Intraoperative neuromonitoring reduces vocal cord injury in open thyroid cancer surgery: results from a randomized controlled trial.

BACKGROUND: Intraoperative neuromonitoring (IONM) has been increasingly used in thyroid surgery, yet its clinical value remains controversial. OBJECTIVES: This randomized controlled trial aimed to evaluate the efficacy and safety of IONM in thyroid cancer surgery. METHODS: The standardized four-step monitoring protocol was used in the IONM group. Primary endpoints included RLN injury rates and postoperative voice function recovery. Secondary endpoints included surgical parameters (operation time, blood loss), complication rates, and oncological outcomes. Voice function was assessed using VHI

Intraoperative monitoring of somatosensory (SSEPs) and transcranial electric motor-evoked potentials (tce-MEPs) during surgical correction of neuromuscular scoliosis in patients with central or peripheral nervous system diseases.

PURPOSE: Combined intraoperative monitoring (IOM) of transcranial electric motor-evoked potentials (tce-MEPs) and somatosensory-evoked potentials (SSEPs) is safe and effective for spinal cord monitoring during scoliosis surgery. However, the literature data regarding the reliability of spinal cord monitoring in patients with neuromuscular scoliosis are conflicting and need to be confirmed. METHODS: We reviewed IOM records of 40 consecutive patients with neuromuscular scoliosis related to central nervous system (CNS) (29 pts) or peripheral nervous system (PNS) (11 patients) diseases, who under

Anaesthesia Considerations on Paediatric Neurosurgery.

Paediatric neurosurgery has seen significant increases and improvements because of advancements in technology and monitoring techniques. This type of surgery presents unique challenges to the anaesthesiology team because of the general characteristics of paediatric patients and the complexity of the procedures. Managing paediatric patients undergoing complex neurosurgery requires profound knowledge of age-related normal physiology and the principles of common paediatric neuroanaesthesia. This review focuses on updated information about various critical topics in paediatric neurophysiology, ble

Frontal brain activity assessment during basic life support skill acquisition and transfer in serious game based learning environments.

Basic Life Support (BLS) is a crucial life-saving skill, and the effectiveness of different training methods for acquiring and transferring BLS knowledge and skills remains an important area of investigation. Traditional methods, such as lectures, may be less engaging and lack the practical application necessary for optimal skill transfer. New training modalities, including serious games on tablets and in virtual reality (VR) environments, offer interactive experiences, enable personalized feedback, and repetitive practice, potentially enhancing the learning process. However, the effectiveness

Percutaneous continuous vagal stimulation provided superior neuromonitoring over the peroral electrode in transoral thyroidectomy.

BACKGROUND: Continuous intraoperative neuromonitoring (C-IONM) has been developed and used in open thyroidectomy to perceive imminent recurrent laryngeal nerve (RLN) injury, but has scarcely been reported in transoral endoscopic thyroidectomy vestibular approach (TOETVA) due to technical difficulty. This study aims to report the percutaneous C-IONM technology in TOETVA and compare it with the conventional peroral method to confirm its feasibility, safety, and effectiveness. METHODS: This prospective study included 102 consecutive patients who received TOETVA and standardized continuous vagal

Hearing preservation outcomes using direct cranial nerve eight and auditory brainstem response neuromonitoring in the resection of vestibular schwannomas.

OBJECTIVE: Advancements in microsurgical technique and technology continue to improve outcomes in patients with skull base tumor. The primary cranial nerve eight monitoring systems used in hearing preservation surgery for vestibular schwannomas (VSs) are direct cranial nerve eight monitoring (DCNEM) and auditory brainstem response (ABR), although current guidelines are unable to definitively recommend one over the other due to limited literature on the topic. Thus, further research is needed to determine the utility of DCNEM and ABR. The authors performed a retrospective cohort study and creat

Assessment of traumatic brain injury treatment guided by continuous monitoring of intracranial pressure and brain tissue oxygen partial pressure: A single-center pilot study.

Severe traumatic brain injury (TBI) is a leading cause of death and disability. Monitoring intracranial pressure (ICP) is recommended, but the data on the outcomes are conflicting. Adding continuous brain tissue oxygen partial pressure (PbtO2) monitoring may have some benefit but the OXY-TC suggested it did not improve 6-month neurological outcomes. This single-center pilot randomized controlled study aimed to evaluate whether adding PbtO2 monitoring was feasible and could improve the prognosis of severe TBI. The participants were randomized into either an ICP alone or an ICP + PbtO2 group for

Anesthetic considerations for scoliosis surgery in a patient with recessive severe/lethal form of osteogenesis imperfecta.

Osteogenesis imperfecta is a heterogeneous genetic disorder characterized by bone fragility, with disease ranging from mild fractures to death in utero. We describe a child with autosomal recessive osteogenesis imperfecta type VIII (severe or lethal phenotype), who successfully underwent posterior spinal fusion, was extubated on postoperative day 1 and discharged home 25 days later. Recently identified recessive forms of osteogenesis imperfecta are associated with severe/lethal phenotype. Special consideration is needed in scoliosis surgery, with challenges arising from prone positioning, neur

Assessment of Neurophysiological Parameters During Anterior Cervical Discectomy and Fusion and Their Correlation with Clinical Findings.

Background: In this study, we used intraoperative neurophysiological monitoring (IONM) during anterior cervical discectomy and fusion (ACDF). Rather than emphasizing its use for safety purposes, our goal was to evaluate how neurophysiological parameters change during surgery and their correlation with clinical findings. Methods: This study included 30 patients who underwent ACDF. Detailed neurological examination was performed together with manual muscle testing (MMT), the Numeric Pain Rating Scale (NPRS), and the Neck Disability Index (NDI) questionnaire. During surgery, somatosensory-evoked …

Auditory evoked responses in the management of acutely brain-injured children and adults.

In recent years, the role of auditory evoked responses (AERs) in the intensive care unit (ICU) setting has expanded dramatically for both pediatric and adult brain-injured patient populations. AERs have unique value in early identification and evaluation of peripheral auditory dysfunction that can result directly from head trauma or as a consequence of intensive medical therapy (such as prolonged intubation and ototoxic drugs). AERs can also be applied in evaluating and monitoring neurologic status during the acute period following a severe brain injury, similar to their common intraoperative

A DC-Sensitive Video/Electrophysiology Monitoring Unit for Long-Term Continuous Study of Seizures and Seizure-Associated Spreading Depolarization in a Rat Model.

There has been a long-term need for a low-cost, highly efficient, and high-fidelity epilepsy monitoring unit (EMU) suitable for synchronized multimodal home-cage monitoring of small-animal models of epilepsy and spreading depolarization. We present an accessible, scalable, highly space- and energy-efficient EMU capable of fulfilling chronic, continuous, synchronized, multiple-animal monitoring jobs. Each rig within the EMU can provide 16-channel high-fidelity, DC-sensitive biopotential recordings, head acceleration monitoring, voltammetry applications, and synchronized video recording on one f

Characteristics and effects of depth of anaesthesia on late motor responses.

OBJECTIVE: Late motor responses, such as F-waves and H-reflexes, may reflect excitability of specific spinal cord segments and motor pathway integrity, offering potential value during intraoperative neurophysiological monitoring (IONM). However, these responses are not routinely utilized. A better understanding of the effect of general anaesthesia on their characteristics, particularly across varying depths, is essential for integration into IONM. METHODS: Participants received propofol/remifentanil-based anaesthesia, targeted at BIS values of 30, 40, and 50. F-waves, H-reflexes and Tc-mMEPs …

Retroperitoneal and pelvic schwannoma/neurofibroma resection: surgical strategies and outcomes in a neurosurgical cohort.

BACKGROUND: Retroperitoneal and pelvic schwannomas and neurofibromas account about 10% of all retroperitoneal tumors. These tumors are almost invariably benign and slow growing. They are either asymptomatic or cause radicular or abdominal pain. The radiologic findings cannot distinguish schwannomas from other retroperitoneal neoplasms. To our knowledge, this study is the first neurosurgical series of this size to employ intraoperative electrophysiological monitoring during resection of schwannomas and neurofibromas arising in the retroperitoneal and pelvic regions. METHODS: A retrospective st

Noninvasive Intracranial Pressure Prediction Using a Multimodal Ultrasound-Based Hemispheric Modeling Strategy: A Prospective Dual-Center Study.

BACKGROUND: Accurate intracranial pressure (ICP) surveillance is a cornerstone of neurocritical care management, yet invasive monitoring still depends on neurosurgical expertise, specialized hardware, and continuous bedside resources-factors that restrict universal use even though insertion-related complications are relatively uncommon. Contemporary noninvasive ultrasound methods have limited predictive accuracy and seldom incorporate affected side information. We therefore preliminarily validated a multimodal, ultrasound-based hemispheric modeling approach that blends hemodynamic and structur

Intraoperative motor evoked potential in meningioma surgery: Diagnostic accuracy from institutional data and Meta-Analysis.

OBJECTIVE: We assessed the diagnostic accuracy of transcranial electrical stimulation motor evoked potentials (TES-MEP) in predicting immediate postoperative motor deficits in meningioma surgery. METHODS: A total of 319 patients who underwent intracranial meningioma resection with TES-MEP monitoring were retrospectively analyzed. The correlation of intraoperative TES-MEP changes with motor outcomes assessed on the day of surgery was examined. A systematic review and meta-analysis of TES-MEP studies, including mixed tumor types because of limited meningioma-only data, was conducted. RESULTS:
