Inhibitory HLA-C/KIR2DL2 Interactions and KIR Allelic Polymorphisms Are Associated With Susceptibility to Philadelphia Chromosome-Negative Myeloproliferative Neoplasms.

BACKGROUND: Immune surveillance is increasingly recognized as a determinant of clonal selection in Philadelphia chromosome-negative myeloproliferative neoplasms (MPN). While HLA-restricted adaptive immune effects have been described, the contribution of inherited variation in natural killer (NK)-cell regulatory pathways remains less well defined. We investigated whether killer immunoglobulin-like receptor (KIR) allelic polymorphisms and HLA-KIR receptor-ligand interactions are associated with susceptibility to MPN. METHODS: We performed high-resolution next-generation sequencing-based KIR gen…

Correction: Prognostic Differences Between P190 and P210 BCR::ABL1 in Patients with Philadelphia-Positive B-cell Acute Lymphoblastic Leukemia.

Tumor induced osteomalacia is a rare paraneoplastic syndrome caused by usually small mesenchymal tumors. The tumors are commonly located in the extremities whereas occurrence in the nasal cavity is quite rare. An 18 year old male presented with asymmetric weakness of both lower limbs and difficulty in walking since one year. On evaluation he was found to have open femur head physes and iliac apophyses, low serum Phosphorus, low vitamin D, normal serum calcium, elevated PTH & alkaline phosphatase levels. FGF-23 levels in serum were elevated. DOTA NOC PET CT revealed tumor in the left nasal cavi

Case Report: An interesting case of vision and hearing loss revealing CNS recurrence in Philadelphia-positive acute lymphoblastic leukemia.

BACKGROUND: Central nervous system (CNS) relapse in acute lymphoblastic leukemia (ALL) may occur despite systemic remission and can present with isolated neuro-ophthalmologic symptoms. Early diagnosis may be challenging as the initial neuroimaging and ophthalmologic evaluation can be non-diagnostic. CASE PRESENTATION/DISCUSSION: We report on a 57-year-old man with Philadelphia chromosome-positive (Ph+) B-cell ALL in remission on dasatinib maintenance therapy who presented with acute vision and hearing loss. The initial evaluation, including magnetic resonance imaging (MRI) and ophthalmologic …

[Prophylactic use of tyrosine kinase inhibitors after allogeneic hematopoietic stem cell transplantation for Philadelphia chromosome-positive acute lymphoblastic leukemia].

The introduction of tyrosine kinase inhibitors (TKIs) has greatly improved outcomes for patients with Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ALL) undergoing allogeneic hematopoietic stem cell transplantation (HSCT). However, patients with measurable residual disease (MRD) positivity at the time of HSCT show a high relapse rate, making post-HSCT TKI administration a promising strategy. However, the indications, administration methods, and optimal duration of TKI therapy remain unclear. Retrospective studies from outside Japan have suggested that prophylactic TKIs may …

Adolescent and Young Adult versus Older Adult Philadelphia-Negative Myeloproliferative Neoplasms: A Single-Center Experience from the MENA Region.

BACKGROUND: Philadelphia-negative myeloproliferative neoplasms (MPNs), including essential thrombocythaemia (ET), polycythaemia vera (PV), and myelofibrosis (MF), usually affect older adults, but a subset occurs in adolescents and young adults (AYA). Regional age-stratified data remain limited. METHODS: We retrospectively studied Philadelphia-negative MPN patients managed at the National Center for Cancer Care and Research from 2014 to 2024. Patients were classified as AYA (15-39 years) or older adults (≥40 years). Clinical, laboratory, molecular, thrombotic, bleeding, transformation, and sur…

Heat stress targets and degrades BCR::ABL1 oncoproteins to overcome drug-resistance in Philadelphia chromosome-positive acute lymphoblastic leukemia.

BCR::ABL1 oncofusion protein drives Philadelphia-chromosome positive acute lymphoblastic leukemia (Ph+ ALL), making it a critical therapeutic target. Tyrosine kinase inhibitors (TKIs) targeting BCR::ABL1 have revolutionized the treatment of Ph+ ALL patients. However, mutations in the kinase domain of BCR::ABL1 commonly impair the sensitivity to TKIs, resulting in drug resistance and poor prognosis in Ph+ ALL. Here we report that heat stress selectively destabilizes BCR::ABL1 and its common drug-resistant mutants without affecting the native BCR and ABL proteins through inducing liquid-to-solid…

Dasatinib and CAR T-Cell Therapy in Newly Diagnosed Philadelphia Chromosome-Positive Acute Lymphoblastic Leukemia: A Nonrandomized Clinical Trial.

IMPORTANCE: A combination of tyrosine kinase inhibitors and chimeric antigen receptor (CAR) T cells has made a breakthrough in refractory or relapsed Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL). However, it remains unclear if this treatment in newly diagnosed Ph-positive ALL is associated with high rates of complete molecular remission (CMR) and leukemia-free survival. OBJECTIVE: To evaluate the efficacy and safety of dasatinib in combination with CAR T cells as frontline therapy in adults with newly diagnosed Ph-positive ALL. DESIGN, SETTING, AND PARTICIPANTS: T…

Disseminated rhino-orbital-cerebral mucormycosis in Philadelphia chromosome-positive mixed phenotype acute leukemia: a case report and literature review.

Rhino-orbital-cerebral mucormycosis (ROCM) is a rapidly progressing and life-threatening fungal infection caused by fungi in the order Mucorales. It predominantly affects immunocompromised individuals, such as those undergoing chemotherapy for hematological malignancies. Despite its high mortality rate, ROCM remains underrecognized, and its clinical features in patients with Philadelphia chromosome-positive (Ph+) mixed phenotype acute leukemia (MPAL) are rarely reported. This report describes a 48-year-old female who presented with a one-week history of fever without localized pain and was dia…

Allogeneic stem cell transplantation is still a highly curative therapy in adults with philadelphia chromosome-positive acute lymphoblastic leukaemia.

The application of tyrosine kinase inhibitors (TKIs) and novel immunotherapies has improved outcomes in patients with Ph + acute lymphoblastic leukaemia (ALL), and the issue of whether there is still a need for stem cell transplantation has become controversial. We performed a retrospective study to explore whether stem cell transplantation still held a place in patients with Ph + ALL if only imatinib and 2nd generation TKIs are available and affordable. A total of 292 patients were included. The median age was 38 years [range 14-64, IQR 28-48]. Patients receiving transplants (n = 216) had bet

Frontline combination of ponatinib and hyper-CVAD in Philadelphia chromosome-positive acute lymphoblastic leukemia: 80-months follow-up results.

The combination of ponatinib, a third-generation BCR::ABL1 tyrosine kinase inhibitor, with hyper-CVAD chemotherapy resulted in high rates of complete molecular remissions and survival, without the need for stem cell transplantation (SCT) in most patients with Philadelphia chromosome(Ph)-positive acute lymphocytic leukemia (ALL). Confirming these results in a large cohort of patients with longer follow-up would establish this regimen as a new standard of care. Adults with newly diagnosed Ph-positive ALL were treated with the hyper-CVAD regimen. Ponatinib was added as 45 mg daily × 14 during ind…

Sudden extramedullary and extranodal Philadelphia-positive anaplastic large-cell lymphoma transformation during imatinib treatment for CML: A case report.

BACKGROUND: Chronic myeloid leukemia (CML) is a malignant hematologic malignancy that can progress to blast phase with a myeloid or lymphoid phenotype. Some patients with CML can also progress to blast crisis phase; however, the transformation of CML into Philadelphia-positive lymphoma is extremely rare. CASE SUMMARY: We present a patient with CML who experienced a sudden transformation to anaplastic large-cell lymphoma (ALCL) after 7 mo of treatment with imatinib, during which she had achieved partial cytogenetic response as well as early molecular response. The patient noticed a mass in her…

Genetic correlates in patients with Philadelphia chromosome-positive acute lymphoblastic leukemia treated with Hyper-CVAD plus dasatinib or ponatinib.

Recurring genetic abnormalities have been identified in Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL). Among them, IKZF1 deletion was associated with poor prognosis in patients treated with imatinib-based or dasatinib-based regimens. However, the molecular determinants for clinical outcomes in ponatinib-treated patients remain unknown. We systematically analyzed genetic alterations in adults with Ph-positive ALL uniformly treated in clinical trials with dasatinib-based regimens or a ponatinib-based regimen and investigated the molecular determinants for treatment out…

Philadelphia Chromosome-positive Acute Lymphoblastic Leukemia Incidentally Detected by Fluorodeoxyglucose-positron Emission Tomography/Computed Tomography at a Health Checkup.

We herein report a case of Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph-ALL) that was incidentally detected by fluorodeoxyglucose-positron emission tomography (18F-FDG PET)/computed tomography (CT) at a health checkup. At that time, the findings of a physical examination and blood tests were all normal, except for the diffuse bone marrow uptake (maximum standardized uptake value: 6.3). One month later, when the blood counts remained in the normal ranges, a bone marrow examination confirmed the diagnosis of Ph-ALL. Although a diffuse bone marrow uptake of 18F-FDG is observe…

Clinical implications of clonal chromosomal abnormalities in Philadelphia negative cells in CML patients after treated with tyrosine kinase inhibitors.

Emergence of clonal chromosomal abnormalities in Philadelphia chromosome-negative (CCA/Ph-) cells in chronic myeloid leukemia (CML) patients during the treatment with tyrosine kinase inhibitors (TKIs) is an interesting phenomenon. Although previous studies revealed some potential impact of CCA/Ph- on CML patients' outcome, clinical significance of CCA/Ph- in CML patients remains to be further elucidated. We retrospectively reviewed the patients with CML evaluated at Genoptix Medical Laboratory in Carlsbad, California from 2005 to 2015. Twenty-four CML patients with CCA/Ph- cells were identifie…
