A patient was a 67-year-old female, 153 cm tall and weighing 47 kg. In 1988, she noticed sudden hypotonicity of her extremities, which led her to visit our hospital. Diagnosis of polymyositis (PM) was made. Treatment started with prednisolone 60 mg.day-1, followed by 2.5-5mg.day-1 as a maintenance dose. Her clinical symptoms were alleviated. We describe a case of a verrucous non-bacterial endocarditis on the native mitral valve in a patient with polymyositis. This case, to our knowledge, represents the first report in the literature we could get. The purpose of this article is to illustrate with a case study the process through which a patient with polymyositis taking part in an occupational therapy program achieved functional improvement in spite of persistent muscle weakness. Although the focus is on the occupational therapy program, close monitoring of patient improvement or regression by the total rehabilitation team was essential in order to adjust the patient's weekly activity level for maximum benefit.

OBJECTIVE: To summarize the association of cancer with adult polymyositis (PM) and dermatomyositis (DM) and to test whether myositis precedes or follows cancer diagnosis, with the latter suggesting that myositis is paraneoplastic.
METHODS: Using primary patient data, we performed an analysis of all published case control and cohort studies evaluating the association of myositis and malignancy. A 54-year-old man with renal cell carcinoma was treated with interferon (IFN)-gamma for 3 weeks soon after nephrectomy. Three months later he received IFN-alpha therapy for 8 weeks due to chronic active hepatitis C. He subsequently contracted polymyositis (PM): proximal muscle weakness, an elevation of muscle enzymes, myogenic patterns on the electromyograph and histologically specific findings in biopsied muscle specimens. After discontinuation of IFN his muscular weakness gradually recovered.

The case of a man with acute onset of muscle pain, weakness, anasarca, severe dysphagia and dysphonia, and biochemical, electromyographic and histologic evidence of polymyositis is presented. The literature on the occurrence of subcutaneous edema in polymyositis was reviewed. It is concluded that this particular symptom, with no other apparent cause, including heart failure from the underlying disease, is a rare but definite feature of polymyositis itself. A correlation of that with severe bulbar muscle involvement is also suggested.

Endomyocardial biopsy pathology in a patient with dermatomyositis cardiac involvement is reported. Clinical findings included acute heart failure and ischemic and conduction changes on electrocardiogram. Light microscopy showed focal myocyte necrosis without inflammation or fibrosis and prominent myocyte vacuolization. An immunohistochemical study was conducted using the biotin avidin technique in 22 muscle specimens from PM/DM patients and 19 control subjects. Deposition of IgG, IgM or C3 in the wall of capillaries and small vessels was demonstrated in 72.7% of PM/DM patients and in 31.6% of those with other neuromuscular disorders. This finding supports the contention that immunoglobulins and complement-mediated immunoreaction may play an important role in PM/DM pathogenesis.

Polymyositis is an inflammatory muscle disease preferentially affecting proximal muscles. Diagnosis is suggested by an EMG study showing abnormal spontaneous activity in proximal muscles and the presence of myopathic motor unit changes. Alternative diagnoses are eliminated by nerve conduction and neuromuscular junction studies and the EMG. Diagnosis is confirmed by muscle biopsy. We present a case of polymyositis (PM) in a patient with no previous relevant medical history, who had received blood transfusion four years ago during surgery. It could be demonstrated that the patient was HIV infected inspite the fact that he did not belong to any other high risk groups. PM was the clinical presentation form of HIV infection, the patient dying five months later due to a Pneumocystis carinii. The main clinical and histologic characteristics of PM in HIV infected patients are described as well as the main myopathies that these patients present.

Candida sepsis in 18-year-old man with acute lymphoblastic leukemia was associated with multisystem alteration accentuated in cardiac and skeletal muscles. Candida tropicalis identified in the post-mortem blood culture was resistant to azole-chemotherapeutics and semi-sensitive to polyene-antibiotics. Mice inoculated intracerebrally with parent, large-plaque (LP) and small-plaque (SP) strains of Kanagawa strain of Getah virus showed clinically recumbency and paralysis.
