Total nasal reconstruction: a review of the past and present, with a peak into the future.

PURPOSE OF REVIEW: The goal of this article is to review the complex topic of total nasal reconstruction and present a wide range of options for completing this difficult surgical procedure. RECENT FINDINGS: Nasal reconstruction has a long history dating back thousands of years. Some historical techniques still exist today, including the paramedian forehead flap. The introduction of free tissue transfer and other pedicled flaps has provided multiple options for the reconstructive surgeon. The future of nasal reconstruction will include facial transplantation and likely bioengineered tissues.

Complications in Reductive Profileplasty.

The ability of a surgeon to create an aesthetically pleasing and functionally competent nasal dorsum weighs heavily on following the foundations of rhinoplasty, beginning with the examination and analysis. These same fundamental principles should be used and applied in secondary rhinoplasty. The most common chief complaints of those patients seeking primary rhinoplasty relates to the presence of a hump or dorsal convexity, and similarly complications surrounding the dorsum are the most common indications for secondary rhinoplasty. The surgeon must be able to appropriately evaluate and correct

Proof-of-concept prototype drill-guide for use in medial patello-femoral ligament (MPFL) reconstruction surgery.

For surgical reconstruction of the medial patello-femoral ligament (MPFL) a variety of techniques are used for fixation of the graft to the medial border of the patella. The bone bridge or V-shaped tunnel technique utilises two tunnels drilled from the medial aspect of the patella that converge centrally creating a tunnel through which the graft is threaded. This technique has advantages: it avoids hardware (bone anchors) and their associated complications, creates a broad attachment of the ligament approximating normal anatomy and the tunnel does not breach the lateral cortex of the patella r

Treatment of Common Congenital Hand Conditions.

Congenital hand difference is caused by abnormal embryonic development of the limb and represents one of the most prevalent congenital birth defects worldwide. Using the new classification system proposed by Oberg, Manske and Tonkin (OMT) and endorsed by the International Federation of Societies for Surgery of the Hand, congenital hand differences are classified into malformations, deformations, and dysplasias and syndromes. Malformations are subdivided into abnormal development of proximal-distal, radial-ulnar (anterior-posterior), dorsal-ventral, and unspecified axis. We introduce here the s

Umbilical reconstruction: different techniques, a single aim.

The umbilicus is a unique physiologic scar of human life resulting from the healing process of the cut umbilical cord at birth. Its absence leads to an unnatural abdominal appearance, and an abnormally shaped or misplaced umbilicus may draw undue attention to the central abdomen. Loss of the umbilicus can be an embarrassing deformity; this occurs when older techniques of umbilical hernia or incisional hernia repair are employed and after abdominoplasty, urachal cyst repair, omphalocele repair, gastroschisis repair, some tumor excisions, and mobilization of bipedicled or bilateral TRAM/DIEP fla

Management of Skull Fractures and Calvarial Defects.

Scalp and calvarial defects can result from a myriad of causes including but not limited to trauma, infection, congenital malformations, neoplasm, and surgical management of tumors or other pathologies. While some small, nondisplaced fractures with minimal overlying skin injury can be managed conservatively, more extensive wounds will need surgical repair and closure. There are many autologous and alloplastic materials to aid in dural and calvarial reconstruction, but no ideal reconstructive method has yet emerged. Different reconstructive materials and methods are associated with different ad

Arm Contouring in the Massive-Weight-Loss Patient.

Brachioplasty is an increasingly popular procedure performed for improved arm contour in the massive-weight-loss population. There are challenging deformities presented in this population, such as redundant skin, posterior arm lipodystrophy, and loosening of fascial layers of the upper arm and chest wall that must be addressed to achieve successful contour of the arms. Common complications can be minimized with meticulous technique and knowledge of surgical anatomy. Additionally, brachioplasty can be combined with liposuction of the posterior arm as a safe and effective method for arm contouri

Recent evolutions in flexor tendon repairs and rehabilitation.

This article reviews some recent advancements in repair and rehabilitation of the flexor tendons. These include placing sparse or no peripheral suture when the core suture is strong and sufficiently tensioned, allowing the repair site to be slightly bulky, aggressively releasing the pulleys (including the entire A2 pulley or both the A3 and A4 pulleys when necessary), placing a shorter splint with less restricted wrist positioning, and allowing out-of-splint active motion. The reported outcomes have been favourable with few or no repair ruptures and no function-disturbing tendon bowstringing.

Advantages of a Beveled Osteotomy on the Zygomatic Arch During Reduction Malarplasty.

During a conventional reduction malarplasty procedure, a dual approach including intraoral incisions and preauricular incisions is made to reduce both the zygomatic body and arch. As the preauricular approach is performed to cut the zygomatic arch, there are several remarkable advantages that can be achieved by simply beveling the osteotomy anteriorly on the zygomatic arch. This has the benefits of enhancing bone union by the increase in cross-sectional area for bone contact, decreasing palpability on the osteotomy site, placing the osteotomy more posteriorly, preventing depression in the ante

Microsurgical thumb repair and reconstruction.

In this article, we review microsurgical reconstructive techniques available to treat thumb amputation at different levels based on our experience. We reference techniques used by other surgeons and identify the most suitable technique for different clinical situations. Indications and techniques for microsurgical partial or composite transfer of the great or second toe for thumb reconstruction are summarized. Different microsurgical transfer techniques suggest a great freedom of surgical choices. However, the choices are considerably restricted if all functional and cosmetic requirements are

SURGICAL CORRECTION OF LABIA MINORA HYPERTROPHY, A PERSONAL TECHNIQUE.

BACKGROUND: Labia minora hypertrophy is a congenital or acquired condition in which both labia minora (or more rarely only one) protrude beyond the edge of the labia majora. The authors present a surgical technique of volumetric reduction of hypertrophic labia minora, associated with lipofilling of the labia majora. METHODS: Between 2005 and 2014, 27 patients underwent surgical reduction of labia minora, as described by Altier and Rouzier. The indications for surgical treatment varied and were as follows: interference with sexual intercourse; poor hygiene; difficulty wearing tight-fitting pan

[URETERAL TISSUE ENGINEERING: CHALLENGES AND PROSPECTS].

A broad range of pathologic conditions of the ureter (strictures, obliterations, fistulas, and so on) requiring reconstructive plastic surgery is a challenging urological problem. A variety of approaches to solve the problem indicates the need of searching for new opportunities. A new direction in reconstructive surgery of the ureter is the tissue engineering. Tissue engineering involves the usage of matrices and cells. The matrices can be used both with cultured cells, and without them. This review represents the results of preclinical studies on feasibility of tissue engineering using as a m
