Necrotizing fasciitis secondary to Wohlfahrtiimonas chitiniclastica in the absence of myiasis.

Wohlfahrtiimonas chitiniclastica is a rare Gram-negative bacillus associated with fly larvae, myiasis, chronic wounds, and severe polymicrobial infection. We report a 71-year-old woman with diabetes, peripheral neuropathy, severe peripheral vascular disease, and previous contralateral above-knee amputation who presented with septic shock secondary to an infected right hallux ulcer. Radiographs demonstrated soft-tissue gas extending into the distal calf. Emergency exploration revealed extensive necrosis and dishwater pus tracking proximally along the flexor hallucis longus tendon, consistent wi…

Ultrasound diagnosis of cutaneous furuncular myiasis in a nonendemic area.

Cutaneous myiasis is an uncommon diagnosis in nonendemic regions and may be overlooked in returning travelers with nonspecific dermatologic complaints. We present a case of cutaneous furuncular myiasis in a 14-year-old female who presented to the emergency department with a neck mass 3 months after returning from Costa Rica after failed treatment with antibiotics and ivermectin. Ultrasonography showed an ovoid structure with a hyperechoic core with visible segmentations and surrounding hypoechoic rim; dynamic movement of the larva was not observed. Color Doppler demonstrated flow within the st…

Subungual Myiasis Presenting to the Emergency Department: A Case Report.

INTRODUCTION: Subungual myiasis is an infection under the fingernail or toenail caused by an infestation of fly larvae. It is rarely reported internationally, with only one previously reported case in the United States. CASE REPORT: An 80-year-old female with a history of polyneuropathy, peripheral artery disease, peripheral venous insufficiency, and chronic bilateral lower extremity edema presented to the emergency department (ED) after staff at her skilled nursing facility noted what appeared to be several whitish-colored maggots moving under the nail of the patient's left great toe. Thorou…

Gasterophilus: A Rare Cause of Cutaneous Myiasis in the United States.

Cutaneous myiasis is most commonly seen in travelers returning to the United States. There are rare reports of US-acquired human myiasis, with Cuterebra larvae being the most common causative organism. Gasterophilus, also known as the horse botfly, however, rarely infests human hosts and typically presents as cutaneous migratory myiasis, unlike this case. We report a cutaneous furuncular myiasis due to Gasterophilus larvae in the neck and cheek of a 4-year-old female after visiting a campground in northern New York.

Psychodid flies and their implicated role in human myiasis and pseudomyiasis.

Several psychodid flies are commonly associated with human-inhabited environments and have been increasingly implicated in cases of human myiasis. However, the basic biology of psychodid larvae is not well-suited for survival in the human intestinal or urogenital tract, making true, prolonged myiasis unlikely. In this review, we performed a systematic literature review of published cases of purported myiasis caused by psychodid flies, their identification, associated clinical findings, and treatment. We also discuss the anatomy and lifecycle of psychodid flies in relation to their purported ab…

A rare case report of penile myiasis in a catheterized patient.

INTRODUCTION AND IMPORTANCE: Penile myiasis is the infestation of the penile area with larvae of certain fly species, especially diptera. It is rare but can be a serious parasitic infestation that typically occurs in individuals with poor hygiene or those living in unsanitary conditions. Due to the non-specific and vague clinical presentation, it holds diagnostic difficulty. PRESENTATION OF CASE: A 76-year-old male with a prior history of benign prostatic enlargement, diabetes mellitus, and hypertension presented with a painless penile wound and sanguineous discharge. Debridement of the wound…

Myiasis of the exenterated orbital cavity: Case report and video.

PURPOSE: We present an unusual case of myiasis involving the orbital cavity. OBSERVATIONS: Our patient had undergone orbital exenteration one month prior to presentation. Post-operatively, he presented with multiple, highly mobile larvae in the orbital cavity. The species was identified to be Lucilia sericata, known commonly as the blowfly. The maggots were removed mechanically. CONCLUSION AND IMPORTANCE: The orbital cavity subsequently healed rapidly without sequalae. This raised the question as to whether L. sericata maggots may have had beneficial effects to the healing process.
