Combined pulmonary fibrosis and emphysema (CPFE) is characterized by emphysematous lesions in the upper lung field and pulmonary fibrosis in the lower lung field and is often associated with pulmonary hypertension and severe exercise-induced hypoxemia (EIH). We herein report a 62-year-old man with CPFE who presented with severe EIH despite relatively preserved lung volumes. Cardiopulmonary exercise testing suggested exercise-induced right-to-left shunt (EIS) through a patent foramen ovale (PFO). EIS was attributed to exercise-induced pulmonary hypertension. In this case report, we highlight th

The complex and variable anatomy of complex double outlet right ventricle makes it imperative to understand the spatial anatomic structures to determine whether it is feasible to repair the anomaly in a biventricular or univentricular fashion. Biventricular repair should be aimed for but is not always feasible. Choosing the correct surgical technique is of great importance in surgical planning of biventricular repair. Conventional imaging is typically insufficient to predict feasibility and technique of biventricular repair. The gap between virtual images and spatial reality can be filled usin

Congenital absence of the right coronary artery (RCA) is an extremely rare coronary anomaly that may occur in isolation or in association with other congenital heart defects. We report the case of a woman in her early 30s with a history of surgically corrected congenital heart disease, who was diagnosed with RCA agenesis during evaluation for mild anginal symptoms. Coronary CT angiography revealed the absence of the RCA origin, which was confirmed by coronary angiography. No obstructive coronary lesions were identified. Given her stable condition and absence of myocardial ischaemia, a conserva

BACKGROUND: Fungal osteomyelitis is rare in clinical practice, and osteomyelitis caused by Scedosporium apiospermum is even rarer, which is easy to misdiagnose, and it is resistant to many antifungal drugs, which makes it tricky to treat. Early diagnosis and accurate treatment are essential.
CASE PRESENTATION: A 38-year-old healthy male has been experiencing recurrent pain in his right foot, accompanied by skin ulcers and exudate, for the past five years. He has been diagnosed with bacterial osteomyelitis at other hospitals as well as at our hospital. In the first stage, osteomyelitis lesion

Adrenal teratomas are rare neoplasms, most often benign but with potential for malignant transformation, particularly in the pediatric population. A seven-year-old girl from kawasoti, Nepal, presented with intermittent right-sided abdominal pain for 2 months, initially attributed to dietary factors such as junk food intake. Ultrasonography revealed a large right suprarenal mass measuring 10.4 × 8.2 cm with internal calcifications. Contrast-enhanced computed tomography demonstrated a heterogeneous lobulated lesion containing fat, fluid, and calcifications in the right suprarenal region, displac

Dissecting aneurysms are common in early stages of life, and minimally invasive intervention is recommended to reduce the risk of complications due to the challenge of identifying structures with a higher risk of bleeding. An 18-year-old patient presented with a dissecting aneurysm of the right middle cerebral artery, characterized by a poorly defined neck and a high risk of rupture. Endovascular treatment with a flow-diverting stent and coils was performed, successfully correcting the lesion without complications. Recent advances in endovascular therapy allow precise localization of blood ves

BACKGROUND: Anatomical abnormalities in the pulmonary vessels have long aroused great interest among thoracic surgeons, and numerous variations of pulmonary vessels have been reported. Heterotaxy syndrome is an anatomical abnormality in which typically asymmetrical organs, including the lungs, develop symmetrically. We report the case of a 71-year-old man with heterotaxy syndrome undergoing radical lobectomy in the treatment of non-small cell lung cancer.
CASE PRESENTATION: Computed tomography (CT) revealed an irregular nodule 25 mm in diameter in the right middle lobe. Two months later, at h

INTRODUCTION: Selecting the appropriate treatment for older patients with non-muscle invasive (NMIBC) or muscle-invasive bladder cancer (MIBC) is challenging due to smoking-related comorbidities, treatment toxicity, and an increased risk of adverse health outcomes. Considering patient preferences prior to treatment is therefore crucial. Here, we aimed to identify the health outcome priorities of older patients with high-risk NMIBC (HR-NMIBC) or MIBC.
MATERIALS AND METHODS: Patients aged 70 years or older or at risk for frailty, diagnosed with HR-NMIBC or MIBC without distant metastases, were

BACKGROUND: Adults with a systemic right ventricle (sRV) are at a high risk for heart failure (HF) hospitalization and mortality. Bioactive adrenomedullin (bio-ADM) has been proposed as a marker of congestion and prognosis in patients with cardiovascular disease. We aimed to evaluate the association between bio-ADM and mortality and HF events in sRV patients.
METHODS: Plasma bio-ADM was measured by a novel immunoassay in plasma of 85 sRV patients. A composite endpoint of all-cause mortality and HF events was used as outcome. HF events were defined as onset or progression of HF signs or sympto

BACKGROUND: It is widely recognized that the right ventricle plays a significant role in the prognosis of numerous diseases. However, the assessment of right ventricular function (RV) has not been given much attention until recently. This study used speckle-tracking echocardiography (STE) to assess RV functions in ischemic cardiomyopathy (ICM) patients.
RESULTS: This study included 74 patients diagnosed with ischemic cardiomyopathy (ICM) and an ejection fraction (EF) of less than 50%. Although all the selected patients had normal RV systolic function by tricuspid annular plane systolic excurs

A 39-year-old woman with intermittent palpitations, psoriasis, and a family history of sudden death presented with dilated right heart chambers and an enlarged coronary sinus. Despite a normal bubble study, further evaluation with transesophageal echocardiography revealed an abnormal pulmonary venous return: the left pulmonary veins drained into the coronary sinus. Cardiac computed tomography confirmed this finding, suggesting a partial abnormal pulmonary venous return as the underlying issue. Cardiac catheterization indicated increased pulmonary artery flow with normal pulmonary vascular resi

Double-chambered right ventricle is a rare form of right ventricular outflow tract obstruction caused by anomalous hypertrophy of muscle bundles in right ventricle. Cases most often occur in children and rarely in adults. Most cases (80-90%) are associated with ventricular septal defect. We describe a case of pulmonary atresia and ventricular septal defect with double-chambered right ventricle. The interesting clinical findings, ECG, echocardiography and angiocardiography features are described here.

BACKGROUND Pericardiocentesis is a commonly used procedure to remove or sample pericardial effusion, and complications of this procedure are rare. This report describes a 75-year-old man with lymphoma and right ventricular perforation during pericardiocentesis for pericardial effusion. CASE REPORT A 75-year-old male patient with diffuse large B cell lymphoma was admitted with concerns of refractory chest tightness and breath shortness after physical exercise. Images from echocardiography showed massive pericardial effusion. After a comprehensive clinical assessment, pericardiocentesis was sele

AIM: Behcet's disease (BD) is a systemic disorder characterized by vasculitis, resulting in thickened vascular walls that reduce elasticity and impair function. BD can involve the cardiovascular system in three ways: cardiac, arterial, and venous. In this study, our objective was to evaluate the efficacy of pulmonary arterial stiffness (PAS) and pulmonary pulse transit time (PPTT) measures in demonstrating right ventricular functions in asymptomatic BD patients. We aimed to objectively evaluate right ventricular function in patients with BD using four-dimensional echocardiography (4DE).

Alien hand syndrome (AHS) is a rare but significant disorder, characterized by involuntary and often complex movements of one's hand. These movements seem to occur independently and unconsciously, separate from a person's intended actions. We report a case of a 70-year-old male who presented to the emergency department with left sided hemiplegia following a stroke in the right middle cerebral artery (MCA) that affected the right temporal, parietal and internal capsule region. Upon additional inquiry, the patient reported experiencing involuntary, abrupt movements in his left upper and lower l

We compared neuropsychiatric evaluations in temporal lobe epilepsy according to the lateralized hemisphere. Forty-one (68.3%) left-sided temporal lobe epilepsy (LTLE) were compared to 19 right-sided temporal lobe epilepsy (RTLE) (31.7%) (p < 0.001). RTLE mean age was 37 (22-46) years, and LTLE mean age 38 was (30-42). RTLE disease duration was 10 (6-20) years, and LTLE was 22 (10-33) (p < 0.013). Gender (man/woman) for RTLE was 7/12, and for LTLE was 18/23. LTLE scored poorer on the Wechsler Memory Scale (WMS)-III's Mental Control Months-error, WMS-V's "Forward Number Range" and "Backward Numb

BACKGROUND: Little is known about how tightly right atrial pressure (RAP) is associated with prognosis in patients with severe tricuspid regurgitation (TR). The aim of this study was to investigate the association of RAP estimated by echocardiography (RAP-echo) with cardiovascular events in patients with severe TR.
METHODS: Two hundred forty outpatients (median age, 75 years; 130 women) who underwent two-dimensional transthoracic echocardiography and were diagnosed with severe TR were retrospectively studied. According to RAP-echo using the diameter of the inferior vena cava and its response

Transcatheter tricuspid valve placement in congenital heart disease is becoming increasingly used when a previously placed surgical ringed valve becomes dysfunctional. Surgically repaired and/or native tricuspid inflows are generally not amenable to transcatheter valve placement without a prior ring being placed. We present the second pediatric case to our knowledge of transcatheter tricuspid valve placement in a surgically repaired tricuspid valve in the absence of a ring.

BACKGROUND AND AIM: Prolonging the QT interval in the right bundle branch block (RBBB) can create challenges for electrophysiologists in estimating repolarization time and eliminating the effect of depolarization changes on QT interval. In this study, we aimed to develop a practice formula to eliminate the effect of depolarization changes on QT interval in patients with RBBB.
METHODS: This prospective study evaluated accidentally induced RBBB in patients undergoing electrophysiological study. Two expert electrophysiologists recorded the ECG parameters, including QRS duration, QT interval, and
