Prevalence of painful temporomandibular disorders in endodontic patients with tooth pain.

BACKGROUND: Pain from temporomandibular disorders (TMDs) may mimic endodontic pain, but its prevalence in endodontic patients is unknown. OBJECTIVES: This cross-sectional study investigated the prevalence of painful TMDs in patients presenting for endodontic treatment of a painful tooth. Contribution of TMD pain to the chief complaint and characteristics associated with TMD prevalence were also assessed. METHODS: Patients reporting tooth pain in the 30 days before attending university clinics for nonsurgical root canal treatment or retreatment were enrolled. Before endodontic treatment, they…

[THE MISTAKE OF CUT AND PASTE FROM ACUTE PAIN TO CHRONIC PAIN].

What is a physician to do when the tools in his toolbox fail him? In the field of chronic pain, we are told that imaging studies are often so non-specific as to barely distinguish between symptomatic and asymptomatic individuals. "Advanced pain management techniques and off-label use of popular pain medicines do not withstand the rigors of controlled clinical trials and in many cases have been shown to be harmful. We are informed by the CDC that we are in the midst of a deadly "physician-driven" epidemic of prescribed opioid use disorder. The British Medical Society refers to "our silent addic…

Pressure-Induced Referred Pain as a Biomarker of Pain Sensitivity in Fibromyalgia.

BACKGROUND: Fibromyalgia (FM) syndrome is characterized by widespread pain, fatigue, and generalized increased pain sensitivity. Appropriate and simple pain models are methods employed to assess pain mechanisms that can potentially lead to improved treatments. Pressure pain thresholds (PPTs) or mapping the referred pain area produced by pressure stimulation at suprathreshold intensities are used to assess pain mechanisms. The optimal suprathreshold stimulation intensity to elicit referred pain with minimal discomfort for patients with FM has yet to be determined. OBJECTIVES: The aim of this s…

Facial Pain Update: Advances in Neurostimulation for the Treatment of Facial Pain.

Craniofacial pain, including trigeminal neuralgia, trigeminal neuropathic pain, and persistent idiopathic facial pain, is difficult to treat and can have severe implications for suffering in patients afflicted with these conditions. In recent years, clinicians have moved beyond treating solely with pharmacological therapies, which are generally not very effective, and focused on new interventional pain procedures. These procedures have evolved as technology has advanced, and thus far, early results have demonstrated efficacy in small patient cohorts with a variety of craniofacial pain states. …

Case Report: Neuropathic pain in a patient with congenital insensitivity to pain.

We report a unique case of a woman with Channelopathy-associated Insensitivity to Pain (CIP) Syndrome, who developed features of neuropathic pain after sustaining pelvic fractures and an epidural hematoma that impinged on the right fifth lumbar (L5) nerve root. Her pelvic injuries were sustained during painless labor, which culminated in a Cesarean section. She had been diagnosed with CIP as child, which was later confirmed when she was found to have null mutations of the SCN9A gene that encodes the voltage-gated sodium channel Nav1.7. She now complains of troubling continuous buzzing in both …

Imagery and pain: the prevalence, characteristics, and potency of imagery associated with pain.

BACKGROUND: There is a dearth of information about imagery in pain sufferers. AIM: The aim of this study was to collect data on the characteristics, prevalence, and potency of imagery associated with pain. METHOD: The images of 59 pain sufferers were assessed by means of a semi-structured interview. The emotional, cognitive, behavioural, and pain-inducing properties (potency) of their index images were assessed by an image induction procedure and self-report scales of anxiety, depression and trauma symptoms. RESULTS: The results showed a remarkably high incidence of images in pain sufferers…

The relationship of changes in pain quality to pain interference and sleep quality.

UNLABELLED: Pain is a complex multidimensional experience that includes overall intensity/magnitude, unpleasantness/bothersomeness (affect), location, and quality. However, there is a paucity of research examining the importance of pain quality to patient functioning; most research focuses only on the intensity and affective components of pain. This study sought to address this gap by examining, in a sample of patients with carpal tunnel syndrome (CTS), the associations between pain quality and 2 domains of patient functioning: pain interference and sleep quality. We found that measures of pai…

Pain-related fear, catastrophizing and pain in the recovery from a fracture.

Background and aims Pain-related fear and catastrophizing are prominently related to acute and persistent back pain, but little is known about their role in pain and function after a fracture. Since fractures have a clear etiology and time point they are of special interest for studying the process of recovery. Moreover, fracture injuries are interesting in their own right since patients frequently do not recover fully from them and relatively little is known about the psychological aspects. We speculated that catastrophizing and fear-avoidance beliefs might be associated with more pain and po…

The influence of repetitive painful stimulation on peripheral and trigeminal pain thresholds.

We were interested in how continuous painful stimulation which is performed as inurement exercises in some Asian martial arts influences sensory and pain perception. Therefore, we examined 15 Kung Fu disciples before and after a 14 day period with repetitive inurement exercises and measured sensory and pain thresholds and intensities in both the trigeminal and the peripheral (peroneal nerve) region. The results of the probands were compared to those of 15 healthy control subjects who were performing sports without painful stimulation during this period. The probands showed a significantly decr…

The effects of sleep deprivation on pain inhibition and spontaneous pain in women.

Impaired central pain modulation is implicated in the pathophysiology of chronic pain. In this controlled experiment, we evaluated whether partial sleep loss altered endogenous pain inhibition and reports of spontaneous pain. Thirty-two healthy females were studied polysomnographically for 7 nights. On Nights 1-2 (Baseline), subjects slept undisturbed for 8 hours. After Night 2, subjects were randomized to Control (N = 12), Forced Awakening (FA, N = 10), or Restricted Sleep Opportunity (RSO, N = 10) conditions. Controls continued to sleep undisturbed. FA underwent 8 forced awakenings (one per …

Continuous papaveretum infusion for the control of pain in painful sickling crisis.

We describe our experience of using continuous papaveretum infusions to control pain in 24 children admitted on 45 occasions with painful sickling crisis. The children were aged from 1.7 to 14.3 years. Infusion duration ranged from one to nine days (median three days), total dose from 0.3 to 21 mg/kg (median 2.4 mg/kg), with a pronounced tendency for dosage to increase with increasing age. No respiratory depression was observed. One infusion was discontinued because of cerebral toxicity.

Pain in the foot. 2. Causes of pain in the hindfoot, midfoot, and forefoot.

Pain in the foot has many causes. A careful history and physical examination, radiographs, and laboratory data usually yield the correct diagnosis so that specific treatment can be directed toward the problem to help relieve the patient's pain. Rarely is surgery necessary. Good conservative management usually produces a satisfactory clinical response.

The Relationship between Multisensory Sensitivity and Pain Interference in Adolescents with Menstrual Pain: The Mediating Roles of Pain Catastrophizing and Fatigue.

OBJECTIVE: Among adolescent girls, primary dysmenorrhea (ie,, menstrual pain without an identified underlying pathology) has been linked to heightened sensitivity to non-painful stimuli (eg,, light, touch, sound, etc.) and future widespread pain. However, less is known about how multisensory sensitivity is related to menstrual and non-menstrual pain interference, or which cognitive and psychophysiological factors contribute to these associations. This study examined the relationships between multisensory sensitivity and menstrual and non-menstrual pain interference, and whether pain catastroph…

Causal Mediation Analysis of the Effects of Pain Education on Disability and Pain Intensity in Individuals with Chronic Low Back Pain.

Background/Objective: The purpose of this study was to determine whether the effects of pain education combined with physiotherapy could be explained by changes in psychological well-being and self-efficacy in individuals with chronic low back pain (LBP). Methods: This study includes a secondary analysis (mediation analysis) of a randomized controlled trial (RCT) that compares the effect of physiotherapy and pain education with physiotherapy alone. The Roland-Morris Disability Questionnaire, assessed at six weeks, was used as a primary outcome in this study, with pain intensity as a secondary …

Applicability of the theory of pain-sensorimotor interactions to orofacial pain and sensorimotor behavior, and implications for orofacial pain research and management.

The orofacial sensorimotor system encompasses a variety of orofacial tissues as well as several pathways and circuits in the central nervous system (CNS) that participate in orofacial sensorimotor behaviors such as chewing, facial expression, speech, and swallowing. Acute or chronic pain can severely affect these behaviors, but the relationships between sensorimotor behaviors and pain, and the factors influencing the interactions and underlying mechanisms have remained unclear. Several theories proposed to account for the interactions and mechanisms have not provided a comprehensive picture of…

Referred pain is associated with greater odontogenic spontaneous pain and a heightened pain sensitivity in patients with symptomatic irreversible pulpitis.

BACKGROUND: Symptomatic irreversible pulpitis often results in heightened reactions to thermal stimuli such as pain evoked by a cold stimulus, and spontaneous odontogenic pain (unprovoked pain). OBJECTIVE: This study primarily compared the clinical manifestations of odontogenic spontaneous pain and pain provoked by cold stimulus specifically focusing on their sensory discriminative characteristics (intensity, duration and quality) between symptomatic irreversible pulpitis patients with and without referred pain. METHODS: Twenty-three patients with symptomatic irreversible pulpitis with refer…
