompson approach utilizes the plane between muscles supplied by the radial and posterior interosseous nerves. The distal radius may be approached volarly. The extended flexor carpi radialis approach is useful for intraarticular fractures, subacute fractures, and malunions. The distal radius can be approached dorsally by releasing the third dorsal compartment and continuing the disse…

Upregulation of Fas/Fas ligand in inclusion body myositis.

In inclusion body myositis, T cells invade and destroy nonnecrotic muscle fibers. The mechanism by which T cells damage muscle fibers in inflammatory myopathies is not known. In this study we have investigated the expression of Fas and Fas ligand in muscle in five patients with inclusion body myositis. Reverse transcription polymerase chain reaction showed upregulation of both Fas and Fas ligand in all inclusion body myositis patients. Immunohistochemical investigation showed expression of Fas ligand in many of the muscle infiltrating mononuclear cells. Fas was expressed both in mononuclear ce

Botulinum-induced alteration of nerve-muscle interactions in the human orbicularis oculi following treatment for blepharospasm.

To assess longstanding alterations in human muscle innervation induced by botulinum toxin, we studied motor axons in the orbicularis oculi of nine patients previously injected with botulinum toxin for treatment of benign essential blepharospasm (BEB). Compared with untreated BEB and normal orbicularis oculi, muscle exposed to botulinum toxin developed persistent and cumulative alterations of innervation, including (1) thin, unmyelinated axonal collaterals that contact muscle end plates, (2) an increased number of muscle fibers innervated by individual terminal motor axons, (3) a profusion of u

Targeted Nerve Implantation: Intramuscular Coaptation in a Denervated Motor Endplate Zone for Below and Above the Knee Amputations.

Post-amputation neuropathic pain, including symptomatic neuroma pain and phantom limb pain (PLP), remains a significant source of morbidity following lower limb amputation and can substantially impair prosthetic use and quality of life. Targeted muscle reinnervation (TMR) has demonstrated effectiveness in reducing these complications by providing transected nerves with physiologic motor targets; however, its application may be limited by recipient motor nerve caliber mismatch, inadequate target availability, or unfavorable anatomy. Regenerative peripheral nerve interface (RPNI) offers an alter

miR-27b-3p reduces muscle fibrosis during chronic skeletal muscle injury by targeting TGF-βR1/Smad pathway.

BACKGROUND: Fibrosis is a significant pathological feature of chronic skeletal muscle injury, profoundly affecting muscle regeneration. Fibro-adipogenic progenitors (FAPs) have the ability to differentiate into myofibroblasts, acting as a primary source of extracellular matrix (ECM). the process by which FAPs differentiate into myofibroblasts during chronic skeletal muscle injury remains inadequately explored. METHOD: mouse model with sciatic nerve denervated was constructed and miRNA expression profiles between the mouse model and uninjured mouse were analyzed. qRT/PCR and immunofluorescence

Measures of bulbar and spinal motor function, muscle innervation, and mitochondrial function in ALS rats.

Symptom onset in amyotrophic lateral sclerosis (ALS) may occur in the muscles of the limbs (spinal onset) or those of the head and neck (bulbar onset). Most preclinical studies have focused on spinal symptoms, despite the prevalence of and increased morbidity and mortality associated with bulbar disease. We measured lick rhythm and tongue force to evaluate bulbar disease in the SOD1-G93A rat model of familial ALS. Body weight and grip strength were measured concomitantly. Testing spanned the early (maturation), middle (pre-symptomatic), and late (symptomatic and end-stage) phases of the diseas

Acute Unilateral Masseter Muscle Paralysis Caused by Pontine Infarction.

In the present report, we discussed the case of a 57-year-old man with unilateral masticatory muscle weakness, nystagmus, skew deviation and facial hypesthesia due to pontine tegmental infarction. Trigeminal motor neuropathy attributed to brain infarction is very rare. Brain magnetic resonance imaging revealed a small dot-like infarction lesion in the pontine tegmentum. Masticatory muscle weakness was confirmed by an electrophysiological study performed on the day after admission in which there was an incomplete interference pattern without spontaneous denervation activity, suggesting that the

Cytokine Signaling in Skeletal Muscle Wasting.

Skeletal muscle wasting occurs in a variety of diseases including diabetes, cancer, Crohn's disease, chronic obstructive pulmonary disease (COPD), disuse, and denervation. Tumor necrosis factor α (TNF-α) is involved in mediating the wasting effect. To date, a causal relationship between TNF-α signaling and muscle wasting has been established in animal models. However, results from clinical trials are conflicting. This is partly due to the fact that other factors such as TNF-like weak inducer of apoptosis (TWEAK) and interleukin 6 (IL-6) are also involved in skeletal muscle wasting. Because mus

The role of embryonic motoneuron transplants to restore the lost motor function of the injured spinal cord.

Spinal cord injury or disease result in the loss of critical numbers of spinal motoneurons and consequentially, in severe functional impairment. The most successful way to replace missing motoneurons is the use of embryonic postmitotic motoneuron grafts. This method may also at least partially restore integrity of the injured spinal cord. It has been shown that grafted motoneurons survive, differentiate and integrate into the host cord and many of them are able to reinnervate the denervated muscles. If grafted motoneurons are provided with a conduit (e.g. reimplanted ventral root) the grafted

Atrophic and regenerative changes in rabbit mimic muscles after lidocaine and bupivacaine application.

Destruction and denervation atrophy in skeletal muscles caused by the injection of local anaesthetics was investigated by injecting lidocaine or bupivacaine around the rabbit facial nerve to produce facial paralysis. Animals were then sacrificed at 2, 4, 6, and 8 weeks post-injection, and changes in mimic muscle tissue were assessed at each stage by light microscopy and electron microscopy. Atrophic changes were observed at 2-6 weeks after injection, and regeneration started at 6-8 weeks. Compared to bupivacaine, lidocaine caused more dramatic atrophic changes and was associated with slower mu

Thbs1 regulates skeletal muscle mass in a TGFβ-Smad2/3-ATF4-dependent manner.

Loss of muscle mass is a feature of chronic illness and aging. Here, we report that skeletal muscle-specific thrombospondin-1 transgenic mice (Thbs1 Tg) have profound muscle atrophy with age-dependent decreases in exercise capacity and premature lethality. Mechanistically, Thbs1 activates transforming growth factor β (TGFβ)-Smad2/3 signaling, which also induces activating transcription factor 4 (ATF4) expression that together modulates the autophagy-lysosomal pathway (ALP) and ubiquitin-proteasome system (UPS) to facilitate muscle atrophy. Indeed, myofiber-specific inhibition of TGFβ-receptor

Effects of brain-derived neurotrophic factor on motor dysfunction in wobbler mouse motor neuron disease.

Brain-derived neurotrophic factor (BDNF) has been shown to promote the survival of developing motor neurons in vitro and to rescue motor neurons from axotomy-induced cell death in vivo. In this study, we examined the effects of exogenous BDNF on the progression of wobbler mouse motor neuron disease (MND). After clinical diagnosis at age 3 to 4 weeks, 20 affected mice received subcutaneous injections of recombinant human BDNF (5 mg/kg, n = 10) or vehicle (n = 10), three times a week for 4 weeks. In a separate experiment done to conduct a histometric analysis of the C-5 and C-6 ventral roots and

Electromyographic detection of paraspinal muscle metastasis. Correlation with magnetic resonance imaging.

Electromyographic (EMG) examination demonstrating marked segmental compromise of the posterior primary ramus distal to the spinal root with relative sparing of the anterior ramus may be the earliest objective evidence of paraspinal muscle metastasis. Antecedent studies are often initially normal, failing to disclose the underlying cause of back pain. Although paraspinal muscle metastasis has been histopathologically demonstrated at postmortem, attempts to image the suspected malignancy with computed tomography have been unsuccessful because the tumor in muscle remains isodense. This study repo

Surface electrical stimulation for facial paralysis is not harmful.

INTRODUCTION: Does electrical stimulation (ES) of denervated muscles delay or prevent reinnervation, or increase synkinesis? In this retrospective study we evaluate the outcome, with and without ES, of patients with acutely denervated facial muscles. METHODS: The effect of ES was analyzed in two experiments. In the first experiment, 39 patients (6 with home-based ES, median 17.5 months) underwent facial nerve reconstruction surgery. Time to recovery of volitional movements was analyzed. The second experiment involved 13 patients (7 with ES, median 19 months) during spontaneous reinnervation.

Botulinum toxin therapy for neurologic disorders.

In the last 20 years, the therapeutic uses of botulinum toxin, a potent neurotoxin, have been investigated. The agent produces chemical denervation of muscle, thereby causing atrophy and weakness. Studies have shown that injection of this agent is an effective therapy for focal dystonias, particularly blepharospasm, hemifacial spasm, and torticollis. Investigation continues into the role of botulinum toxin in the treatment of anismus, detrusor-sphincter dyssynergia, writers' cramp, and other disorders in which focal weakening of selected muscles could be useful.

Altered terminal Schwann cell morphology precedes denervation in SOD1 mice.

In mice that express SOD1 mutations found in human motor neuron disease, degeneration begins in the periphery for reasons that remain unknown. At the neuromuscular junction (NMJ), terminal Schwann cells (TSCs) have an intimate relationship with motor terminals and are believed to help maintain the integrity of the motor terminal. Recent evidence indicates that TSCs in some SOD1 mice exhibit abnormal functional properties, but other aspects of possible TSC involvement remain unknown. In this study, an analysis of TSC morphology and number was performed in relation to NMJ innervation status in m

The reorganisation of motor units in different motor neuron disorders.

A study was made of the degree and sequence of neurophysiological changes during motor unit reorganisation in motor neuron disease (MND), spinal muscular atrophy (SMA), and prior polio. Concentric needle EMG was used in conjunction with our own computerized EMG-LAB system. Motor unit action potential (MUAP) parameters were measured in 543 muscles on weak and maximum effort. MUAP amplitude and area were found to increase in the early stages of damage, declining to normal or subnormal values in the course of the disease. It was concluded that in MND there is a pathological sequence: denervation-
