Dose-dependent effect of esketamine on preventing rocuronium-induced limb withdrawal in preschool children.

BACKGROUND: Rocuronium injection often causes limb withdrawal reactions in preschool children during anesthesia induction, which can lead to adverse effects. Esketamine has been reported to reduce such reactions, but the optimal dose remains unclear. OBJECTIVES: To observe the effect of different doses of esketamine administered in advance on limb withdrawal reactions caused by rocuronium in preschool children. MATERIAL AND METHODS: From January to December 2024, children aged 2-6 years (American Society of Anesthesiologists Physical Status Classification System (ASA) I-II) undergoing electi

Effect of caudal block injection speed on optic nerve sheath diameter in pediatric surgeries: A prospective observational cohort study.

Perioperative pain management in children is challenging due to limited communication and the risk of long-term psychological effects. Regional anesthesia, particularly the caudal block (CB), is frequently used in pediatric surgery for lower abdominal and urogenital procedures. However, the influence of injection speed during CB on intracranial and hemodynamic responses remains unclear. This prospective observational cohort study included 64 pediatric patients (American Society of Anesthesiologists I-II, aged 1-7 years) undergoing elective lower abdominal or urogenital surgery under general an

MiR-483-5p has the potential to serve as a biomarker in perioperative respiratory adverse events in children under general anesthesia.

BACKGROUND: Perioperative respiratory adverse events (PRAE) may cause critical illness in children undergoing anesthesia and surgery. So, this study aims to explore the diagnostic value of miR-483-5p in PRAE. METHODS: This study included 112 patients with PRAE and 93 patients without PRAE. MiR-483-5p expression was quantified utilizing RT-qPCR. The receiver operating characteristic (ROC) curve was utilized to assess the diagnostic value. The clinicopathological characteristics of PRAE and miR-483-5p level were analyzed by the Chi-squared test. Risk factors for PRAE were analyzed using multivar

Postanesthesia Apnea in Former Preterm Infants for Inguinal Herniorrhaphy: An Update of Risk Factors from an Individual Participant Data Meta-analysis.

Postmenstrual age (PMA) is the major risk factor for postanesthesia apnea. The authors hypothesized that modern anesthesia may reduce the at-risk PMA cutoff threshold. The aim of this individual participant data meta-analysis was to identify the PMA above which postanesthesia apnea risk is less than 1%. Individual participant data from 12 prospective studies undergoing open inguinal herniorrhaphy were analyzed. A total of 132 of 751 (17.6%) former preterm infants experienced postanesthesia apnea. The data indicate that the PMA cutoff at which the risk of postanesthesia apnea was approximately

Effects of regional analgesia on stress responses to pediatric surgery.

Invasive surgery induces a combination of local response to tissue injury and generalized activation of systemic metabolic and hormonal pathways via afferent nerve pathways and the central nervous system. The local inflammatory responses and the parallel neurohumoral responses are not isolated but linked through complex signaling networks, some of which remain poorly understood. The magnitude of the response is broadly related to the site of injury (greater in regions with visceral pain afferents such as abdomen and thorax) and the extent of the trauma. The changes include alterations in metab

Thoracic epidural catheter in the management of a child with an anterior mediastinal mass: a case report and literature review.

We describe a case of an anterior mediastinal mass compressing the right main bronchus that required a biopsy through a thoracotomy incision. The anesthetic management of these patients is associated with several risks and challenges, including potential airway compression and cardiovascular collapse. Inhalation induction and maintenance of spontaneous respiration is recommended to preserve normal transpulmonary pressure gradient and improve flow through conducting airways. We placed a thoracic epidural catheter under general anesthesia as the main analgesic technique in order to maintain spon

Effectiveness of buffered and non-buffered lignocaine and articaine in children aged 6-12 years undergoing invasive dental procedures: A randomized clinical trial.

BACKGROUND: Effective pain control is fundamental to successful pediatric dental care. However, intraoral injections cause significant anxiety, due to burning sensations from the acidic pH of anesthetic solutions. Buffering with sodium bicarbonate raises pH closer to physiological levels, reducing pain and accelerating anesthesia onset. AIM: To evaluate the effectiveness of buffered and non-buffered lignocaine and articaine in children aged 6 to 12 years undergoing invasive dental procedures in terms of pain perception on injection, onset of action, and duration of action. MATERIALS AND METH

Impact of perioperative clonidine on postoperative respiratory adverse events in children: a post-hoc observational analysis of prospectively collected datasets.

BACKGROUND: Clonidine, an alpha-2-agonist, has several applications in pediatric anesthesia. Sympathetic nervous system-mediated pathways contribute to airway reactivity, suggesting that clonidine could mitigate respiratory adverse events similarly to more selective agents. METHODS: This was a post-hoc analysis of prospectively collected observational datasets from three research projects on children (1‒16 years) undergoing elective extracapsular (adeno-) tonsillectomies between January 2018 and November 2021. Patients' study data sheets and medical records were reviewed for clonidine use. Th…

What's New in Wide-Awake Local Anesthesia No Tourniquet (WALANT) Hand Surgery?

Wide-awake local anesthesia no tourniquet (WALANT) has evolved from a novel technique to an increasingly adopted anesthetic strategy in hand surgery. In the post-coronavirus disease (COVID)-19 era, there has been a marked expansion in WALANT-related literature, necessitating synthesis of contemporary evidence to guide clinical practice. This narrative review examines recent WALANT studies organized into six thematic domains: comparative trials versus traditional anesthesia, use in the pediatric population, patient satisfaction and experience, environmental impact, cost comparison and value-bas

Up-and-Down Sequential Allocation Study Comparing the Median Effective Dose of Remimazolam Versus Propofol for Inducing Unconsciousness in Pediatric Patients.

BACKGROUND: Remimazolam is a novel ultra-short-acting benzodiazepine used for general anesthesia. Compared with propofol, it offers reversibility with flumazenil and lacks injection pain. However, its optimal bolus dose for anesthesia induction in pediatric patients remains unclear. The aim of this study was to determine the effective dose of remimazolam for loss of consciousness during anesthesia induction in children aged 2 to 14 years. METHODS: A prospective, randomized, controlled trial using the Dixon up-and-down sequential design was conducted. It comprised two separate, parallel dose-f

Does general anesthesia or intravitreal injection affect neurodevelopment in children undergoing ophthalmic procedures?

PURPOSE OF REVIEW: To review recent studies on potential neurodevelopmental impacts on the pediatric population through general anesthesia events or intravitreal anti-vascular endothelial growth factor (VEGF) injection. RECENT FINDINGS: Studies on this topic have been extensive with varied reported neurodevelopmental outcomes. Initial investigations in rodents and primates showed negative impact of anesthetics on neurodevelopment. Subsequent retrospective human reviews had mixed results whereas more recent sibling and prospective studies have been published without evidence of clinically sign

A case of difficult airway due to lingual tonsillar hypertrophy in a patient with Down's syndrome.

In this report, we describe airway management of symptomatic lingual tonsillar hypertrophy in a pediatric patient with Down's syndrome. Besides obstructive sleep apnea, the history included a small atrial septal defect with mild aortic regurgitation and Moyamoya disease. Anesthesia was induced with IV administration of 1 mg/kg of propofol, followed by inhalation of sevoflurane in 100% oxygen. Muscle relaxants were not used on induction. Rigid laryngoscopy could not visualize the epiglottis because of hypertrophied tonsillar tissue, and mask ventilation became difficult when spontaneous breathi

Effect of end-tidal carbon dioxide level on the optic nerve sheath diameter measured by transorbital ultrasonography in anesthetized pediatric patients: A randomized trial.

BACKGROUND: Intraoperative hypercapnia and hypocapnia are common during pediatric anesthesia, and the cerebral blood flow and intracranial pressure may be affected by the partial pressure of arterial carbon dioxide. Transorbital ultrasound measurement of the optic nerve sheath diameter is a simple and non-invasive method for intracranial pressure assessment. The objective of this study was to evaluate the effect of end-tidal carbon dioxide (ET CO2 ) on optic nerve sheath diameter in a healthy anesthetized pediatric population. METHODS: Pediatric patients scheduled for elective surgery under g…

One week stenting after pediatric laparoscopic pyeloplasty; is it enough?

INTRODUCTION: The use of ureteric stents for urinary diversion after pediatric dismembered pyeloplasty and its duration remain debatable. Classically, an indwelling Double J ureteric stent has to be left for 4-6 weeks. However, such a duration is not free of stent-related complications, in addition to the need to remove it under general anesthesia in the pediatric age group. OBJECTIVES: This study aims to evaluate the outcome of short-term stenting after laparoscopic pyeloplasty in pediatric sector. METHODS: A prospective randomized study of 37 children (less than 16 years-old) with pelvi-ur

Ultrasound Rounds: Intraoperative Point-of-Care Ultrasound Identifies Intracardiac Thrombus and Pulmonary Embolism Allowing for Alteration in Surgical Management.

Most of the cases demonstrating describing acute findings using point-of-care ultrasound (POCUS) have been described in emergency medicine and critical care medicine. While the use of POCUS has become more prevalent in anesthesia practice, documentation of acute findings resulting in alteration in management based on real-time ultrasound findings during pediatric anesthesia remains limited. This case highlights the use of POCUS during cardiopulmonary collapse occurring during correction of neuromuscular scoliosis. POCUS excluded the presumed diagnosis of venous air embolism and identified an i…
