Sleep health profiles across six population-based cohorts and recommendations for validating clustering models.

OBJECTIVES: Model-based clustering is increasingly used to identify multidimensional sleep health profiles. However, generalizability is rarely assessed because of complexities of data sharing and harmonization. Our goal was to evaluate the generalizability of multidimensional sleep health profiles across older adult populations in Western countries and assess whether they predict depressive symptoms over time. METHODS: We harmonized five self-reported sleep health indicators (satisfaction, alertness, timing, efficiency, and duration) across six population-based cohorts from the United States

Women's Health Disorders in a Coeliac Disease Population After Diagnosis-A Nationwide Cohort Analysis.

BACKGROUND: There is a female predominance of diagnosed coeliac disease with sex-related differences in clinical presentation. Delayed menarche, infertility and pregnancy complications have been linked to poor nutritional status and autoimmune mechanisms, but women's health data in coeliac disease are scant and contradictory. AIM: To describe rates of women's health disorders in US patients with coeliac disease. METHODS: We used TriNetX, a database of 80 healthcare organisations, for a retrospective observational analysis. Coeliac disease was identified using ICD-10 code (K90.0) and positive

Long-term exposure to ambient PM2.5 and population health: evidence from linked census data.

Particulate matter suspended in the air that is comprised of microscopic particles with a diameter of 2.5μm or less (PM2.5) is among the most impactful pollutants globally. Extensive evidence shows exposure to ambient PM2.5 is associated with a wide range of poor health outcomes. However, few studies examine long-run pollution exposures in nationally representative data. This study exploits Census data for Northern Ireland, linked to average PM2.5 concentrations at the 1x1km grid-square level during the period 2002-2010. We combine outcome measures in 2011 with data on complete residential his

Polypharmacy in primary care: A population-based retrospective cohort study of electronic health records.

BACKGROUND: Polypharmacy, prescription of multiple medications to a patient, is a major challenge for health systems. There have been no peer-reviewed studies of polypharmacy prevalence and medication cost at a population level in England. AIMS: To determine prevalence and medication cost of polypharmacy, by patient characteristics. Design and setting: Retrospective cohort study of North West London electronic health records. METHOD: We quantified prevalence and direct cost of polypharmacy (five or more regular medications), stratified by demographics and frailty. We fitted a mixed-effects l

Health care utilisation following childhood acute lymphoblastic leukaemia: a population-based matched cohort study.

OBJECTIVES: Survival among children diagnosed with acute lymphoblastic leukaemia (ALL) has increased considerably. However, morbidity in survivors constitutes a potential increasing burden not limited to secondary health care. Our objectives were to compare health care utilisation, including both primary and secondary health care, between childhood ALL survivors and matched references up to 15 years after curative treatment. Moreover, to increase knowledge on survivors' health service seeking behaviour as time from treatment elapsed. DESIGN AND SETTING: A Danish population-based matched cohor

Liver health literacy and social stigma of liver disease: A general population e-survey.

BACKGROUND: Health literacy is increasingly recognized as an essential determinant for the health of the population. Liver patients report perceived stigma to be a considerable problem. Little is however known about liver health literacy in the general population and to what extent liver disease is considered stigmatizing in comparison with other chronic diseases. We aimed to explore these knowledge gaps. METHODS: We performed an exploratory e-survey in a statistically representative sample of 500 Swedes from the general population. A questionnaire developed for this purpose investigated awar

Use of sequence analysis for classifying individual antidepressant trajectories to monitor population mental health.

BACKGROUND: Over the past decade, antidepressant prescriptions have increased in European countries and the United States, partly due to an increase in the number of new cases of mental illness. This paper demonstrates an innovative approach to the classification of population level change in mental health status, using administrative data for a large sample of the Scottish population. We aimed to identify groups of individuals with similar patterns of change in pattern of prescribing, validate these groups by comparison with other indicators of mental illness, and characterise the population

Oncology Clinic-Based Hereditary Cancer Genetic Testing in a Population-Based Health Care System.

New streamlined models for genetic counseling and genetic testing have recently been developed in response to increasing demand for cancer genetic services. To improve access and decrease wait times, we implemented an oncology clinic-based genetic testing model for breast and ovarian cancer patients in a publicly funded population-based health care setting in British Columbia, Canada. This observational study evaluated the oncology clinic-based model as compared to a traditional one-on-one approach with a genetic counsellor using a multi-gene panel testing approach. The primary objectives were

[Sleep-related problems and associated factors among the Brazilian population: National Health Survey, 2013].

The scope of this study was to estimate the frequency of sleep-related problems and associated factors. It was a study based on data from the National Health Survey conducted in Brazil in 2013. Sleep-related problems were assessed with the question "How often have you had sleep problems in the last two weeks, such as difficulty falling asleep, waking up frequently during the night or sleeping more than usual?" The answers were grouped into: none of the days, less than half of the days and more than half of the days. Of the individuals evaluated, 71.1% reported having no problems with sleep on

Post-war (1946-2017) population health change in the United Kingdom: A systematic review.

We systematically reviewed the evidence on secular trends in main chronic conditions, disability and self-assessed general health among adults in the United Kingdom, as reported in primary/secondary care databases and population-based surveys. Searches were conducted separately for: (1) trends in age-standardised or age-specific prevalence of major non-communicable diseases, disability, and self-reported general health; (2) trends in health expectancy. The databases searched were MEDLINE, EMBASE/EMBASE Classic and Web of Science (all from 1946/7). The evidence was synthesised narratively. Ther

Health care utilization prior to out-of-hospital cardiac arrest: A population-based study.

INTRODUCTION: Although out-of-hospital cardiac arrest (OHCA) is thought of as a sudden event, recent studies suggest that many patients have symptoms or have sought medical attention prior to their arrest. Our objective was to evaluate patterns of healthcare utilization before OHCA. METHODS: We conducted a population-based cohort study in Ontario, Canada, which included all patients ≥20 years, who suffered out-of-hospital cardiac arrest and transferred to an emergency department (ED) from 2007 to 2018. Measurements included emergency room assessments, hospitalizations and physician visits pri

Health Literacy Among a Formerly Incarcerated Population Using Data from the Transitions Clinic Network.

Health literacy is increasingly understood to be a mediator of chronic disease self-management and health care utilization. However, there has been very little research examining health literacy among incarcerated persons. This study aimed to describe the health literacy and relevant patient characteristics in a recently incarcerated primary care patient population in 12 communities in 6 states and Puerto Rico. Baseline data were collected from 751 individuals through the national Transitions Clinic Network (TCN), a model which utilizes a community health worker (CHW) with a previous history o

Population based report on health related quality of life in adolescents born very preterm.

BACKGROUND: As the survival rate of preterm infants constantly improves, knowledge on the impact of prematurity on long-term health-related quality of life (HRQoL) is important for clinical and parental guidance. We aimed to assess HRQoL in a national cohort of young adolescents born very preterm, and to identify predictors for poorer HRQoL. PATIENTS AND METHODS: All surviving Swiss live-born children below 30weeks of gestation during the year 2000 (290 subjects) were contacted at age 12years, together with their parents (262 families). HRQoL of the study children was assessed using both the
