Effects of age and hearing impairment on the ability to benefit from temporal and spectral modulation.

OBJECTIVES: At poor signal-to-noise ratios, speech understanding may depend on the ability to combine speech fragments that are distributed across time and frequency. The goal of this study was to determine the effects of development and hearing impairment on this ability. DESIGN: Listeners in the present study included adults and children with normal hearing and with hearing impairment. The children with normal hearing included a younger group (aged 4.6-6.9 yr, N = 10) and an older group (aged 7.3-11.1 yr, N = 11). The adults with normal hearing were aged 19 to 27 yr (N = 10). Adults (aged 1

The effect of gabapentin on gap detection and forward masking in young and old gerbils.

OBJECTIVES: Auditory temporal processing frequently appears more affected in old subjects than would be predicted by the degree of peripheral hearing loss, pointing to an age-dependent central processing deficit. In parallel, an age-dependent decline of inhibitory function has been demonstrated in the auditory pathway, suggesting a causal relationship between temporal processing and inhibition. Gabapentin has been specifically synthesized as a potential gamma-amino-butyric-acid (GABA) mimetic with the capability to cross the blood-brain barrier. Gabapentin treatment ameliorated tinnitus in a r

Level-dependent changes in detection of temporal gaps in noise markers by adults with normal and impaired hearing.

Compression in the basilar-membrane input-output response flattens the temporal envelope of a fluctuating signal when more gain is applied to lower level than higher level temporal components. As a result, level-dependent changes in gap detection for signals with different depths of envelope fluctuation and for subjects with normal and impaired hearing may reveal effects of compression. To test these assumptions, gap detection with and without a broadband noise was measured with 1, 000-Hz-wide (flatter) and 50-Hz-wide (fluctuating) noise markers as a function of marker level. As marker level i

Automated hearing tests: applying the otogram to patients who are difficult to test.

OBJECTIVE: The Otogram is an automated audiometer capable of determining air and bone conduction thresholds with masking when appropriate. The manufacturer claims that testing can be done in a quiet physician's consultation room without a sound-treated booth. We aimed to test the validity of the Otogram on "difficult-to-test" patients, all of whom require masking. METHODS: Twenty-eight difficult-to-test patients underwent three audiograms: two by an audiologist and one by the Otogram. First, audiograms performed by the audiologists were compared, establishing test-retest reliability. Second,

Distortion product otoacoustic emission (DPOAE) in tinnitus patients.

The aim of this study was to determine the effect of tinnitus (experiment I) and the combined effect of tinnitus and sensorineural hearing loss (experiment II) on the distortion product otoacoustic emission (DPOAE) for two age groups of tinnitus patients. Tinnitus patients with normal earing, along with normal-hearing control subjects, participated in experiment I. They were divided into two age groups, below 50 and above 50 years. Experiment I showed that the DPOAE levels in the tinnitus patients were lower than those in the normal-hearing (nontinnitus) subjects. The differences depended on t

Spectral ripple discrimination and use of speech context factors in relation to speech perception in noise in single-sided deaf cochlear implant users.

OBJECTIVE: To investigate the relation between spectral ripple discrimination and speech perception in noise in single-sided deaf (SSD) cochlear implant (CI) users, and to investigate the use of speech context in speech perception in noise, for both the normal-hearing (NH) and the CI ear. DESIGN AND STUDY SAMPLE: Twenty adult SSD CI users (40 ears) participated in the study. We used the spectral-temporally modulated ripple test (SMRT) and measured speech reception thresholds (SRTs) with the consonant-vowel-consonant (CVC)-in-noise test, the hearing-in-noise (sentence) test (HINT), and the dig

Middle-aged CBA/CaJ mice exhibit auditory dysfunction in background noise.

Aging is associated with deficits in auditory functioning. Characterization of auditory deficits that originate in middle-age is crucial for understanding the initial age-related functional impairments and the spatio-temporal progression of age-related auditory pathophysiology. Early age-related deficits in auditory processing are evident in difficult listening conditions, such as background noise, before becoming evident in quiet. To investigate the effect of noise on age-related auditory dysfunction, we collected suprathreshold auditory brainstem responses (ABRs) from young, middle-aged, and

Passage comprehension performance in children with cochlear implants and/or hearing aids: the effects of voice quality and multi-talker babble noise in relation to executive function.

Purpose: Speech signal degradation such as a voice disorder presented in quiet or in combination with multi-talker babble noise could affect listening comprehension in children with hearing impairment. This study aims to investigate the effects of voice quality and multi-talker babble noise on passage comprehension in children with using cochlear implants (CIs) and/or hearing aids (HAs). It also aims to examine what role executive functioning has for passage comprehension in listening conditions with degraded signals (voice quality and multi-talker babble noise) in children using CI/HA. Afferent-efferent connectivity between auditory brainstem and cortex accounts for poorer speech-in-noise comprehension in older adults.

Speech-in-noise (SIN) comprehension deficits in older adults have been linked to changes in both subcortical and cortical auditory evoked responses. However, older adults' difficulty understanding SIN may also be related to an imbalance in signal transmission (i.e., functional connectivity) between brainstem and auditory cortices. By modeling high-density scalp recordings of speech-evoked responses with sources in brainstem (BS) and bilateral primary auditory cortices (PAC), we show that beyond attenuating neural activity, hearing loss in older adults compromises the transmission of speech inf

Evaluation of dichotic listening performance in normal-hearing, noise-exposed young females.

Recent animal studies have shown that intense noise exposures that produce robust temporary threshold shift (TTS) can inflict irreversible damage to the synaptic connections between the inner hair cells and auditory neurons. It was hypothesized that noise-induced cochlear synaptopathy may cause impaired acoustic encoding in the central auditory nervous system leading to impaired speech perception, particularly in challenging listening situations. The aim of the study was to evaluate the influence of high noise exposure background (NEB) on dichotic listening performance, speech-in-noise perform

Brainstem-cortical functional connectivity for speech is differentially challenged by noise and reverberation.

Everyday speech perception is challenged by external acoustic interferences that hinder verbal communication. Here, we directly compared how different levels of the auditory system (brainstem vs. cortex) code speech and how their neural representations are affected by two acoustic stressors: noise and reverberation. We recorded multichannel (64 ch) brainstem frequency-following responses (FFRs) and cortical event-related potentials (ERPs) simultaneously in normal hearing individuals to speech sounds presented in mild and moderate levels of noise and reverb. We matched signal-to-noise and direc

Segregation of face sensitive areas within the fusiform gyrus using global signal regression? A study on amygdala resting-state functional connectivity.

The application of global signal regression (GSR) to resting-state functional magnetic resonance imaging data and its usefulness is a widely discussed topic. In this article, we report an observation of segregated distribution of amygdala resting-state functional connectivity (rs-FC) within the fusiform gyrus (FFG) as an effect of GSR in a multi-center-sample of 276 healthy subjects. Specifically, we observed that amygdala rs-FC was distributed within the FFG as distinct anterior versus posterior clusters delineated by positive versus negative rs-FC polarity when GSR was performed. Social anxiety predicts unconsciously provoked emotional responses to facial expression.

Previous psychometric studies using a visual search task suggested that interpersonal fear in individuals with social anxiety disorder (SAD) may be processed by unconscious preattentive mechanisms. However, little is known about relationships between social anxiety and preattentive emotional responses. We explored whether social anxiety is associated with preattentive emotional responses to facial expression. Groups with high and low social anxiety were selected from 125 healthy volunteers according to scores on the Social Phobia Inventory. Fearful and happy faces were presented subliminally u

An alternative diagnostic test for active Ménière's disease and cochlear hydrops using high-pass noise masked responses: the complex amplitude ratio.

We [Don et al.: Otol Neurotol 2005;26:711-722] previously demonstrated that patients diagnosed with an active case of Ménière's disease could be distinguished from non-Ménière's normal-hearing subjects by a special auditory brainstem response method involving clicks and ipsilateral high-pass masking pink noise. Specifically, auditory brainstem responses to clicks presented alone and clicks with masking noise high-pass filtered at 8, 4, 2, 1 and 0.5 kHz were recorded. It was shown that the level of masking noise sufficient to progressively mask the response to clicks in non-Ménière's normal-hea

The effect of presentation level and compression characteristics on sentence recognition in modulated noise.

The effect of fast-acting compression on speech recognition in fully modulated (FUM) noise in listeners with normal and impaired hearing was investigated in two experiments We wanted to determine the relationships between the benefit from compression and some audiological factors. Furthermore, the sensitivity to changes in compression parameters was also evaluated. The results showed that two-thirds of the listeners performed worse with fast-acting compression than with linear processing. Normal-hearing listeners showed the most benefit from compression. A significant relationship was found be

[Experiences in Kiel with the cochlear implant].

Since 1988 22 deaf patients were provided with a 22-channel cochlear implant at the Kiel University Hospital. No surgical or postoperative complications were seen in any of the patients. Side effects like facial stimulation and pain sensation during electrode activation do not reduce the utility. Tinnitus may initially be severe but usually subsides to preop levels after a few days. A tinnitus masking effect is pronounced during stimulation but hardly lasts long after switch off. Besides an individually arranged hearing training the accurate speech processor fitting to the needs of the patient

Low-frequency specificity of the auditory brainstem response threshold elicited by clicks masked with 1590-Hz high-pass noise in subjects with sloping cochlear hearing losses.

In this study, the frequency specificity of the ABR threshold to stimulation with a click masked with 1590-Hz high-pass noise was determined in subjects with sloping cochlear hearing losses both high- and low-frequency in character. The results show that the ABR threshold elicited by this stimulus is low-frequency specific. The standard error in estimating the 1,000-Hz pure-tone threshold from the high-pass-noise-masked click-evoked ABR threshold is 10.2 dB which equals that for estimating the 3,000-Hz pure-tone threshold from the routinely used unmasked click ABR threshold. The ABR threshold

Inherited low-frequency hearing loss. A new mixed conductive/sensorineural entity?

Eighteen patients with low-frequency hearing loss ( LFHL ), 10 males and 8 females with an average age of 27 years, were examined in order to classify their hearing loss, i.e. to find the topical site of the hearing lesion and to evaluate the aetiology of the hearing disorder. Based on extensive audiological test procedures, the patients could be subdivided into two groups: Group A comprising 11 patients with true sensorineural hearing impairment caused by various well-known aetiologies, and group B comprising 7 patients in whom no classification of the LFHL could be made because of conflictin
