Incidence and Predictors of Reflux Esophagitis Following Per-oral Endoscopic Myotomy: A Two-Year Prospective Study.

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) is a highly effective treatment option for achalasia cardia. However, lack of an anti-reflux mechanism predisposes patients to gastroesophageal reflux disease (GERD). This study was undertaken to assess the incidence of reflux esophagitis two years post POEM and to identify predictive factors associated with GERD development. METHODS: Consecutive patients who underwent POEM at our institution between July 2021 and December 2021 were prospectively studied to assess the incidence and predictors of GERD. Subjective GERD was evaluated using a…

Contractility patterns on functional lumen imaging probe prior to peroral endoscopic myotomy predict outcomes in achalasia.

BACKGROUND AND AIMS: Functional lumen imaging probe (FLIP) assessment of esophagogastric junction (EGJ) opening confirms suspected achalasia. However, the importance of the contractile response (CR) in the esophageal body to volumetric distension in established achalasia is unclear. This study evaluated the value of CR on FLIP before peroral endoscopic myotomy (POEM) to predict post-POEM esophageal emptying on timed barium esophagram (TBE). METHODS: Patients with types 1 and 2 achalasia who underwent FLIP at a tertiary center from March 2020 to June 2024 were identified. FLIP was interpreted …

Association between preoperative pan-esophageal pressurization and peristalsis recovery after per-oral endoscopic myotomy in achalasia.

BACKGROUND: Per-oral endoscopic myotomy (POEM) has become the mainstream treatment for achalasia. Previous studies have found that some patients with achalasia might experience partial recovery of esophageal peristalsis after POEM. However, the clinical significance of this recovery and the associated factors remain to be further elucidated. METHODS: This was a multicenter retrospective cohort study conducted by four academic hospitals in China. Patients diagnosed with achalasia who underwent POEM between 2020 and 2025 were included. Demographics, Eckardt score, gastroesophageal reflux diseas…

Pulmonary Mycobacterium fortuitum Infection Associated With Achalasia Treated With Peroral Endoscopic Myotomy Alone: A Case Report.

Pulmonary infection caused by Mycobacterium fortuitum is uncommon and often occurs in patients with underlying conditions such as achalasia. We report a 50-year-old man with achalasia who developed persistent pulmonary consolidation associated with M. fortuitum. His respiratory symptoms fluctuated in parallel with gastrointestinal symptoms, suggesting chronic aspiration. Histopathological findings and sputum culture supported the diagnosis of nontuberculous mycobacterial disease. Peroral endoscopic myotomy (POEM) was performed to treat achalasia, resulting in marked clinical and radiological i…

Short Versus Standard Peroral Endoscopic Myotomy for Esophageal Achalasia: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) has become the treatment of choice for achalasia. However, the impact of myotomy length on POEM outcomes remains unclear. We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing the outcomes of short versus standard-length myotomy in achalasia patients. METHODS: We conducted a comprehensive search in PubMed, Embase, and Web of Science databases to identify all RCTs that compared POEM using short myotomy with standard myotomy. The primary outcomes were clinical success and post-operative gastroes…

Per-oral Endoscopic Myotomy (Z-POEM): An Effective Treatment for Zenker's Diverticulum with Long-Term Results.

Zenker's diverticulum (ZD) arises from impaired cricopharyngeal (CP) muscle distensibility during deglutition (swallowing), leading to a clinical presentation characterised by dysphagia, regurgitation, aspiration, cough, and potential weight loss. The definitive treatment for ZD is a CP myotomy. Traditionally, open surgical approaches (transcervical diverticulectomy, diverticulopexy, or diverticular inversion) with or without concomitant CP myotomy and rigid endoscopic techniques utilizing stapling or CO2 laser therapy have been employed. However, these interventions are often associated with …

First Description of Redo Gastric Per-Oral Endoscopic Myotomy for Refractory Gastroparesis: Technical Conduct and Outcomes.

Introduction: Gastroparesis is a progressive disease that may require endoscopic or surgical intervention, such as gastric per oral endoscopic myotomy (G-POEM), when conservative measures are ineffective or not tolerated. Limited data exist on redo G-POEM safety and effectiveness. We describe outcomes of patients undergoing repeat G-POEM for refractory gastroparesis. Methods: A retrospective review of patients who underwent redo G-POEM at our center from 2008 to 2024. Data included demographics, gastroparesis etiology, previous treatments, Gastroparesis Cardinal Symptom Index (GCSI), gastric e…

Elevated preoperative lower esophageal sphincter pressure predicts improved clinical outcomes after per oral endoscopic myotomy (POEM).

BACKGROUND: Although per oral endoscopic myotomy (POEM) has shown to be beneficial for the treatment of achalasia, it can be difficult to predict who will have a robust and long-lasting response. Historically, it has been shown that higher lower esophageal sphincter pressures have been associated with poorer responses to alternative endoscopic therapies such as Botox therapy and pneumatic dilation. This study was designed to evaluate if modern preoperative manometric data could similarly predict response to therapy after POEM. METHODS: This was a retrospective study of 237 patients who underw…

Peroral endoscopic myotomy for spastic esophageal dysmotility among opioid users: a multicenter propensity score matching study.

BACKGROUND AND AIMS: Opioid-induced esophageal dysfunction (OIED) often presents as spastic esophageal disorders (SEDs) and esophagogastric junction outflow obstruction (EGJOO). The aim of this study was to evaluate and compare clinical outcomes of peroral endoscopic myotomy (POEM) for SEDs and EGJOO among opioid users and nonusers. METHODS: This propensity score (PS) matching study included consecutive opioid users and nonusers who underwent POEM for SEDs and EGJOO between January 2018 and September 2022. The following covariates were used for the PS calculation: age, sex, duration of sympto…

Preoperative manometric data can predict symptom improvement after per oral endoscopic myotomy for treatment of achalasia.

BACKGROUND: While per oral endoscopic myotomy (POEM) has been shown to be efficacious in the treatment of achalasia, it can be difficult to predict who will have a robust and durable response. Historically, high lower esophageal sphincter pressures have been shown to predict a worse response to endoscopic therapies such as botox therapy. This study was designed to evaluate if modern preoperative manometric data could predict a response to therapy after POEM. METHODS: This was a retrospective study of 144 patients who underwent a POEM at a single institution by a single surgeon over an 8-year …

Single-Versus Multiple-Dose Antimicrobial Prophylaxis in Peroral Endoscopic Myotomy: A Randomized Controlled Study (SMAPP Trial).

Infections are uncommon after peroral endoscopic myotomy (POEM) procedure. However, prophylactic antibiotics are routinely administered for variable duration during peri-operative period. In this study, we aimed to determine the difference in the rate of infections between single (SD-A) and multiple doses (MD-A) antibiotic prophylaxis groups. The study was a prospective, randomized, non-inferiority trial conducted at a single tertiary care centre from Dec 2018 to Feb 2020. Eligible patients undergoing POEM were randomized into SD-A and MD-A groups. SD-A group received one dose of antibiotic (I…

Per-oral Endoscopic Myotomy Biopsies of Achalasia Patients Reveal Schwann Cell Depletion in the Muscularis Propria.

Achalasia is a neurodegenerative condition resulting in abnormal lower esophageal sphincter relaxation and impaired upstream esophageal body peristalsis.1 The pathophysiology and natural history of achalasia remain unclear, and evaluation of the histopathogenesis of achalasia has traditionally been challenging because the esophageal wall muscularis propria is not typically accessible via routine endoscopic biopsies.
