A clinical index for predicting visual acuity after cataract surgery.

We developed a clinical index for predicting postoperative visual acuity of cataract patients and cross-validated it using data from 182 patients aged 70 years and older. The index consisted of four statistically combined indicators: age, preoperative visual acuity, frequency of reading, and comorbidity. Validation of the index included comparisons to two standard technical instruments for measurement of retinal visual acuity. For the clinical index, 72% of predictions were accurate within one Snellen line of postoperative visual acuity compared to 37% using a laser interferometer and 33% usin

Prostaglandin E1 infusion therapy in dry age-related macular degeneration.

OBJECTIVE: The pilot study is intended to show whether prostaglandin E1 (PGE1) infusions are able to stop the gradual vision loss in dry age-related macular degeneration (AMD) and, further, to stabilize or improve visual acuity. METHODS: With PGE1 infusions 11 patients with different forms of dry AMD were treated and compared with a control group of 10 untreated patients with dry AMD. The target parameter was the visual acuity, as determined with the ETDRS logMAR charts. Other examinations performed during the study were tests of contrast vision, colour vision and central visual fields, as we

Bilateral Trichosporon asahii keratitis after ptosis correction: A case report.

RATIONALE: Fungal keratitis (FK) is a severe vision-threatening disease that can lead to corneal perforation or endophthalmitis despite proper treatment. It is important to diagnose the disease promptly due to its indolent nature and disproportionate disease symptoms. Trichosporon asahii is reported rarely as the causative organism of FK. We report a case of highly unusual bilateral T asahii keratitis following ptosis surgery. PATIENT CONCERNS: An 86-year-old female underwent bilateral levator resection surgery for ptosis. Postoperatively, the patient complained of gradually worsening bilater

Bardet-Biedl syndrome and cystinuria.

An unusual association of Bardet-Biedl syndrome with cystinuria was described in one patient. A 21-year-old male was admitted to hospital because of renal failure, severe deterioration of visual acuity, polydactyly, brachydactyly, and mental retardation. Laboratory investigations revealed a serum creatinine of 292 mumol/L (3.3 mg/dL) and a GFR of 25 mL/min per 1.73 m2. Quantitative ion exchange chromatography demonstrated an increased urinary excretion rate of cystine, lysine, arginine, and ornithine. The ophthalmologic examination showed a severe atypical retinal dystrophy. Visual acuity was

Color vision in anterior ischemic optic neuropathy.

PURPOSE: The non-arteritic form of anterior ischemic optic neuropathy (AION) is the most common acute optic neuropathy among individuals over the age of fifty, yet little is known about how the disorder affects color vision. We tested the hypothesis that color vision correlates with visual acuity in patients with non-arteritic AION. We also evaluated the patterns of visual field loss in a subgroup of patients who manifested relative sparing of color vision. OBSERVATIONS: Records of forty-five patients with non-arteritic AION who had been evaluated at Duke University over a consecutive four-ye

Presumed eccrine carcinoma metastatic to the choroid.

The purpose of this article is to describe a patient with presumed choroidal metastasis from an eccrine adenocarcinoma of the scalp. A 45-year-old Caucasian woman presented with decreased visual acuity. Ophthalmologic examination was unremarkable. Her past medical history was significant for eccrine carcinoma of the scalp. The patient developed bilateral cervical lymph node metastases, and received chemotherapy and radiotherapy. She had recurrence of the scalp lesion and developed bone metastasis. The patient was again referred to an ophthalmologist owing to reduced visual acuity. Multiple cho

Algorithm to reduce inter-rater variability in assessment of intermacs patient profiles.

BACKGROUND: Intermacs Patient Profiles (IPP) are subjective assessments of patient acuity of illness assigned at the time of durable left ventricular assist device implantation (dLVAD). Previous studies have demonstrated provider variability with IPP assignment. We developed a novel algorithm to derive a patient's IPP acuity of illness to assist clinicians with IPP assignment. METHODS: Twenty standardized clinical vignettes were developed characterizing a patient's status at time of evaluation for dLVAD implantation. Vignettes included the full range of patient acuity of illness represented b…
