Hip screening: are health visitors adequately prepared?

Developmental dysplasia of the hip is a crippling disorder, causing early arthritis if left undetected. In Northern Ireland health visitors routinely screen infants' hips at specific intervals up to pre-school age. The screening tests used appear simple but involve a high degree of skill in order to identify abnormality. A video study of three health visitors found that their performance was poor, but a marked improvement was noted after a period of specialised training. Bernadette Trainor et al suggest that more initial training and post-qualification update sessions are necessary to support

Hostility Patterns: Implications for Nursing Practice.

In order to present the state of the science of hostility among and across disciplines, a review of the literature was completed. The knowledge gained may influence nursing practice. Scholarly works from nursing, medical and basic sciences, psychology, sociology, education, philosophy, business, communication, and criminology were reviewed. Similar patterns in the use of the concept were discovered. The patterns revealed five themes: hostility as a health-risk factor, hostility as a factor in family relationships, hostility as a factor in perceived challenge, hostility as a factor in criminal

A health visitor affects the problems others do not reach.

Difficulties associated with old age that were previously unknown to the general practitioner were detected by a health visitor attached to a general practice for 2 years and given a case-load of 296 people aged 70 years or more. Previously unknown conditions included physical and mental illnesses and social difficulties related to the patient's environmental setting or to the person (carers) who looked after the patient regularly. Most of the severe illnesses were known to the general practitioner but many minor ailments and other aspects of old age were not. Even though the practice had a po

Exploring Natural Language Processing through an Exemplar Using YouTube.

There has been a growing emphasis on data across various health-related fields, not just in nursing research, due to the increasing volume of unstructured data in electronic health records (EHRs). Natural Language Processing (NLP) provides a solution by transforming this unstructured data into structured formats, thereby facilitating valuable insights. This methodology paper explores the application of NLP in nursing, using an exemplar case study that analyzes YouTube data to investigate social phenomena among adults living alone. The methodology involves five steps: accessing data through You

Association of Social Support With Functional Outcomes in Older Adults Who Live Alone.

IMPORTANCE: Older adults who live alone are at risk for poor health outcomes. Whether social support mitigates the risk of living alone, particularly when facing a sudden change in health, has not been adequately reported. OBJECTIVE: To assess if identifiable support buffers the vulnerability of a health shock while living alone. DESIGN, SETTING, AND PARTICIPANTS: In this longitudinal, prospective, nationally representative cohort study from the Health and Retirement Study (enrollment March 2006 to April 2015), 4772 community-dwelling older adults 65 years or older who lived alone in the com

Frailty Focus: Empowering Rural Health With Advanced Nurse Practitioners: A Discussion Paper.

BACKGROUND: Frailty is a multidimensional condition with symptoms relating to falls, immobility, incontinence, impaired memory and medication side-effects. With increasing numbers of frailty, particularly in rural areas, healthcare systems are being challenged globally. Moreover, frailty may be more common in rural communities as a consequence of transportation issues, limited access to healthcare services and health promotion activities. Advanced Nurse Practitioners are ideally placed to undertake comprehensive geriatric assessments and identify frailly syndromes. AIMS: Explore the function

POMSNAME: an aide-mémoire to improve the assessment and documentation of palliative care - a longitudinal project.

BACKGROUND: Evidence-based palliative care requires comprehensive assessment and documentation. However, palliative care is not always systemically documented - this can have implications for team communication and patient wellbeing. The aim of this project was to determine the effectiveness of an aide-mémoire - POMSNAME - to prompt the comprehensive assessment of the following domains by clinicians: pain, orientation and oral health, mobility, social situation, nausea and vomiting, appetite, medication, and elimination. METHODS: A placard depicting the aide-mémoire was distributed to communi

[Paroxysmal feeling of suffocation during sleep--a diagnostic challenge].

Our patient suffered from nocturnal awakenings due to paroxysmal feelings of suffocation for three years. He was extensively examined at the community health centre and at the departments of neurology, psychiatry, oral diseases and respiratory medicine of the district hospital and the university hospital. The clinical hunch of the night nurses of the department of respiratory medicine and video EEG monitoring recommended by the sleep disorders team eventually revealed the real cause of the patient's symptoms.

Attitudes towards communication and perceived self-efficacy in nursing students: a longitudinal observational study.

BACKGROUND: Communication is recognised as a critical component of all nursing interventions. For nurses to be able to communicate effectively, they need to develop communication skills during their training. Despite this recognition, there is still a lack of consensus about where and when in the syllabus this content should be covered, and how much time should be devoted to this competence, resulting in a inadequate and inconsistent training. Consequently, students develop negative or positive attitudes towards communication. The aim of this study was to analyse the evolution of attitudes tow

Home care by and for relatives of MS patients.

Relatives and family members are the core of the care provided in the home. This also applies to relatives of multiple sclerosis (MS), patients who are dependent on assistance by others. If we want to keep the system of home care going, nurses must keep these care-providing relatives on their feet. We can only do this if we give timely support to these relatives in carrying out their care task and the problems that may arise from it. Once the relatives are overburdened, help is too late. Thus, prevention is an important task for home care nurses. It is crucial that the support the nurses offer
