Real-world use of Serial Clinical Observation in culture-proven early-onset sepsis: timing of recognition, treatment and retrospective comparison with the Neonatal Sepsis Calculator.

UNLABELLED: To assess the real-world performance of the Serial Clinical Observation (SCO) strategy in identifying and treating Early-Onset Sepsis (EOS) cases, and to compare it with the recommendations the Neonatal Sepsis Calculator (NSC) would have provided. This multicentre study analyzed prospectively culture-confirmed EOS cases caused by Group B Streptococcus (GBS) or Escherichia coli (2016-2022) in term and late-preterm infants managed with the SCO approach in Italy. We evaluated the proportion of infants assigned to routine care at birth and the time to antibiotics. NSC scores at 4, 12, …

Effect of probable neonatal sepsis on the development of infants in Eastern Uganda (ENON): a cohort study.

BACKGROUND: Neonatal sepsis is a common cause of mortality and morbidity during the first 28 days of life, particularly in resource-limited settings. Of those infants who survive sepsis, there is a lack of clarity as to the likelihood of developmental difficulties in these children. This study aimed to understand the impact of probable neonatal sepsis on the development of 2-year-old children in Uganda. METHODS: We assessed 404 infants aged between 18 and 36 months using the WHO's Global Scales for Early Development (GSED) Long Form (directly observed) and Short Form (parent report). This inc…

Antibiotic stewardship in the context of management of early onset neonatal sepsis-an evolving paradigm shift.

Early onset neonatal sepsis (EOS) is a rare but potentially life-threatening condition in term and late preterm infants. Clinicians face the challenge of promptly identifying and treating the small subset of neonates with true sepsis, while avoiding unnecessary intervention in the majority who remain unaffected. The clinical overlap between sepsis and normal physiological adaptation in the first 72 hours of life, combined with the limitations of current diagnostic modalities, has historically led to a culture of presumed safety, resulting in the overtreatment of many healthy infants with antib…

Lactate dehydrogenase to albumin ratio as an independent factor for 28-day mortality of neonatal sepsis.

Neonatal sepsis significantly contributes to infant mortality rates. The absence of predictive markers has hindered timely clinical intervention, leading to elevated mortality. This study aimed to assess the prognostic relevance of the lactate dehydrogenase (LDH)-to-albumin ratio (LAR) in neonatal sepsis. A retrospective examination was conducted on a cohort of 130 neonates diagnosed with sepsis. Admission laboratory data were gathered. The optimal threshold for LAR was established using receiver operating characteristic curve analysis. Both univariate and multivariate analyses were performed …

Colistin-resistant Klebsiella pneumoniae sepsis in neonates and infants: an alarming crisis in an Indian NICU.

Emergence of multidrug-resistant strain of Klebsiella pneumoniae has led to limited number of antimicrobial options for treatment. Even though outbreaks of colistin-carbapenem-resistant K. pneumoniae (CCRKp) are evolving, reported literature is scarce. This is a retrospective case series of three infants who acquired CCRKp sepsis during an outbreak in a Level III neonatal intensive care unit (NICU) of a tertiary care centre in Northern India. Data were retrieved from medical records of patients. Three babies developed new onset clinical worsening during an outbreak of sepsis. All had respirato

MicroRNA Profiling as a Novel Tool in the Diagnostics of Late-Onset Neonatal Sepsis: A Scoping Review.

Background/Objectives: The incidence of late-onset sepsis (LOS) has increased with improved survival rates in premature infants. Blood culture, the diagnostic "gold standard", requires at least 36-72 h for results, leading to empiric antibiotic use and potential resistance. MicroRNAs (miRNAs) have emerged as promising biomarkers for sepsis in adults, but their role in neonatal LOS remains unclear. The aim of this scoping review is to identify the miRNA expression profiles of bacterial LOS in neonates. Methods: A scoping review of the literature was performed between 1 November 2023 and 31 Dece

Association Between Vitamin D Deficiency and Sepsis in Term Neonates: A Case-Control Study.

Background Neonatal sepsis remains a major global health challenge, contributing significantly to morbidity and mortality in term neonates. Despite advancements in neonatal care, the early identification and prevention of sepsis continue to pose challenges. Emerging research suggests that vitamin D, traditionally recognized for its role in bone health, also plays a crucial role in immune regulation. Objectives To evaluate and compare serum vitamin D levels in term neonates with sepsis and without sepsis. Methods The study included 60-term neonates, with 30 neonates diagnosed with sepsis as cas…

Platelet transfusions in a murine model of neonatal polymicrobial sepsis: Divergent effects on inflammation and mortality.

BACKGROUND: Platelet transfusions (PTxs) are often given to septic preterm neonates at high platelet count thresholds in an attempt to reduce bleeding risk. However, the largest randomized controlled trial (RCT) of neonatal transfusion thresholds found higher mortality and/or major bleeding in infants transfused at higher thresholds. Using a murine model, we investigated the effects of adult PTx on neonatal sepsis-induced mortality, systemic inflammation, and platelet consumption. STUDY DESIGN AND METHODS: Polymicrobial sepsis was induced via intraperitoneal injection of cecal slurry preparat…

A CLINICAL CASE OF ARTERIAL THROMBOSIS IN A PREMATURE BOY WITH NEONATAL SEPSIS AND HIGH RECTAL ATRESIA.

Occlusion of the venous or arterial vessels in childhood is rather rare but dangerous complication. Occurrence of neonatal thrombosis is 0.5 per 10 000 of live newborns. Promoting factors include congenital defects of coagulation, maternal diabetes, neonatal sepsis, necrotic enterocolitis, asphyxia, and metabolic diseases. More than 90 % cases of neonatal thrombosis are associated with catheterization (umbilical arterial or venous, other central venous lines). Acute arterial thrombosis caused by a peripheral venous catheter is very rare complication, but it can result in severe consequences. T…
