A solitary papilloma of the left main bronchus in a 48 year old man is described. The patient had a one year history of cough, hemoptysis and bronchospasm. Bronchoscopy and repeated removal of the tumour through the bronchoscope failed to control it, and local recurrence finally made pneumonectomy necessary.

In a 22-year period from 1962 to 1984, 51 patients with malignant lung disease had a sleeve resection performed. In 33% of the patients, pneumonectomy was contraindicated because of limited lung function. The operative mortality was 8%. Six per cent of the patients developed complications after the operation. The 5-year survival of the total group of patients was 30%. Between August 1983 and October 1990, 42 patients with resistant Mycobacterium tuberculosis underwent 44 pulmonary resections. Forty-six patients with bronchial carcinoid tumors were operated on over a 37-year period. The results were reviewed with special reference to presenting complaint, histological diagnosis, location of the tumor, lymphatic involvement, and type of surgical resection. Age at operation ranged from 9 to 86 years (mean, 43.6 years). Presenting symptoms were hemoptysis in 21 instances, chronic cough in 17, and pneumonia in 15. The case of a 10-year-old girl with plasma cell granuloma of the lung and an associated hypergammaglobulinemia is reported. A thoracotomy and biopsy were done. Total resection would have required a pneumonectomy. The authors describe a case of malignant neoplasm of the mediastinum affecting the S.V.C. and the right lung. Radical removal of the neoplasm involved a right pneumonectomy and resection of the S.V.C. Replacement of the latter with a prosthesis in K Dacron double velour was successful--a success which has lasted 5 months to date.

Right-to-left interatrial shunt (RLIAS) resulting in platypnea orthodeoxia after a right pneumonectomy is an infrequent postoperative complication. Percutaneous device closure or surgical closures using cardiopulmonary bypass have been the standard interventions for RLIAS correction. We describe a technique using a tissue expander to correct a RLIAS.

Three patients are presented who had had a chronic cough for several months, a normal physical examination and a negative chest X-ray. Fiberoptic bronchoscopic examination revealed ulcerohaemorrhagic and/or stenotic lesions which proved to be of tuberculous origin. Bronchial artery embolization was attempted several times in a 74-year-old man undergoing right pneumonectomy for persistent hemoptysis. Paraplegia developed after surgery. Both thoracotomy for lung surgery and bronchial artery embolization have been reported to cause spinal cord ischemia. We analyze the possible mechanisms through which such ischemia may develop.

Following right-sided pneumonectomy and hemidiaphragm resection in a 58-year-old man with epithelioid mesothelioma, acute respiratory insufficiency and life-threatening circulatory collapse developed after a forced Valsalva maneuver. Intubation of the left main bronchus via a tracheostomy tube was performed in a patient with local recurrence of lung cancer associated with invasion and obstruction of the left main bronchus after right sleeve pneumonectomy. The result was satisfactory not only for preventing asphyxia, but also for maintaining the patency of the airway after extubation of the endotracheal tube.

A case of congenital lobar emphysema associated with pulmonary sequestration, presenting with respiratory distress in infancy, is reported. The lobar hyperinflation was managed by emergency lobectomy. The ipsilateral lower lobe affected with vascular sequestration was salvaged by therapeutic embolization, thus avoiding the long-term sequelae of pneumonectomy. The case is reported for its rarity.
