Primary Tibiotalocalcaneal Nailing vs Open Reduction and Internal Fixation for Fragility Ankle Fractures in Older Adults: A Markov Model.

BACKGROUND: Fragility ankle fractures are common in the elderly population. The cost-effectiveness of primary tibiotalocalcaneal (TTC) nailing compared with open reduction and internal fixation (ORIF) in this population remains unclear. METHODS: We constructed a Markov cohort model with a 4-year time horizon to compare TTC nailing vs ORIF for fragility ankle fractures in patients aged 75 years and older. All complication rates were derived from the McDonald et al's (2025) systematic review and meta-analysis. Costs were in 2024 US dollars with 3% annual discounting for costs and outcomes. Prob

Comparative effectiveness of arthroscopic-assisted versus open reduction and internal fixation in the treatment of tibial plateau fractures: a retrospective study.

BACKGROUND: Tibial plateau fractures require accurate articular reduction and stable fixation to restore knee function and minimize complications. Arthroscopic-assisted reduction and internal fixation (ARIF) may reduce soft-tissue disruption compared with open reduction and internal fixation (ORIF). To compare clinical outcomes and complication profiles of ARIF versus ORIF for tibial plateau fractures. METHODS: This retrospective cohort study included adults (≥ 18 years) with radiographically confirmed tibial plateau fractures treated surgically between January 2020 and January 2024 at a sing

Evaluating the Effects of Perioperative Ketorolac Use on Outcomes After Surgical Treatment of Open Reduction and Internal Fixation of Ankle Fractures.

BACKGROUND: Ketorolac is commonly used as part of multimodal analgesia after orthopaedic surgery; however, concerns persist regarding its potential effects on postoperative wound healing and fracture union. While previous studies have evaluated ketorolac use in arthroplasty and long bone fractures, data specific to ankle open reduction and internal fixation (ORIF) remain limited. This study evaluated the association between perioperative ketorolac use and short-term postoperative complications and longer term healing outcomes after ankle ORIF. METHODS: This retrospective cohort study obtained

Patient Demographics and Risk Factors for Surgical Site Infections After Open Reduction and Internal Fixation for Bimalleolar Ankle Fractures.

BACKGROUND: The purpose of this study was to identify the incidence and risk factors associated with surgical site infection (SSI) after open reduction and internal fixation of bimalleolar ankle fractures. Bimalleolar ankle fractures are one of the most common subtypes of ankle fractures, accounting for 15% to 20% of all ankle fractures. Recent studies have shown that 4.37% of patients undergoing ORIF of ankle fractures develop an SSI postoperatively. The literature detailing the risk factors for the development of SSI after open reduction and internal fixation (ORIF) of bimalleolar ankle frac

Comparative Outcomes of Single- Versus Dual-Incision Approaches for Open Reduction and Internal Fixation of Complex Tibial Plateau Fractures.

Background/Objectives: Open reduction and internal fixation with plates and screws is the treatment of choice for bicondylar tibial plateau fractures. The use of a surgical approach remains a topic of debate regarding the site and number of incisions that ensure best access for reduction with minimum additional soft tissue damage. This retrospective cohort study compared clinical, radiological, and functional outcomes of single- (anterior) versus dual-incision (anterolateral and medial) approaches that are widely used in the operative treatment of Schatzker V-VI tibial plateau fractures. Trends, complications, and comorbidity risk in total elbow arthroplasty versus open reduction and internal fixation for distal humerus fractures.

BACKGROUND: Treatment of distal humerus fractures (DHF) in older patients presents challenges due to high complication rates. Total elbow arthroplasty (TEA) has emerged as an alternative to open reduction and internal fixation (ORIF). This study investigates trends, outcomes, and risk factors associated with TEA and ORIF for the treatment of DHF. METHODS: Using the PearlDiver Mariner database, patients aged 55 years and older treated with either TEA or ORIF for DHF between 2010 and 2021 were identified. Trends in procedure volume, 90-day complications (postoperative bleeding, wound disruption

Retrospective analysis of open reduction and locking plate fixation in three and four part proximal humeral fractures with efficacy and complications.

Three- and four-part proximal humeral fractures are common in young and elderly populations. Open reduction and fixation with a locking plate is a commonly used surgical technique; however, it is associated with a high incidence of complications. This study aimed to retrospectively evaluate the clinical effectiveness of open reduction and fixation with a locking plate for treating three- and four-part proximal humeral fractures and to analyze potential risk factors for complications. The clinical data of 126 patients with three-part (57 cases) or four-part (69 cases) fractures of the proximal

Open versus closed reduction and internal fixation in children with femoral neck fractures. Systematic review and meta-analysis of the literature.

BACKGROUND: Femoral neck fractures in children are uncommon, making up little more than 1% of all paediatric fractures. It's not apparent which type of internal fixation-closed or open-is preferable in these situations when it comes to therapeutic options. When treating children with displaced femoral neck fractures, serious problems can arise. METHODS: Five databases were examined: Medline Plus, PubMed, Scopus, Science Direct, and Web of Science. The outcomes that were relevant for the meta-analysis were non-union, coxa vara, and avascular necrosis. Using the RevMan file, we extracted the da
