Utilizing a Physician Scribe in a Pediatric Plastic Surgical Practice: A Time-driven Activity-based Costing Study.

BACKGROUND: To use time-driven activity-based costing methodology to compare the costs of routine pediatric plastic surgical patient visits with and without a physician scribe. METHODS: Pediatric plastic surgical clinic visits at a tertiary care facility with the following diagnoses were studied: skin lacerations, skin lesions, and plagiocephaly. Two plastic surgeons saw patients individually either with or without a scribe over a 10-month period. The time that the scribe and physician spent on the patient was recorded, including the duration of the clinic visit and time spent creating, dicta…

The fate of facial asymmetry after surgery for "muscular torticollis" in early childhood.

AIMS AND OBJECTIVES: To study wheather the facial features return to normal after surgery for muscular torticollis done in early childhood. MATERIALS AND METHODS: This is a long-term study of the fate of facial asymmetry in four children who have undergone operation for muscular torticollis in early childhood. All the patients presented late, i.e., after the age of 4 years with a scarred sternomastoid and plagiocephaly, so conservative management with physiotherapy was not considered. All the patients had an x-ray of cervical spine and eye and dental checkup before making a diagnosis of muscu…

Sudden infant death syndrome.

Sudden infant death syndrome is the leading cause of death among healthy infants, affecting 0.57 per 1,000 live births. The most easily modi … e. Supine sleeping position has increased the incidence of flattening of the occiput (deformational plagiocephaly), but this condition can be prevented and treated by encouraging supervised "tummy time," meaning that when awake, infants should spend as much time as possible on their stomachs. All apparent deaths from sudden infant death syndrome should be caref …

Are infants with torticollis at risk of a delay in early motor milestones compared with a control group of healthy infants?

Recently it has been claimed that infants with congenital muscular torticollis (CMT) are at risk of a delay in early motor milestones. The aim of the present study was to investigate whether infants with CMT are indeed at risk in comparison with a control group of healthy infants. A second aim was to investigate whether the time spent in a prone position and plagiocephaly had any influence on motor development. Eighty-two infants with CMT (35 females and 47 males) were compared with 40 healthy infants (18 females and 22 males). Motor development was assessed with the Alberta Infant Motor scale…

Unoperated craniosynostosis patients: correction in adulthood.

BACKGROUND: The purpose of this project was to determine whether adult patients with frontal deformities caused by untreated craniosynostosis can undergo successful surgical correction. METHODS: Thirteen unoperated adult craniosynostosis patients are reported: seven women and six men, with a mean age of 24 years. There were four plagiocephalies, four scaphocephalies, three oxycephalies, and two brachycephalies. Computed tomographic scans with cerebral contrast were obtained, and the projected computerized correction was discussed with the patients. Frontocranial remodeling with an intracrania

[On the effect of mutations of the fibroblast growth factor receptors as exemplified by three cases of craniosynostoses].

PURPOSE: Craniosynostoses are premature ossifications of cranial sutures. They occur isolated and syndromic. Syndromic craniosynostoses are … at the time of surgery to correct the craniosynostosis. RESULTS: In a girl with seemingly isolated plagiocephaly we identified a P250L (749C-->T) mutation in FGFR3. Her mother showed minor signs of craniosynostosis when the family was re-evaluated. She was shown to carry the same mutation. In two patients with suspected Crouzon's syndrome 2 different mutations …

New insights into craniosynostosis.

Craniosynostosis is a congenital developmental disorder involving premature fusion of cranial sutures, often associated with multiple neurol … d other less common syndromes also are discussed. The differences between positional and synostotic plagiocephaly are important, as well as the cause of nonsyndromic craniosynostosis. The prognosis and neurological outcome of patients, including "benign" forms of craniosynostosis, are other important aspects. Major advances have occurred in understanding pathog …

[Cephalometric studies on X-ray pictures to evaluate postoperative cranial growth of children with craniosynostoses].

OBJECTIVE: A preoperative CT scan seems indispensible for the evaluation of the bones and soft tissue in children with craniosynostoses. Regular postoperative follow-up is limited because of general anaesthesia and radiation exposure. Therefore, cranial growth of these children has hardly been examined so far. A method is needed to replace CT for this indication and to allow differentiated growth analysis besides clinical inspection. METHODS: 15 children (7 trigonocephalies, 4 plagiocephalies, 4 syndromal craniosynostoses) were operated on using a standardized bilateral fronto-orbital advance

Early radical treatment of pancraniofacial synostosis.

Premature fusion of all major cranial and facial sutures, termed pancraniofacial synostosis by us, was first described as "Kleeblattschädel deformity," but can also be found in anomalies such as Crouzon's disease, Apert's syndrome, Pfeiffer's syndrome, and severe forms of plagiocephaly. This rare craniofacial anomaly often presents an immediate threat to life right after birth. Early subtotal craniectomy frequently combined with frontoorbitofacial advancement must be performed to decrease intracranial pressure, preserve vision, and open up the blocked upper airway. Concomitant hydrocephaly is …

Infantile idiopathic scoliosis. Causative factors, particularly in the first six months of life.

An attempt has been made to determine the aetiological factors in infantile idiopathic scoliosis from a clinical, genetic and epidemiological survey of 134 infants, ninety-seven of whom developed a curve in the first six months of life. Plagiocephaly was present in all cases; mental retardation occurred in 13 per cent of males with progressive scoliosis; congenital dislocation of the hip occurred in 3-5 per cent of cases and congenital heart disease in 2-5 per cent; and inguinal hernia was found in 7-4 per cent of males. Approximately 3 per cent of parents and 3 per cent of sibs had the same d…

Two-Staged Cranial Reconstruction: Tissue Expansion, Galeocapsular Flaps, and CAD/CAM Three-Dimensional Titanium Implants.

Reconstruction of cranial defects after prior surgery remains aesthetically and technically challenging, particularly when osseous deficiency is accompanied by limited soft-tissue coverage. Recent advances in computer-aided design/computer-aided manufacturing have expanded the role of patient-specific implants in late cranial reconstruction and provided alternatives in cases in which revision osteotomy or repeat cranioplasty is not suitable. A 19-year-old male with residual cranial deformity after fronto-orbital advancement for unilateral anterior plagiocephaly underwent a 2-stage procedure. I…

Optimal Period for Surgery for Congenital Muscular Torticollis Following the Failure of Physiotherapy.

Congenital muscular torticollis is the third most common cause of orthopedic deformity. It is a risk factor for plagiocephaly. Physiotherapy is the preferred treatment. However, if it fails, surgery is recommended. This study examined the surgical results of congenital muscular torticollis in association with age. A retrospective descriptive study of patients who underwent surgery (tenotomy of the sternocleidomastoid muscle) for congenital muscular torticollis at a single center between 2000 and 2022 was, accordingly, performed. Clinical results (excellent, good, or poor) were examined in the …
