[Percutaneous "sandwich" endopyeloplasty technique: a new endourological measure for ureteropelvic junction obstruction].

OBJECTIVE: To report a new endourological technique-percutaneous "sandwich" endopyeloplasty for ureteropelvic junction obstruction (UPJO). METHODS: In the study, 71 patients with UPJO had endopyeloplasty by percutaneous balloon dilation before and after endopyelotomy, so called "sandwich" endopyeloplasty. The ureter stent was removed after 2 months and all the patients were followed up at least 12 months by ultrasound, intravenous urogram (IVU) or renal scintigraphy. RESULTS: There were 12 patients with primary UPJO, and 59 patients with iatrogenic UPJO after pyelolithotomy, pyeloplasty, ure

[Early calculogenesis caused by ureolytic infection of the double-J ureteral catheter].

Stone formation in the double J ureteral catheter is an uncommon complication that generally appears in patients that are predisposed to stone formation and persistent infection of the urinary tract. We report on a 29-year-old female patient in whom stone formation was observed in both ends of the catheter, less than 3 months after it had been placed. Urine culture was positive for P. mirabilis. Percutaneous nephrolithotomy was warranted in the treatment of the pyelic stone and for removal of the double J catheter. We underscore the importance of doing urine cultures and radiologic control in

The impact of transurethral ureteroscopy on the management of ureteric calculi.

Transurethral ureteroscopy has now been used in an attempt to retrieve ureteric calculi in 48 patients. Six stones were located in the upper ureter, 11 in the middle and 31 in the lower third of the ureter. Successful retrieval was achieved in 27 patients at the first attempt and in 4 at a second ureteroscopy. Of the 17 failures, 7 patients have required open ureterolithotomy, 3 percutaneous nephrolithotomy and the remainder passed their stones either after in situ disintegration or ureteric meatotomy. There have been no serious complications to date. Transurethral ureteroscopy should now be r

[Perioperative management of the patient with chronic spinal cord injury using bilateral diaphragmatic pacemaker. Clinical case].

Patients with chronic spinal cord injury suffer from a number of pathophysiological alterations that can lead to important morbidity and mortality in the perioperative period. The diaphragmatic pacemaker is a device that enables pulmonary ventilation in patients with high cervical cord injuries and provides them with a better quality of life when compared to mechanical ventilation. We present here the clinical case of a patient with chronic spinal cord injury who used a diaphragmatic pacemaker, and who was scheduled for percutaneous nephrolithotomy and double-J stent implantation. The anesthes

Urinary calculus containing gas.

The combination of gas in the upper urinary tract along with renal calculi has been reported in the setting of emphysematous renal infections, urinary tract instrumentation, and urinary-gastrointestinal fistulas. We present a case of a 46-year-old nondiabetic man with the isolated finding of a gas-containing renal calculus. The significant history in this case included recurrent urinary tract infections and primary hyperparathyroidism with persistent hypercalcemia despite previous parathyroid explorations. The patient was treated with oral antibiotics and rendered stone and infection free by p

Longitudinal Modeling of Non-Pharmacological Factors Related to Frequency, Severity and Duration in Both Migraine and Tension-Type Headaches.

PURPOSE: With the invention of miniature devices, it has been advised to apply less aggressive methods for the management of upper urinary tract stones, especially in children. In the recent years, ultra-mini percutaneous nephrolithotomy (UMP) has been used for the treatment of upper urinary tract stones in order to perform surgeries with less complications and more acceptable outcomes. Results reported from different medical centers have been promising. MATERIALS AND METHODS: Twenty-two children aged less than 8 years old with upper urinary stones sized between 10-20 mm underwent UMP. Inclus

Changing composition of staghorn calculi.

PURPOSE: The 2005 American Urological Association Guideline on the Management of Staghorn Calculi suggests that metabolic stones are uncommon in the composition of staghorn calculi. We determined the incidence and treatment outcomes of metabolic stones in patients with complete staghorn calculi compared to infection stones in those undergoing percutaneous nephrolithotomy. MATERIALS AND METHODS: We retrospectively analyzed all patients who underwent percutaneous nephrolithotomy for complete staghorn calculi between April 2005 and April 2010. Stone analysis was reviewed to classify stone type a

Analysis of the Value of Flexible Ureteroscopy in the Treatment of Renal Calculi.

OBJECTIVE: Kidney stones (renal calculi) are a prevalent medical condition, causing significant pain and discomfort to patients. The existing treatment options for kidney stones include extracorporeal shock wave lithotripsy (ESWL), percutaneous nephrolithotomy (PCNL), and flexible ureteroscopy. It is crucial to evaluate the effectiveness of different treatment modalities, including flexible ureteroscopy, to ensure optimal patient outcomes. This study aims to assess the effectiveness of flexible ureteroscopy in treating renal calculi and determine its value in managing this condition. METHODS:

Isolated renal aspergilloma.

Aspergilloma localised solely to the kidneys is very rare and poses diagnostic and therapeutic challenges. We present the case of a 35-year-old male with a background of urolithiasis, type II diabetes and chronic kidney disease, who was found to have isolated renal aspergilloma associated with obstructing uric acid renal tract calculi. After poor progress to clear the fungal infection and remaining stones with ureteroscopy, he was successfully treated by percutaneous nephrolithotomy (PCNL) to surgically remove fungal balls and calculi. Additionally, he was treated with peri-operative oral and

[Squamous cell carcinoma of the renal pelvis associated with renal stones in a patient with chronic renal failure: a case report and a review of the Japanese literature].

A case of squamous cell carcinoma of the left renal pelvis associated with chronic renal failure on hemodialysis is reported. The patient, a 59-year-old man, had undergone bilateral nephrolithotomy, in 1966, followed by right ureterolithotomy and bilateral percutaneous nephrolithotripsy, but residual stones existed. He suffered from left flak pain and fever, and computed tomography (CT) and magnetic resonance imaging (MRI) revealed left perirenal abscess in July 1994. Percutaneous drainage and antibacterial chemotherapy were performed, but his symptoms did not improve. Three months later, CT a

Results of percutaneous endoscopic ureterolithotomy compared to that of ureteroscopy.

Percutaneous endoscopic ureterolithotomy is not a well-known method for the treatment of impacted ureteral stones. The authors performed a retrospective study to compare the effectiveness of this procedure to ureteroscopy for the treatment of ureteral calculi. Impacted ureteral stones were removed in 93 patients by percutaneous endoscopic ureterolithotomy (Group 1). In 142 patients, ureteroscopy was performed for the treatment of the ureteral stones (Group 2). The same instruments were used to carry out percutaneous endoscopic ureterolithotomy as were used for percutaneous nephrolithotomy. Dur

Medullary sponge kidney: causes and treatments.

Medullary sponge kidney is a little known and little understood disease. A patient with a medullary sponge kidney may undergo decades of suffering in the form of infections and pain before any diagnosis is even made. When a diagnosis is made, it is more than likely to be an incidental finding from a test for another problem. However, on diagnosis there are a number of options available to treat this condition. These include non-invasive treatments such as drug therapy, diet or extracorporeal shock wave lithotripsy, to invasive therapies such as percutaneous nephrolithotomy, ureteroscopy and mo

Endoscopic combined intrarenal surgery for staghorn calculus in a horseshoe kidney with double collecting system: A case report.

Horseshoe kidney (HSK) with a duplicated collecting system and a bifid ureter is a unique surgical challenge. We report the case of a 43-year-old male patient with a left HSK, duplicated collecting system, bifid ureter, staghorn calculus in the lower moiety, and proximal ureteral stone. The procedure of mini-endoscopic combined intrarenal surgery (mECIRS) with mini-percutaneous nephrolithotomy (mini-PCNL) was successfully performed, resulting in minimal blood loss and no intraoperative complications. This case exemplifies the feasibility of employing mECIRS with a single-tract, tubeless mini-P

Robotic-assisted laparoscopic pyelolithotomy in a horseshoe kidney.

Nephrolithiasis is one of the most common indications for surgery in patients with a horseshoe kidney. Robotic-assisted surgery has become a staple in urologic practice, yet its application in stone management is largely undefined. We present a patient with a horseshoe kidney, who underwent a robotic-assisted laparoscopic pyelolithotomy (RPL) to treat a 3 cm stone burden. This procedure allowed for safe access that could not be obtained with percutaneous nephrolithotomy (PCNL) and stone removal without fragmentation, which would have been challenging with traditional laparoscopy. We advocate f

Upper Urothelial Tract Extraosseous Bone Formation: An Unexpected Finding and Differential Diagnostic Considerations.

Extraosseous bone formation of the upper urothelial tract is an unusual phenomenon with limited documentation in the uropathology literature, reported in only 2 clinical series of patients undergoing percutaneous nephrolithotomy for the management of renal stones. While speculations regarding the pathogenesis of this occurrence have been published, heterotopic ossification is still poorly understood. We report the finding of extraosseous bone formation in the renal pelvis of a 30-year-old male patient with a history of kidney stones. Histologic sections of the ureter and renal pelvis showed su

Mini Percutaneous Kidney Stone Removal: Applicable Technologies.

Percutaneous nephrolithotomy (PCNL) remains the treatment of choice for large and complex renal stones. The technological advances over the past several decades gave birth to different varieties of minimally invasive PCNLs, including the mini-PCNL, ultra-mini PCNL, super mini-PCNL, and micro-PCNL, with indications being extended to stones even larger than 20 mm. This article provides an update of all these available techniques of miniaturized PCNL along with its anatomic and physiologic impact. This should assist urologists in providing a personalized approach to the patient based on various p

Buckshot colic: utilizing holmium:yag laser for ureteroscopic removal of a bullet fragment within the proximal ureter.

Buckshot colic is a rare phenomenon, presenting as firearm-induced urinary tract obstruction. We present a case of gunshot-induced ureteral obstruction in a 49-year-old male, treated endoscopically with the holmium:YAG (holmium) laser. CT revealed a 1 cm bullet fragment within the left proximal ureter. A percutaneous nephrolithotomy was performed utilizing the holmium laser to fragment the metal into basket-retrievable pieces. At 4 month follow up the patient is without evidence of stricture. To our knowledge, this is the first reported utilization of the holmium laser to treat "buckshot colic

Ureteroscopic manipulation of stones in the ureter: four years experience.

Our experience of 223 ureteroscopies and stone manipulations is reported. Stone removal was successful in 82% of the cases whereof the stones were pushed back into the renal pelvis and subsequently managed with percutaneous nephrolithotomy or extracorporeal shock wave lithotripsy in 24%. For distal stones the success rate was 88%. Reasons for failure were acute obstruction of the ureter and previous surgery or irradiation to the pelvis. In six cases (4%) strictures requiring treatment developed secondary to ureteroscopy. Ureteroscopy can be a tedious and difficult procedure with a considerable
