Assessing social determinants of health needs in newly diagnosed gynecologic oncology patients and their relationship with social support and stress levels.

OBJECTIVE: Social determinants of health (SDoH) account for 80% of health outcomes and, with social support and perceived stress, shape treatment experiences and quality of life in gynecologic cancer. This study characterized the prevalence of unmet SDoH needs among patients with newly diagnosed gynecologic cancer and examined associations of SDoH burden with social support and perceived stress. METHODS: Patients with newly diagnosed gynecologic cancer prospectively completed a PRAPARE-based survey assessing 11 SDoH domains, perceived stress, and social support. Logistic regression examined a

Do We Need Body Composition Analysis in Patients with Obesity? A Rethinking of Body Composition Assessment in Preclinical and Clinical Obesity.

BACKGROUND: Recent consensus statements from the European Association for the Study of Obesity and the 2024 Lancet Obesity Commission have renewed the debate on how obesity should be defined and clinically characterized. Although both frameworks acknowledge the limitations of body mass index (BMI), they continue to rely largely on anthropometric measures, including BMI and waist circumference, for diagnosis and risk stratification. Body composition analysis (BCA), however, has received comparatively little attention. Obesity is a heterogeneous condition characterized not only by excess adiposi

Assessing palliative care needs in Swabia: a data-driven simulation framework for hospice and specialized outpatient palliative care demand.

BACKGROUND: The impact of demographic change has far-reaching consequences for the entire healthcare system. One of the areas particularly affected is palliative and hospice care. To meet the demand for palliative and hospice care services, adequate capacity planning in inpatient and outpatient settings is essential. However, the available capacities per one million inhabitants in Germany vary significantly within the individual regions. METHODS: For an improved assessment of care needs, we propose a simulation-based analysis, carried out both retrospectively for 2019 and prospectively for 203

Spirituality, symptom burden and palliative care needs assessed by the Palliative Outcome Scale in those with cancer: a cross-sectional study.

BACKGROUND: Understanding symptom prevalence and palliative care needs is essential to advancing palliative care research in South America. This study aimed to assess these outcomes using the Palliative Outcome Scale (POS) and explore their relationship with the self-rated importance of spirituality. METHODS: A cross-sectional study was conducted among adult persons with cancer receiving palliative care at an outpatient clinic in a Brazilian public hospital. Data were collected through structured interviews using the POS, which assesses physical and psychosocial symptoms, communication, pract

Disease Investigation in Norwegian Killer Whales (Orcinus orca): Challenges and Needs for Improving Free-Living Cetacean Health Assessments.

Knowledge of the occurrence and etiology of cetacean diseases remains limited in northern areas, including in the Arctic and sub-Arctic. In November 2023, three killer whales (Orcinus orca) were observed in northern Norway exhibiting abnormal behavior, including inability to swim upright, loss of directional control, short and shallow dives, and lethargy. These unusual cases occurred within a 2-wk period and in the same area, suggesting a common underlying condition. However, due to the lack of diagnostic biologic samples, the cause of disease remains speculative. Herein, we discuss possible d

Evaluation of early-death predictive factors and assessment of the benefits-risks ratio among patients with vertebral metastases needing surgery.

INTRODUCTION: Preoperative survival prognostication is the main factor to consider when selecting patients who will benefit from metastatic spine surgery. Since patients' selection for surgery is not fully standardized, we aim to identify clinical, oncological and biological markers associated with poor post-operative prognosis. METHODS: A retrospective analysis was conducted in 105 patients referred to a specialized center for surgery of spinal metastasis between October 2020 and January 2022. Detailed clinical and biological variables were collected. Univariate and multivariate analyses wer

Addressing insanity in paedophilic disorder: The need for a cognitive approach for forensic assessment to correctly identify idiopathic, acquired and iatrogenic forms.

Assessing insanity in individuals with paedophilic disorder presents unique challenges, particularly due to the diverse aetiologies of the condition, which lead to varying symptomatology and behavioural manifestations. To address this complexity, in this article we overview the characteristics of three variants of paedophilic disorder - idiopathic, acquired, and iatrogenic - and highlight the key obstacles in differentiating these forms, resulting in inconsistent and, at times, biased judicial outcomes. Through the analysis of three real-life case studies, we illustrate how forensic evaluation

Optimal strategies for assessing and managing pain, agitation, and delirium in the critically ill surgical patient: What you need to know.

Pain, agitation, and delirium (PAD) are primary drivers of outcome in the ICU, and expertise in managing these entities successfully is crucial to the intensivist's toolbox. In addition, there are unique aspects of surgical patients that impact assessment and management of PAD. In this review, we address the continuous spectrum of assessment, and management of critically ill surgical patients, with a focus on limiting PAD, particularly incorporating mobility as an anchor to ICU liberation. Finally, we touch on the impact of PAD in specific populations, including opioid use disorder, traumatic

Assessing walking ability using a robotic gait trainer: opportunities and limitations of assist-as-needed control in spinal cord injury.

BACKGROUND: Walking impairments are a common consequence of neurological disorders and are assessed with clinical scores that suffer from several limitations. Robot-assisted locomotor training is becoming an established clinical practice. Besides training, these devices could be used for assessing walking ability in a controlled environment. Here, we propose an adaptive assist-as-needed (AAN) control for a treadmill-based robotic exoskeleton, the Lokomat, that reduces the support of the device (body weight support and impedance of the robotic joints) based on the ability of the patient to foll

Optimal assessment of upper body motion - Which and how many landmarks need to be captured for representing rigid body orientation?

In biomechanical studies, the thorax is often considered rigid. Since it's a well-known simplification, usually more than three markers are used to describe its movement in motion analyses. However, there is uncertainty about how many markers are advisable and which landmarks should be used. The results of the present study describe the expected error depending on the number of markers used. Furthermore, a recommendation is given for the landmarks with the least errors. This recommendation is valid for men and women as well as for different movements. The recommendations roughly reduce the err

A viewpoint describing the American Society of Transplantation rationale to conduct a comprehensive patient survey assessing unmet immunosuppressive therapy needs.

This viewpoint aims to "set the stage" and provide the rationale for the proposed development of a large-scale, comprehensive survey assessing transplant patients' perceived unmet immunosuppressive therapy needs. Research in organ transplantation has historically focused on reducing the incidence and impact of rejection on allograft survival and minimizing or eliminating the need for chronic immunosuppressive therapies. There has been less emphasis and investment in therapies to improve patient-reported outcomes including health-related quality of life and side-effects. Patient-focused drug de

Assessing Concordance Across Nonprofit Hospitals' Public Reporting on Housing as a Community Health Need in the Era of the Affordable Care Act.

Although the Affordable Care Act requires nonprofit hospital organizations to report how they identify and invest in community health needs, the utility of mandated reporting documents for tracking investments in the social determinants of health has been questioned. Using public reporting documents and focusing on housing as a social determinant of health, we describe how nonprofit hospital organizations in 5 communities with the highest rates of homelessness document needs and investments related to housing on their Community Health Needs Assessments, Implementation Strategies, and Schedule …

Assessing Health and Human Services Needs to Support an Integrated Health in All Policies Plan for Prince George's County, Maryland.

With evolving demographics and a changing health system landscape, the Prince George's County Council, acting as the County Board of Health, is considering its future policy approaches and resource allocations related to health and well-being. To inform this path forward, the authors of this study used primary and secondary data to describe both the health needs of county residents and drivers of health within the county, inclusive of the social, economic, built, natural, and health service environments. This study integrates these findings, an analysis of budget documents, and a review of pro
