Towards implementation of precision medicine biomarkers in early detection and prognostication of prostate cancer.

Prostate cancer is the second-highest cause of cancer-related incidence and the fifth-highest cause of cancer mortality in males. Prostate cancer is a heterogeneous disease with a wide spectrum of clinical behaviour, ranging from indolent to highly aggressive. Molecular approaches, such as genomic testing, can augment existing clinical risk stratifications and tailor management to the individual. Genomic tests that sample biopsy or surgical tissue can provide a molecular risk assessment and identify actionable therapy targets. Liquid biopsy, while still emerging, may provide a non-invasive alt

Conversion and Neoadjuvant Therapy for Intrahepatic Cholangiocarcinoma: Integrating Precision Medicine to Redefine Surgical Curability.

Intrahepatic cholangiocarcinoma (ICC) is a type of primary liver cancer that is highly aggressive. In recent years, its incidence has continued to increase around the globe, yet outcomes remain poor and stagnant. Although surgical resection is the only treatment option capable of curing the cancer, most patients present with either advanced disease or unresectable tumors. Over the past decade, our understanding of ICC biology has evolved rapidly, with improved molecular classification and the clinical introduction of targeted therapies and immunotherapies. As a result of these advances, neoadj

New understanding and treatment of adult mitochondrial dysfunction: an example of personalized precision medicine.

The principal cellular energy-generating pathways of mitochondria used to produce adenosine triphosphate (ATP) are oxidative phosphorylation and β-oxidation of fatty acids. Under anaerobic conditions, glycolysis in the cytoplasm is an alternative mechanism for production of ATP. Mitochondrial diseases result from one or more of the over 350 mutations in mitochondrial DNA (10%) or nuclear DNA (90%) that cause defective mitochondrial ATP production. The most common manifestations in adults with mitochondrial DNA mutations are diminished vision, myopathy, cardiomyopathy, neuropathy, encephalopath

Emerging biomarkers in breast fibroadenoma: implications for differential diagnosis, malignant transformation, and precision medicine.

Fibroadenoma is the most common benign fibroepithelial lesion of the breast; it occurs most frequently in adolescents and women of reproductive age. While fibroadenoma is considered a non-invasive lesion with a good prognosis, some fibroadenomas may show rapid growth, recurrence, stromal hypercellularity, epithelial proliferative changes and even show radiological overlap with phyllodes tumour and breast carcinoma resulting in significant diagnostic dilemmas. In recent years, molecular pathology and biomarker studies have led to better understanding of the biology of fibroadenomas, and their a

Towards Precision Medicine in Inflammatory Bowel Diseases: Pharmacogenetic Predictors of Ustekinumab Effectiveness and Persistence.

Background/Objectives: Inflammatory bowel diseases (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), show substantial variability in response to biologic therapies. Ustekinumab, which targets the IL-12/23 pathway, is an established treatment for moderate-to-severe IBD; however, a proportion of patients fail to achieve sustained remission or discontinue treatment over time. This study aimed to evaluate the effectiveness and persistence of ustekinumab and to explore the potential role of pharmacogenetic markers in predicting treatment outcomes. Methods: In this observational coh

Underutilization of syndrome-specific ICD-10 codes for genetic epilepsies: Implications for precision medicine.

Syndrome-specific International Classification of Diseases, 10th Revision (ICD-10) codes have the potential to improve identification of patients for precision therapies, clinical trials, and research, yet their real-world uptake is not well characterized. We evaluated the utilization of syndrome-specific ICD-10 codes at a large academic medical center among patients with pathogenic or likely pathogenic variants in 10 monogenic epilepsy genes with established codes (CDKL5, EHMT1, KCNQ2, MECP2, MED13L, SCN1A, SHANK3, SLC13A5, SLC2A1, SYNGAP1). Patients were identified from an institutional gene

Cutting-Edge Diagnostics and Nanotechnology-Enabled Precision Medicine in Hepatocellular Carcinoma: A Comprehensive Review.

INTRODUCTION: Hepatocellular carcinoma (HCC) is one of the largest health burdens in the world because of high mortality rates, low early diagnostic sensitivity, and poor response to conventional treatment. The existing diagnostic and treatment methods tend to be non-specific and linked with systemic toxicity and late cancer detection, which provokes the necessity of more accurate, precision-driven methods. METHODS: This review analyzes recent developments in HCC diagnosis and treatment, including new biomarkers, liquid biopsy, and nanotechnology-based therapeutic systems. We critically analy

From cellular heterogeneity to precision medicine: single-cell multi-omics in CNS disease research.

Single-cell sequencing and multi-omics technologies are revolutionizing research on central nervous system (CNS) diseases by enabling high-resolution analysis of cellular heterogeneity and molecular dynamics. Traditional technologies (e.g., bulk sequencing, routine histology) often lack cellular resolution, fail to capture heterogeneity among individual cells, and struggle to reveal subtle molecular changes in early pathogenesis, limiting their ability to clarify complex CNS disease mechanisms and develop precise diagnostic tools. This review comprehensively summarizes the latest advances in s

Paradigm Shift in Inflammatory Bowel Disease Management: Precision Medicine, Artificial Intelligence, and Emerging Therapies.

Inflammatory bowel disease (IBD) management stands at the cusp of a transformative era, with recent breakthroughs heralding a paradigm shift in treatment strategies. Traditionally, IBD therapeutics revolved around immunosuppressants, but the landscape has evolved significantly. Recent approvals of etrasimod, upadacitinib, mirikizumab, and risankizumab have introduced novel mechanisms of action, offering renewed hope for IBD patients. These medications represent a departure from the status quo, breaking years of therapeutic stagnation. Precision medicine, involving Artificial Intelligence, is a…

Distinct Longitudinal Clinical-Neuroanatomical Trajectories in Parkinson's Disease Clinical Subtypes: Insight toward Precision Medicine.

BACKGROUND: Parkinson's disease (PD) varies widely across individuals in clinical manifestations and course of progression. Identification of distinct biological subtypes could explain this heterogeneity, identify its pathophysiology, and predict disease progression. OBJECTIVES: Our aim was to compare longitudinal clinical trajectories and brain atrophy patterns between clinical subtypes defined at the baseline de novo PD. METHODS: We analyzed data from 421 PD patients (mean follow-up: 8.2 years) in the Parkinson's Progression Markers Initiative (PPMI). Using multi-domain motor and non-motor
