This manuscript describes the results of invasive testing of renal artery and arteriolar reactivity in two patients using a panel of endothelium-dependent and -independent agents including radiographic contrast medium. We found that the renal artery and microcirculation dilate in response to graded acetylcholine infusions and to bolus nitroglycerin infusion; the renal microcirculation dilates in response to papaverine but constricts after adenosine and after radiographic contrast medium. Future indications for this testing are briefly discussed.

Two patients admitted to hospital because syncope and chest pain are presented. In both patients, hyperventilation test caused severe myocardial ischaemia (ST segment elevation) and sudden development of presyncopal sustained ventricular tachycardia which immediately responded to intravenous nitroglycerin. The relationship between coronary vasospasm and sudden death secondary to polymorphic ventricular tachycardia is discussed. Also, the usefulness of the hyperventilation test to detect this problem and to monitor its therapeutic response is addressed.

A 50-year-old man without previous coronary disease presented with an inferior myocardial infarction following exercise. He was initially treated with thrombolytic therapy and nitroglycerin. Subsequent coronary angio-graphy and cardiac computed tomography demonstrated an anomalous right coronary artery originating from the left coronary sinus and passing between the aorta and main pulmonary artery. The coronary arteries were otherwise patent. The patient later underwent transaortic unroofing of the anomalous right coronary artery and was discharged in good condition.

Recruitable coronary collaterals may appear when spasm suddenly occludes the coronary artery. We report a patient with variant angina who had visible collateral vessels on a control coronary angiogram, despite the presence of normally appearing coronary arteries. These collaterals disappeared after intracoronary administration of nitroglycerin. These findings suggest that recruitable collateral vessels can remain patent long after spontaneous attacks of angina have resolved, and become visible when there is a pressure difference between two small coronary arteries.

A patient with Wolff-Parkinson-White syndrome and variant angina developed progressive ST-segment elevation in the inferior leads after hyperventilation-induced right coronary artery spasm. At the same time, increasing pre-excitation ("concertina effect") developed with gradual prolongation of the AH interval on His bundle ECG. The ECG changes promptly disappeared after sublingual nitroglycerin, with termination of the chest pain. Transient ischemia induced by coronary artery spasm can be an etiology of increasing pre-excitation.

A woman in her 50s with HER2 (human epidermal growth factor receptor 2) positive, estrogen/progesterone receptor negative, metastatic invasive ductal carcinoma of the breast, presented with acral cyanosis and severe throbbing pain after recent administration of gemcitabine. She was treated with aspirin, heparin, amlodipine, topical nitroglycerin and analgesics. Gemcitabine was discontinued permanently. She had a gradual recovery except for a small necrotic area over the right 4th digit. However, surgical intervention was avoided.

Methemoglobinemia, although rare, must be considered in surgical patients presenting with acute respiratory distress and cyanosis. We report two cases of methemoglobinemia in patients undergoing aortic reconstruction. The first patient developed methemoglobinemia while on a nitroglycerin infusion, and the second after receiving benzocaine spray before intubation. Both patients were treated with methylene blue and ascorbic acid, with resolution of their hypoxia and cyanosis. The pathophysiology, etiology, diagnosis, and treatment of methemoglobinemia are reviewed.
