Resynthesis of sternal dehiscence with autologous bone graft and autologous platelet gel.

Postoperative management of sternal dehiscence requires the organised effort of a multidisciplinary medical team, including orthopaedic surgeons, plastic surgeons, microbiologists, critical care nurses and rehabilitation experts. Clinical care of this complication impacts heavily on health-care costs, length of hospital stay, and the time to full recovery and return to regular work activity. There are various surgical approaches to sternal resynthesis, but they are often unsuccessful. In this paper, we describe the case of a 67-year-old male complaining of chronic pain due to sternal dehiscenc

Analyzing SARS-CoV-2 case numbers and clustering to predict a nursing home outbreak.

The COVID-19 pandemic devastated nursing homes, highlighting the urgent need for effective outbreak control measures. This study analyzed twice-weekly PCR surveillance data from 134 Veteran Affairs Community Living Centers (December 2021-June 2022) to identify early predictors of SARS-CoV-2 outbreaks. Among 16,353 residents (mean age 74, 96% male, 68% white), we identified 1,868 infections and evaluated neighborhood ward-level case counts and their association with subsequent infections over two-week periods. Epidemic unit-days with no initial cases had an 87.49% likelihood of remaining case-f

Effectiveness of a Teaching Program on Ebola Virus Knowledge Among Nursing Students.

Introduction Ebolaviruses are negative-stranded RNA viruses in the Filoviridae family. They are transmitted to humans through direct contact with the bodily fluids of infected individuals or animals, and they cause severe and often fatal hemorrhagic fever, with mortality rates ranging from 25% to 90%, depending on the outbreak and available healthcare resources. These viruses cause severe systemic disease with high mortality rates. Given the infectious nature of Ebola and the critical role of healthcare providers in managing outbreaks, educating nursing students on the virus's transmission, pr

[Open versus arthroscopic shoulder stabilization. An analysis of the treatment costs].

In this study the total costs of clinical open and arthroscopic anterior shoulder stabilization were evaluated, analyzed and compared. From 1988 to 1998 147 patients underwent open (Bankart) or arthroscopic (ASK) anterior shoulder stabilization. We randomized two groups of 30 patients for each method (Bankart: 25 male, 5 female, 29 years of age; ASK: 25 male, 5 female, 26 years of age) and evaluated the costs of their clinical treatment. The total cost was significantly ( p<0.05, Mann-Whitney U-Test) higher for the open (5639 euro) than for the arthroscopic (4601 euro) therapy. There was a sig

Atrial electrogram interpretation improves after an innovative education program.

BACKGROUND: To avoid adverse patient outcomes from inappropriate treatment, it is recommended that an atrial electrogram (AEG) be recorded whenever atrial arrhythmias develop in patients after cardiac surgery. However, AEGs are not commonly performed because nurses lack knowledge about differentiating atrial rhythms on AEGs. OBJECTIVE: To investigate whether completing a novel online evidence-based education program on interpreting AEGs would improve critical care nurses' AEG interpretation. METHODS: Specialized critical care nurses were taught about obtaining and interpreting atrial rhythms

Habit reversal training for the itch-scratch cycle associated with pruritic skin conditions.

Habit reversal therapy was originally developed by Azrin and Nunn in 1977 as a multicomponent treatment for nervous tics and habits. Their premise for clinical intervention was that an old habit can be broken by replacing it with a new, more desirable habit. Dermatology nurses trained in habit reversal therapy can enhance system management and quality of life for patients. Recommendations for the evaluation of habit reversal in patients with eczema and prurigo nodularis are made.

A Multimodal Educational Programme to Enhance Nurses' Proficiency in Delirium Recognition in a Paediatric Intensive Care Unit.

BACKGROUND: Delirium, a common complication in paediatric intensive care units (PICUs), can significantly impede recovery and lead to adverse outcomes. The Cornell Assessment of Pediatric Delirium (CAPD) scale, a subjective tool for delirium identification, is influenced by nurses' clinical expertise and cognitive abilities. Therefore, targeted education and training for health care providers are essential. AIM: This study aimed to enhance PICU nurses' ability to assess paediatric delirium through a Multimodal Educational Programme based on the PRECEDE (Predisposing, Reinforcing and Enabling

The effectiveness of physical restraints in reducing falls among adults in acute care hospitals and nursing homes: a systematic review.

BACKGROUND: Physical restraint was introduced as the primary measure to maintain patient safety in preventing falls. However, physical restraints may lead to complications such as functional loss, immobility, delirium, pressure sore, and even falls or injuries. Despite all these negative effects and many alternatives that are available, physical restraint is still commonly used in hospitals and nursing homes. Hence, it is crucial to understand the effectiveness of physical restraints in protecting adult patients from falling. OBJECTIVE: The overall objective was to examine the effectiveness o
