Cauda Equina Syndrome: Poor Recovery Prognosis Despite Early Treatment.

STUDY DESIGN: A prospective patient's database operated on a cauda equina syndrome (CES). OBJECTIVE: The aim of our study was to identify prognosis factors for favorable functional recovery after CES. SUMMARY OF BACKGROUND DATA: CES is a neurologic impairment of variable symptoms associating urinary, bowel, and sexual dysfunctions with or without motor or sensitive deficits caused by nerve root compression of the cauda equina. The definition of CES remains debated, as well as the prognosis factors for favorable functional recovery and the benefit of early surgery. METHODS: One hundred forty

Innervation and function of rat tail muscles for modeling cauda equina injury and repair.

INTRODUCTION: The rat tail exhibits functional impairment after cauda equina injury. Our goal was to better understand the innervation and roles of muscles that control the tail. METHODS: Adult rats received either: (1) ventral root injury; (2) caudales nerve injury; or (3) mapping of sacrococcygeal myotomes. Activation of small muscles within the tail itself (intrinsics) was compared with that of larger lumbosacral muscles acting on the tail (extrinsics). Behavioral testing of tail movement was done 1 week later. RESULTS: Rats that received ventral root injury exhibited multiple behavioral

Presentation of cauda equina syndrome due to an intradural extramedullary abscess: a case report.

BACKGROUND CONTEXT: Cauda equina syndrome is caused by compression or injury to the nerve roots distal to the level of the spinal cord. This syndrome presents as low back pain, motor and sensory deficits in the lower extremities, and bladder as well as bowel dysfunction. Although various etiologies of cauda equina syndrome have been reported, a less common cause is infection. PURPOSE: To report a case of cauda equina syndrome caused by infection of an intradural extramedullary abscess with Staphylococcus aureus. STUDY DESIGN/SETTING: Case report and review of the literature. METHODS: The li

[Brachioradial pruritus effectively treated with gabapentin].

Brachioradial pruritus is a characteristic clinical condition with a deep intense itching, tingling or burning sensation localized to the dorsolateral part of the upper extremities. The symptoms occur primarily in the late summer in fair skin type people who are extensively sun-exposed. The condition is difficult to treat and does not respond to topical steroids and antihistamines. We describe a 57-year-old woman with longstanding brachioradial itching due to cervical radiculopathy and sun exposure, which was successfully treated with gabapentin.

Low back pain and physiotherapy use of red flags: the evidence from Scotland.

BACKGROUND: Red flags are recognised as indicators of possible serious spinal pathology, and their use is indicated by numerous guidelines. Similar to other countries worldwide, Scotland lacked a national view about the overall quality of the physiotherapy management of low back pain and the use of red flags. Anecdotal evidence suggested that practice varied considerably. AIM: To improve the use and documentation of red flags by physiotherapists during the assessment and management of low back pain. DESIGN: Prospective, multicentred, national data collection and improvement initiative. SETT

Spinal extradural angiolipoma: report of two cases and review of the literature.

Spinal angiolipomas are benign uncommon neoplasm composed of mature lipocytes admixed with abnormal blood vessels. They account for only 0.04-1.2% of all spinal tumors. We report two cases of lumbar extradural angiolipoma and review previously reported cases. We found 118 cases of spinal epidural angiolipoma (70 females and 48 males; age range 1.5-85 years, mean 44.03) spanning from 1890 to 2006. Prior to diagnosis 40.6% of the patients had weakness of the lower limbs. The interval between the initial symptoms and tumor diagnosis ranged from 1 day to 17 years (mean 20.2 months). Except for fou

Cauda equina syndrome: an anatomically driven review.

Cauda equina syndrome results from an injury to the lumbosacral nerve roots below the tip of the conus medullaris, occuring in between 2 and 6% of all laminectomies performed for lumbar disc herniation. This article relates the anatomy of the nervous system of the bladder, rectum, anus and sexual organs to the signs and symptoms of cauda equina syndrome, and reviews the literature for the acute management of these patients.

Intraneural cavernous malformation of the cauda equina.

Spinal cavernous angiomas are rare vascular malformations occurring mainly in the vertebral body with or without an extradural extension. Only 3-5% of these lesions are entirely located in the spinal canal where they can occupy an extradural, intradural-extramedullary or intramedullary position. We present a 75-year-old woman with signs and symptoms of multiple lumbar radiculopathy. The lumbosacral MRI showed an intradural cauda equina lesion with heterogeneous contrast enhancement that was subsequently radically removed through an L3-L4 laminectomy. The microscopic appearance was suggestive o

Bilateral vs. unilateral electromyographic examination of the external anal sphincter muscle.

AIM OF THE STUDY: In patients with cauda equina or conus medullaris lesions, bilateral electromyographic (EMG) examination of the subcutaneous external anal sphincter (EAS) muscles has been suggested. In spite of its circular shape, EAS should be considered as two separate semicircular muscles. The aim of the present study was to test the hypothesis that information obtained by bilateral examination of the EAS muscle outweighs inconvenience due to additional needle insertions. PATIENTS AND METHODS: A group of 67 patients with clinical and radiological data supportive of cauda equina or conus

Repair of multiple cervical root avulsion with sural nerve graft.

To obtain easier access to avulsed roots in the intradural space for patients suffering cervical root avulsion, the authors of this study developed a novel repair method. This involves using nerve grafts to bridge corresponding segments of the spinal cord and the trunk or cord level of the plexus, respectively, in two surgical stages. All eight patients admitted to this study received pre- and post-operative workups of electrophysiological evaluations and muscle power grading through Medical Research Council (MRC) scores. The degrees of impairment were also graded according to a modified versi

Microsurgical DREZotomy for pain of spinal cord and Cauda equina injury origin: clinical characteristics of pain and implications for surgery in a series of 26 patients.

The result of the DREZotomy procedure used for the treatment of chronic intractable neuropathic pain caused by injuries at the T9-L4 spine level in 26 patients has been reported. For the purpose of identifying the most favorable pain pattern for DREZ surgery we retrospectively analyzed the effectiveness of surgical treatment on different forms of pain in the follow-up period of 13-50 months, 37 months on average. All pain forms were classified according to subjective sensory pain expression including the rhythm and topography of the pain. Three groups of pain were formed according to subjectiv

"EMG disease" with bulbar muscle involvement: a case report.

We report the first case of diffuse abnormal insertional activity with bulbar muscle involvement. Electromyography performed 5 months earlier reported multilevel radiculopathy. A repeat electromyography study revealed short trains of positive waves without fibrillation potentials, diffusely present in all tested muscles. Positive waves were also found in the bulbar innervated muscles; these included the trapezius, frontalis, and the orbicularis oculi. This entity is important to recognize because confusion with neuromuscular syndromes can occur.

Reinnervation of anal sphincter muscle by low and high-threshold motor neurones.

The external anal sphincter (EAS) has continuously active low-threshold and recruitable high-threshold motor units (MUs), the latter being 'larger'. On performing concentric needle electromyography (EMG) of the EAS, the high-threshold MUs seemed to reveal more neuropathic changes than the low-threshold MUs. To verify this hypothesis, low- and high-threshold motor unit potentials (MUPs) were compared in patients with neuropathic EAS and controls. Fifteen subjects without pelvic disorders and 29 patients with sequela after cauda equina lesions were studied. In patients, only muscles ipsilateral
