Vascular tumors represent only a sliver of all tumors affecting the mediastinum, but they pose diagnostic challenges due to significant overlap among entities, ever-evolving classification schemes, and the exquisite rarity of some of the entities not only in the mediastinum but in pathology practice as a whole. Most of the vascular tumors are better known to the practice of soft tissue pathology, from which some of the knowledge of clinical behavior can be extrapolated. For example, the stratification of epithelioid hemangioendothelioma (EHE) into two biologically separate categories has effec

Bone or soft tissue sarcoma of the extremity represents a varied group of mesenchymal tumors, with an extensive range of histopathology, epidemiology, and anatomical involvement. Many of these tumors grow to involve the adjacent major neurovascular bundles, leading to a paradigm shift of management between limb salvage and limb amputation. MRI is the gold standard imaging modality for assessing neurovascular involvement, primarily on T2-weighted fat suppressed (T2 W FS) axial and T1-weighted (T1 W) axial sequences. The differential imaging pattern or appearance of peritumoral edema and tumor h

Round cell sarcomas represent a continuously evolving category of malignant mesenchymal tumors, currently encompassing 5 main tumor subtypes: Ewing sarcoma, Capicua Transcriptional Repressor (CIC)-rearranged sarcoma, sarcomas with BCL6-corepressor (BCOR)-genetic abnormalities, POZ/BTB and AT hook containing zinc finger 1 (PATZ1)-rearranged sarcoma, and nuclear factor of activated T cells 2 (NFATC2)-rearranged sarcoma. These tumors show some degree of histomorphologic overlap, which, coupled with their infrequency, may render their diagnosis challenging for the pathologist. Furthermore, althoug

BACKGROUND: Soft tissue sarcomas (STSs) are a heterogeneous group of tumors. Wide surgical resection is standard, often combined with neoadjuvant chemotherapy, radiotherapy, or both. Studies have shown the predictive value of tumor necrosis in bone sarcoma (BS); however, the role of necrosis in STS after neoadjuvant therapies is still unclear. This study aimed to investigate the role of chemo- and radiotherapy in the formation of tumor necrosis and to evaluate the influence of tumor necrosis on overall survival and local recurrence-free survival. Data from BS patients and patients who did not

Bizarre parosteal osteochondromatous proliferation (BPOP) is also known as Nora's disease. It is a benign lesion. Even though recent studies showed probable neoplastic etiology, the exact cause is unknown. BPOP commonly involves small bones of hands and feet. This condition is rare and very few cases are reported. In this report, two cases are presented with clinical, radiological, and histopathological findings. The first case was a 38-year-old female presented with 3-year history of mild painful swelling in the left middle finger and the second case was a 28-year-old male with the left leg s

IMPORTANCE: Treatment options for patients with unresectable and/or metastatic dedifferentiated liposarcoma (DDLPS) are limited. New drugs are required.
OBJECTIVE: To assess whether cabazitaxel demonstrated sufficient antitumor activity in patients with metastatic or inoperable locally advanced DDLPS to justify further investigation in a phase 3 setting.
DESIGN, SETTING, AND PARTICIPANTS: This international multicenter, open-label single-arm phase 2 trial was conducted at 10 institutions in 4 European countries from March 2015 to March 2019. Eligible patients had to have metastatic or locall

CASES: Two women presented with newly growing callosities beneath the first and second metatarsal heads, initially believed to reflect gastrocnemius tightness and plantar plate pathology. In another man, swelling at the posterolateral aspect of the heel was mistaken for a Haglund deformity. Subsequent imaging of each patient led to delayed diagnosis of extraskeletal osteochondroma (ESO). Surgical excision resolved symptoms in all 3 with no recurrence over 12 months later.
CONCLUSIONS: Whenever bony prominences newly develop in soft tissues of the foot, ESO should be suspected and appropriate

Extraskeletal osteosarcoma (ESOS) is an uncommon malignant mesenchymal tumour deriving from soft tissues or visceral organs without any evidence of a primary bone osteosarcoma. Although ESOS has been mainly reported in the extremities, it can also occur in unusual locations such as the colorectal region. A 58-year-old man with a 3-month history of dyspepsia, constipation and soreness in the left upper abdominal quadrant was admitted to hospital. Computed tomography and magnetic resonance imaging demonstrated a huge, irregular, mixed cystic and solid, partially calcified mass measuring 116mm×14

STUDY DESIGN: Between 1982 and 1997, the authors treated 32 patients with sciatica who subsequently were found to have a tumor along the extraspinal course of the sciatic nerve.
SUMMARY OF BACKGROUND DATA: Extraspinal compression of the sciatic nerve by a tumor is a rare cause of sciatica. Signs and symptoms overlap those of the more common causes of sciatica (i.e., herniated disc and spinal stenosis).
OBJECTIVE: To characterize the unique clinical presentation of these patients and to formulate guidelines that may lead to early diagnosis.
METHODS: All pertinent clinical data and studies we

Postoperative treatment of osteosarcoma faces the dual challenges of tumor recurrence and extensive bone defect repair, necessitating the development of a synergistic strategy capable of simultaneously eliminating residual tumor cells and promoting bone regeneration. Existing prosthetic implants are often limited by their singular functionality, leading to issues such as high recurrence rates and non-union. This study innovatively constructs a functional titanium alloy scaffold system synergistically driven by a biodegradable "shell-like" triboelectric nanogenerator (BS-TENG). The BS-TENG effi

Melorheostosis is a rare non-hereditary sclerosing bone dysplasia which predominantly affects the appendicular skeleton. Although melorheostosis is typically recognized as an osseous lesion, associated soft-tissue components have been reported. Advanced imaging with MRI may allow for more complete evaluation of these soft tissue components; however, there is little information regarding their MRI characteristics which may lead to confusion with malignant processes. We present a case of melorheostosis in a 32-year-old woman with an associated paraarticular enhancing soft tissue mass and emphasi

Primary Ewing sarcoma (ES) of the kidney is rare. We describe the clinicopathologic features of primary renal ES with emphasis on gene fusion partners. A multi-institutional study was conducted to obtain clinicopathologic data on primary ES of the kidney. All tumors with available tissue underwent NGS to determine fusion partners. Twenty-four patients (8 male, 16 female) were identified. Mean age was 33.2 (±12.3). Mean tumor size was 10.5 cm (±4.2). Clinical presentation was available in 21 patients: flank/abdominal pain (13, 61.9%), hematuria (4, 19%), mass (2, 9.5%), hypertension (1, 4.8%),

INTRODUCTION: Bone gigantic cell tumours (BGCTs) are locally aggressive benign tumours, which can lead to significant functional impairment due to extensive bone destruction and soft tissue invasion and typically occur in the meta-epiphyseal region of long bones with the distal radius being the third most common site. Advanced BGCTs of the wrist present a significant challenge for the medical staff due to their occurrence in subarticular bone. Bone reconstruction of the hand with 3D assisted engineering has been described with promising results. In this article, we present a novel approach for

Although rare in everyday practice, malignancies that classically arise from bone or cartilage have been reported to arise de novo in various soft tissues in the body, resulting in a diagnostic challenge for the clinician, radiologist, and pathologist. Differential diagnoses of bone tumors often depend on anatomic location of the lesion. For example, the classic location of osteosarcoma is in the metaphysis of long bones about the knee. Histologically osteosarcoma is characterized by tumor cells that directly produce osteoid, bone, or cartilaginous matrix. In extraskeletal osteosarcoma, the cl

PURPOSE: To evaluate the impact of (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET) on management of patients with apparently isolated plasmacytoma.
METHODS AND MATERIALS: Twenty-one patients with apparently solitary plasmacytoma who underwent FDG-PET for staging or restaging were identified from a central PET database. They were either candidates for or had received definitive radiation therapy (RT).
RESULTS: Seventeen patients had initial staging scans for bone (n = 11) or soft tissue (n = 6) plasmacytomas, and 11 had PET scans after RT. Only 1 of 14 known untreated sites of

The CD5 antigen is a T-cell associated marker that is also usually expressed by two B-cell neoplasms, chronic lymphocytic leukemia/small lymphocytic lymphoma and mantle cell lymphoma. We observed CD5 antigen expression in a subset of cases of intravascular large B-cell lymphoma (IVLBL), and we report here five cases. The patients, two men and three women, ranged in age from 59 to 81 years. Biopsy specimens were obtained from kidney, lung, bone marrow, abdominal wall, and neck, the latter involving a lymphangioma. All of the cases had histologic features typical of IVLBL, with large and atypica

We describe three cases of primary low-grade B-cell lymphoma of the endometrium and contrast the histological, immunohistochemical, and molecular features with two examples of benign endometrial lymphoid infiltrates. The first case was an incidental finding in a curettage specimen, confirmed on a subsequent hysterectomy. The other two cases of lymphoma were incidental findings on hysterectomy procedures performed for prolapse and cervical dysplasia, respectively. All three lymphomas occurred in patients in their sixties; none formed gross tumors. Histologic examination revealed lymphoid nodule

BACKGROUND: The goal of this study was to evaluate outcomes after vascularized bone flap (VBF) reconstruction of oncologic bony extremity defects. A secondary goal was to compare union rates based on various insetting methods, including onlay, intermedullary, and intercalary.
METHODS: The authors conducted a retrospective review of consecutive patients who received an extremity reconstruction with a fibula flap after oncologic resection between 2001 and 2019.
RESULTS: The authors identified a total of 60 fibular VBFs in 55 patients (67% lower extremity, 33% upper extremity). The overall unio
