Trends of potentially inappropriate prescribing among older outpatients in China between 2015-21.

BACKGROUND: Potentially inappropriate medication (PIM) remains a significant global concern due to its association with adverse drug events in older adults. The prevalence of potentially inappropriate prescriptions (PIPs) varies considerably across different countries. However, there is a lack of evidence using standardised Chinese national criteria to explore variations in prescribing trends. We aimed to evaluate the trends in the prevalence of PIP based on these criteria. METHODS: In this descriptive epidemiological study, we utilised prescription data from older outpatients across 59 hospi…

Exploratory study about the relationship between leukocytosis and mental disorders in psychiatric outpatients.

Elevated levels of inflammatory cytokines and white blood cells (WBCs) have been observed in some patients with psychiatric disorders. However, the prevalence of leukocytosis, its association with psychiatric conditions, and its relevance in psychiatric outpatient care remain underexplored. This study included psychiatric outpatients who visited Akebono Clinic and underwent blood testing between October 2022 and September 2023. From patient records, we obtained age, sex, DSM-5-TR classification, and the highest and lowest WBC counts during the observation period. Leukocytosis was defined as an…

Awareness of the risk factors and interventions for dementia among outpatients with diabetes.

AIMS/INTRODUCTION: As the prevalence of diabetes increases with age, the number of elderly patients with diabetes in Japan, a super-aged society, continues to increase. We conducted a survey to investigate the extent to which outpatients with diabetes recognize dementia as a complication of diabetes. MATERIALS AND METHODS: A questionnaire was distributed among 777 patients with diabetes to investigate their awareness of dementia and its risk factors. Among these, the Mini-Mental State Examination was also administered to patients over 65 years of age who wished to undergo a cognitive function…

A drug-based model to predict hyponatremia in outpatients of a geriatric clinic.

PURPOSE: Chronic hyponatremia in older people is associated with adverse outcomes including gait disturbances, falls, osteoporosis, fractures, cognitive impairment, and cardiovascular disease. Diagnosis in outpatient settings is challenging due to the non-specific nature of its symptoms. While hyponatremia is well-studied in hospitalized patients, little research has focused on outpatient settings. This study aimed to develop a drug-based model to non-invasively predict hyponatremia in older adults attending a geriatric outpatient clinic. METHODS: Cross-sectional data from 2181 outpatients ag…

Understanding the Diagnosis of Primary Headaches in Outpatients Referred to Japanese Headache Specialists.

Background The timely and accurate diagnosis of primary headaches is crucial for enhancing patients' quality of life. However, primary headaches have been reported to be misdiagnosed and underdiagnosed worldwide, especially by nonspecialists. In Japan, despite the high prevalence of primary headaches, research on the extent of headache misdiagnosis and underdiagnosis remains limited. Therefore, we aimed to assess the rates of concordant primary headache diagnoses between previous physicians and headache specialists among outpatients who were referred to the headache center in Japan. Methodolog…

Influenza Vaccine Effectiveness Against Medically Attended Outpatients Illness, United States, 2023-2024 Season.

BACKGROUND: The 2023-2024 US influenza season was characterized by a predominance of A(H1N1)pdm09 virus circulation with cocirculation of A(H3N2) and B/Victoria viruses. We estimated vaccine effectiveness (VE) in the United States against mild-to-moderate medically attended influenza illness in the 2023-2024 season. METHODS: We enrolled outpatients aged ≥8 months with acute respiratory illness in 7 states. Respiratory specimens were tested for influenza type/subtype by reverse-transcriptase polymerase chain reaction. Influenza VE was estimated with a test-negative design comparing odds of tes…

Intrinsic Capacity and Its Association with Adverse Health Outcomes in Older Japanese Outpatients.

OBJECTIVES: This study aimed to provide evidence regarding the clinical significance of assessing intrinsic capacity (IC). DESIGN: Longitudinal study. SETTING: Frailty clinic. PARTICIPANTS: 351 disability-free outpatients aged ≥65 years. MEASUREMENTS: Adverse health outcomes were a composite of adverse health outcomes, including mortality, emergency hospitalization, nursing home placement, and new certification or exacerbation for long-term care. Comparison of the Effectiveness of Baloxavir and Oseltamivir in Outpatients With Influenza B.

This retrospective cohort study analyzed data from a Japanese health insurance database to assess the effectiveness of baloxavir (n = 4822) for preventing severe events compared with oseltamivir (n = 10,523) in patients with influenza B. The primary endpoint was hospitalization incidence (Days 2-14). The secondary endpoints included intravenous antibacterial drug use, pneumonia hospitalization, heart failure hospitalization, inhalational oxygen requirement, and use of other anti-influenza drugs. The hospitalization incidence was significantly lower with baloxavir (0.15% vs. 0.37%; risk ratio:

Predictors of health-related quality of life in outpatients with coronary heart disease.

INTRODUCTION: Health-related quality of life (HRQoL) is an important treatment target in patients with coronary heart disease (CHD) and is associated with poor outcomes. Therefore, it is of clinical importance to identify the key determinants of HRQoL among these patients. There is, however, limited knowledge of how a comprehensive set of psychosocial factors influence HRQoL. We aimed to determine the relative associations of clinical and psychosocial factors with mental and physical components of HRQoL in a sample of CHD outpatients. METHODS: This cross-sectional study included 1,042 patient…

Effect of acute and long-term exercise on leptin levels in depressed outpatients.

BACKGROUND: Depression is a leading cause of disability worldwide and a significant contributor to the global burden of disease. Altered leptin levels are known to be associated with depressive symptoms, however discrepancies in the results of increased or decreased levels exist. Due to various limitations associated with commonly used antidepressant drugs, alternatives such as exercise therapy are gaining more importance. Therefore, the current study investigates whether depressed patients have higher leptin levels compared to healthy controls and if exercise is efficient to reduce these leve

Personality Organization and Depressive Symptoms Among Psychiatric Outpatients: The Mediating Role of Shame.

Previous research has demonstrated relations between lower level of personality organization (PO) and multiple indices of mental health disturbances. The goal of this research was to examine whether experience of shame would mediate the relationship between PO and depressive symptoms in a sample of 321 Croatian psychiatric outpatients (64% female; mean age, 38.67 years). The total PO level exhibited positive associations with depressive symptoms and with all three subscales of shame ( p < 0.01). Characterological, behavioral, and bodily shame showed significant mediating effects in the PO-depr…

Comparison of Extrapyramidal Symptoms Among Outpatients With Schizophrenia on Long-Acting Injectable Antipsychotics.

BACKGROUND: Extrapyramidal symptoms (EPSs) are adverse effects of antipsychotics. Different risks of EPSs have been attributed to the 3 classes of antipsychotics. This study aimed to assess EPS in a clinical sample of schizophrenia patients who are on LAI and compare the severity of EPSs among the following 3 antipsychotic groups: (1) partial agonist, (2) second-generation antipsychotics, and (3) first-generation antipsychotics. METHODS: Ninety-two patients were recruited from the Schizophrenia Program Injection Clinic. Using the Extrapyramidal Symptom Rating Scale (ESRS), severity of EPS was
